These services offer support and edu<strong>ca</strong>tion for families who have an adult member with a<br />
mental illness. Please note which programs and support groups listed below are designed<br />
specifi<strong>ca</strong>lly for family members and do not include the outpatient/consumer.<br />
Understanding <strong>Mental</strong> Illness - Seminar for Families<br />
This meeting takes place once a week in the evening. Speakers address groups of families on<br />
topics such as: <strong>ca</strong>re for <strong>ca</strong>re givers, Schizophrenia, Mood Disorders, coping with stress, grief,<br />
medi<strong>ca</strong>tion compliance, limit setting, boundaries, etc. This seminar does not include the<br />
outpatient/consumer.<br />
Just<br />
Between Us- Support group for Families<br />
This psycho-edu<strong>ca</strong>tional support group is led by a counsellor(s) and meets once a week in the<br />
evening. Families are encouraged to analyze and understand one another's challenges<br />
regarding mental illness in the family. This seminar does not include the outpatient/consumer.<br />
Specific Information and <strong>Resources</strong> for Your Family<br />
A counselor is available to meet with individual families or members to enhance their<br />
understanding of mental illnesses, community resources and advo<strong>ca</strong>cy specifi<strong>ca</strong>lly related to<br />
their particular needs. This seminar does not include the outpatient/consumer.<br />
Family Focus<br />
A counsellor is available to meet with all family members including the outpatient/consumer if<br />
requested, to enhance family coping skills.<br />
416-530-6486 x4283 The Geriatric <strong>Mental</strong> <strong>Health</strong> Outreach Team provides the following services for designated<br />
Long-Term Care Homes:<br />
-Assessment/consultation of residents 55 years and older with serious mental illness and/or<br />
severe behavioural issues<br />
-Early identifi<strong>ca</strong>tion of potential mental health crisis and/or <strong>ca</strong>re management issues<br />
-On-going follow-up, as required in collaboration with attending mental health physician and<br />
LTCH staff<br />
-<strong>Mental</strong> health edu<strong>ca</strong>tion and support for front line staff<br />
-Weekly visitation by a Registered Nurse<br />
-Monthly session by a Geriatric Psychiatrist<br />
For urgent or crisis mental health situations, established LTCH procedures will still remain in<br />
effect.<br />
