This form should be completed and sent to us whenever an additional investment is made by you to the Russell Private
Investment Series (RPIS) into Fund(s) in which you currently hold units. If you are an existing investor making an investment
in a Fund in which you currently do not hold units, you will need to fill out the Application Form that accompanies the
relevant Product Disclosure Statement (PDS). If you would like to arrange for us to direct debit regular additional investments
into RPIS, please complete the Change to Regular Investment Plan Form that is available on our website.

5. Declaration and authorisation
I/We authorise Russell Investment Management Limited (RIM) to process my entitlement in accordance with my instructions.
I/We acknowledge and declare that:
1. I/We have read and understood the current PDS to which the
form relates;
2. I/We agree to be bound by the terms and conditions of the current
PDS and the Constitution for each Fund in which I/we apply for units,
as amended from time to time;
3. I/We acknowledge that the RIM reserves the right to refuse applications
for units at its discretion;
4. I/We acknowledge that the repayment of capital or the performance of
any option of the Russell Private Investment Series is not guaranteed;
5. I/We consent to the use of my/our personal information in accordance
with the ‘Your Privacy’ section of the current PDS, including the
provision of information to my/our nominated financial adviser;
6. All the information provided on this form is true and correct;
7. If I/we have completed the initial adviser fee section of this form I/we:
a. have read the adviser service fees section on page 24 of the current
PDS; and
b. request that RIM pay to my/our nominated adviser out of my
total additional investment amount set out in this form the initial
adviser service fee I/we selected in this form.
8. RIM may provide information on the status of my account balance
and investments in the Russell Private Investment Series to my/our
nominated financial adviser;
9. I/We have read and understood the contents of this form and have
checked that all the information I/we have provided on this form
is correct.
