<strong>Health</strong> promotion programs and initiatives for people facing mental health conditions or<br />
substance use issues<br />
To become a client of the Bramalea Community <strong>Health</strong> Centre you must 1) live in the area<br />
served and 2) be a member of one or more of the following priority populations;<br />
- youth (13 - 25)<br />
- senior (65+)<br />
- newcomer to Canada (less than 5 years)<br />
Promote the mental health of all people and to service mental health consumers, their families<br />
and friends. The work of the organization focuses on combating mental health problems and<br />
emotional disorders. The tools include research and information services, sponsored research<br />
projects, workshops, seminars, pamphlets, newsletters and resource services. The organization<br />
runs programs that assist with employment, housing, early intervention for youth, peer support<br />
and recreation services for people with mental health issues. Stress reduction workshops and<br />
public edu<strong>ca</strong>tion <strong>ca</strong>mpaigns are scheduled for the community. In addition, CMHA acts as a<br />
social advo<strong>ca</strong>te to encourage public action and commitment to strengthening community<br />
mental health services and legislation and policies affecting services. All the mental health<br />
projects are based on principles of empowerment, peer and family support, participation in<br />
decision-making, citizenship, and inclusion in community life.<br />

Long Term Case Management Access to Recovery provides <strong>ca</strong>se management support for<br />
individuals with long term mental illness * clients <strong>ca</strong>n expect * supportive partnership with a<br />
<strong>ca</strong>se manager * assistance in understanding and managing illness * guidance in developing a<br />
recovery plan and reaching goals * linkage and coordination of community resources and<br />
family, friends and other service providers * meetings at home or in the community<br />
Mon-Fri 9 am-5 pm (some flexibility to meet the needs of individuals)<br />
The ACT team's role is to support you to live in the community. It <strong>ca</strong>n be hard to manage alone.<br />
The ACT team <strong>ca</strong>n help by providing services in a way that is different from traditional<br />
programs. The ACT team will work with you to understand your goals, and help you work<br />
towards them. Members of the ACT team will visit you in your home, your favourite coffee<br />
shop or in other places in the community to help you manage your illness.<br />
