Public Edu<strong>ca</strong>tion Coordinators offer dementia specific edu<strong>ca</strong>tion to professionals who work<br />

with people who have dementia. Programs are flexible and geared toward the learning needs<br />

of different groups including personal support workers, activationists, nursing, dietary and<br />

housekeeping staff, students, volunteers, and visitors. Topics include an overview of AD,<br />

communi<strong>ca</strong>ting with people who have ADRD, responding to responsive behaviours, working<br />

with families, an aging simulation, activities of daily living, and therapeutic activities.<br />

Coordinators are trained in the Gentle Persuasive Approach, Advance Care Planning, and U-<br />

First. They <strong>ca</strong>n also modify and develop programs as requested.<br />

and expected outcomes. Our comprehensive social work program provides practi<strong>ca</strong>l support to<br />

<strong>ca</strong>regivers we are providing edu<strong>ca</strong>tion about the disease, coping techniques for the <strong>ca</strong>regivers<br />

and providing therapeutic counselling. The Alzheimer Society of York Region’s social workers<br />

play an integral role in assisting families in navigating throughout the course of the disease. The<br />

To become a client of the Bramalea Community <strong>Health</strong> Centre you must 1) live in the area<br />

served and 2) be a member of one or more of the following priority populations;<br />

Promote the mental health of all people and to service mental health consumers, their families<br />

and friends. The work of the organization focuses on combating mental health problems and<br />

emotional disorders. The tools include research and information services, sponsored research<br />

projects, workshops, seminars, pamphlets, newsletters and resource services. The organization<br />

runs programs that assist with employment, housing, early intervention for youth, peer support<br />

and recreation services for people with mental health issues. Stress reduction workshops and<br />

public edu<strong>ca</strong>tion <strong>ca</strong>mpaigns are scheduled for the community. In addition, CMHA acts as a<br />

social advo<strong>ca</strong>te to encourage public action and commitment to strengthening community<br />

<strong>Mental</strong> <strong>Health</strong> Diversion is defined as a pre-trial procedure where crown attorneys <strong>ca</strong>n, at their<br />

discretion, decide not to prosecute when the charge is a minor offense and major mental illness<br />

is determined as the underlying <strong>ca</strong>use. Instead, the accused is referred to appropriate mental<br />

health services in the community, professional treatment and support to reduce the chances of<br />

re-offending. Furthermore, if the individual meets specific criteria he or she may also qualify for<br />

short-term <strong>ca</strong>se-management. A Court Worker or Case Manager will work with the client to<br />

develop an individual service plan. The plan will outline the treatments and services that the<br />

individual will be expected to utilize.<br />

All referral for <strong>Mental</strong> <strong>Health</strong> Diversion must go through a 6-week screening process to<br />

These goals are achieved is by hosting discussion groups that are 1.5hrs in length with 8-12<br />

background to the program. Each one has a current police check. Facilitators serve as a bridge<br />

to youth-friendly resources, identified as youth-friendly by youth themselves. Youth Net<br />

The program supports individuals who are suffering from serious mental illness and who live or<br />

work in York Region. Individual, couple, family and group therapy are provided. The treatment<br />

of choice is based on the needs of the client. Model of service is Cognitive Behaviour Therapy<br />

(CBT) and Dialecti<strong>ca</strong>l Behaviour Therapy (DBT) and other models as required. All treatment<br />

models are recovery based. Average length of stay is four to six months.<br />

Provides opportunities for people with mental illness to develop life skills including social,<br />

interpersonal, leadership and employment. The program has several lo<strong>ca</strong>tions through York<br />

Region and South Simcoe., the Canadian <strong>Mental</strong> <strong>Health</strong> Association and its community partners<br />

hold a weekly social program <strong>ca</strong>lled VENT and a Youth Coffee Club. These groups offer peer<br />

discussion, movies, music, arts and crafts, games, awareness nights and supportive counselling.<br />

Model of service is recovery. Average length of stay is one to two years.<br />

Services are provided by a psychogeriatric nurse and/or a <strong>ca</strong>se manager. Seniors receive in-home<br />

assessment, clini<strong>ca</strong>l and supportive monitoring and primary <strong>ca</strong>re treatment services as well as access to<br />

additional community supports. Frequency of home visits are based on the senior’s health status but<br />

<strong>ca</strong>n be weekly, as needed.<br />

The Concurrent Disorders Service (CDS), part of Toronto’s Centre for Addiction and <strong>Mental</strong><br />

<strong>Health</strong> (CAMH), offers specialized community-based (outpatient) treatment to people facing<br />

both addiction and mental health problems. Specialized concurrent disorder services include:<br />

consultation, treatment planning and preparation, group therapy, individual therapy, <strong>ca</strong>se<br />

management, pharmacotherapy, treatment research, and edu<strong>ca</strong>tion and training.<br />

CDS offers more intensive outpatient treatment for specific subgroups of clients in the<br />

following 3 Concurrent Disorders Specialty Clinics:<br />

(1) Borderline Personality Disorder (BPD) Clinic for those with BPD with and without concurrent<br />

substance use problems,<br />

(2) Eating Disorders and Addictions Clinic,<br />

The Anxiety Disorders Clinic provides assessment, consultation and multi-model treatment<br />

services for adults with anxiety disorders.<br />

Assessments include combined structured diagnostic interviews and psychiatric consultation<br />

with the view toward offering expert pharmacotherapy recommendations.<br />

A second major treatment option offered through the clinic is cognitive-behavioural therapy.<br />

The Mood Disorders Clinic (MDC) provides multidisciplinary and multi-modal clini<strong>ca</strong>l <strong>ca</strong>re and<br />

edu<strong>ca</strong>tion for patients, families and professionals with diagnoses of depression or bipolar<br />

disorder. Services include psychiatric assessment, diagnosis, treatment recommendations and<br />

brief follow-up where appropriate. Where patients are referred back to the referring physician<br />

with recommendations, the clinic physicians will function as a back up and resource to the<br />

treating physician in the community and a repeat consultation <strong>ca</strong>n be requested. Involvement<br />

of non-medi<strong>ca</strong>l clini<strong>ca</strong>l staff in treatment plans is designed to best meet the needs of individual<br />

patients. If direct patient <strong>ca</strong>re, particular research interests include early intervention and<br />

exploring the gene/environment interaction.<br />

Provides group and limited one-to-one counselling for problem gamblers, and support for<br />

family members, who may be seen with or without the individual with the gambling problem.<br />

Counselling is available in person and by phone, and is generally solution-focused, using<br />

cognitive-behavioural interventions, relapse prevention and harm reduction strategies. The<br />

services include open-cycle Preparation groups, closed-cycle 8-week Skills for Change groups,<br />

open After<strong>ca</strong>re groups, and other groups designed for specific issues and concerns. Couple and<br />

family counselling is available. Some staff members specialize in specific populations: women,<br />

youth, older adults and ethnocultural communities. Staff also provide professional training,<br />

outreach and public information sessions on problem gambling issues, treatment and<br />

prevention. Problem gambling treatment services are provided in downtown Toronto at 33<br />

Russell Street, and at two satellite offices in S<strong>ca</strong>rborough and Etobicoke (limited service).<br />

Community-based outreach teams that provide a range of services to seniors. The clinics<br />

provide assessment and psychogeriatric consultation in the client's own environment. For<br />

Designed to promote wellness and recovery from substance abuse in a residential setting, our<br />

new state-of-the-art Withdrawal Management Services facility was opened on February 6,<br />

2006. Housing 26 beds for both male and female clients, Glendale House is the first such facility<br />

to open in the GTA since the early 1990's, and one of only two in the GTA providing withdrawal<br />

management to women. In addition, day withdrawal and withdrawal support in the community<br />

are also provided. Assessment, counseling and support are provided to hospital inpatients and<br />

outpatients through the Addiction Medicine Service.<br />

improve the client’s quality of life. Assertive outreach is provided to clients who have had<br />

being hospitalized. This program is for people who experience recurrent hospitalizations,<br />

benefit from treatment and usually do not engage in outpatient follow-up. Only physicians <strong>ca</strong>n<br />

initiate a community treatment order, which is limited to six months in duration.<br />

The CTO coordinator provides assistance with drafting the plan, ensuring Rights Advice and<br />

securing community services for the client. The CTO <strong>ca</strong>se management provided is for support<br />

(only for Mississauga residents), monitoring and developing linkages to community supports<br />

stability and reducing symptoms of mental illness. The ACTT team provides psychiatric,<br />

rehabilitative and support services including <strong>ca</strong>se management, crisis assessment and<br />

intervention, individual supportive therapy, vo<strong>ca</strong>tional assessment and job development, and<br />

other assistance.<br />

Information and Referral-<strong>Mental</strong> <strong>Health</strong>/<strong>Mental</strong> Illness<br />

CMHA/Peel offers a <strong>Mental</strong> <strong>Health</strong> Resource Centre, which serves as a welcoming point of entry to the mental health system for people living and/or working in<br />

Brampton, Caledon, and Mississauga. CMHA/Peel –Resource Center hours of operations is 9am to 4:30pm, Mondays to Fridays. CMHA/Peel will be able to help<br />

individuals from Etobicoke, Woodbridge and Dufferin by providing them with information of services and if need make referrals to services in there area.<br />

<strong>Mental</strong> <strong>Health</strong> is defined as knowing and accepting yourself, building meaningful relationships, coping with problems day to day living, and maintaining a sense of humor.<br />

It also means striking a balance in all aspects of your life: social, physi<strong>ca</strong>l, spiritual, economic and mental<br />

<strong>Mental</strong> Illness is a generic term for a variety of diagnosable illnesses, based on set criteria of symptoms. <strong>Mental</strong> illness affects all aspects of a person’s life: work, family,<br />

relationships, school, and recreation. It affects everyday life.<br />

The primary functions of the Certified Information and Referral Specialist at CMHA/Peel is to provide information around <strong>Mental</strong> <strong>Health</strong> and/or Addictions Inquiries. CMHA<br />

staff are edu<strong>ca</strong>ted and trained to provide information, appropriate referrals to lo<strong>ca</strong>l <strong>Mental</strong> <strong>Health</strong> and Addiction Services, brief services or interventions, follow-ups and<br />

<strong>ca</strong>n begin the intake process for CMHA/Peel and other programs.<br />

CMHA/Peel Certified Information and Referral Specialist are equipped to take <strong>ca</strong>lls from individuals whom may have an enquiry of a family member or friend with a<br />

mental illness, or they have been diagnosed with a <strong>Mental</strong> Illness. This includes Anxiety Disorders, Mood Disorders, Personality Disorders, Psychosis, Schizophrenia,<br />

self injury and Concurrent Disorders (Addictions and <strong>Mental</strong> <strong>Health</strong>). In regards to Dementia, Alzheimer’s CMHA/Peel is familiar with programs in the Peel Region and<br />

would link the <strong>ca</strong>ller to the various services.<br />

Should an individual <strong>ca</strong>ll in and is enquiring about information in the area of <strong>Mental</strong> <strong>Health</strong>/<strong>Mental</strong> Illness regarding services for <strong>Mental</strong> <strong>Health</strong> , CW CCAC <strong>ca</strong>n<br />

connect/link the client or family to a CMHA Certified Information and Referral Specialist at (905) 451-2123. There is always a live person to take these <strong>ca</strong>lls but from time<br />

to time all lines may be busy. There is the ability to leave a message and a <strong>ca</strong>ll back will happen within the business day.<br />

CMHA/Peel Certified Information and Referral Specialist are trained to work with individuals whom may be encountering a situation or a series of problems that feel<br />

overwhelming and out of control; or the individual has a mental illness and are overwhelmed in coping with there activities of daily living; or the individual expresses an<br />

inability to see options, choices that may be helpful. CW CCAC may link the client through a warm transfer to CMHA/Peel Resource Center to de-es<strong>ca</strong>late the individual<br />

and then assist the individual in putting a safety plan in place.<br />

When an individual is in acute distress, CMHA/Peel will involve the services of Peel Mobile Crisis or 911. Similar practice is in place for CW CCAC.<br />

With client’s/ family verbal consent, CMHA/Peel Certified Information and Referral Specialist will do a follow up <strong>ca</strong>ll back to inform a CW CCAC Case Manager as to what<br />

information was provided and/or what referrals were made<br />
