Discharge to Municipal, County, or State DitchesAnswer <strong>the</strong> question “yes” or “no” whe<strong>the</strong>r <strong>the</strong> treated effluent is discharged to a city, county,state highway right-of-way, or flood control district drainage ditch. Authorization from thisentity must be obtained prior to commencing discharge. The wastewater permit does not grantthis authorization; it must be authorized by <strong>the</strong> owner of <strong>the</strong> structure. If <strong>the</strong> answer to <strong>the</strong>question for this item is “yes”, please read <strong>the</strong> following and answer <strong>the</strong> remaining questions asappropriate.For renewal applications, indicate by a check mark whe<strong>the</strong>r <strong>the</strong> entity granted authorization.For new and amendment applications, indicate by a check mark whe<strong>the</strong>r <strong>the</strong> entity grantedauthorization or if authorization is still pending. Provide a copy of <strong>the</strong> letter sent to <strong>the</strong> owner of<strong>the</strong> drainage structure with <strong>the</strong> application. Upon receipt, provide a copy of <strong>the</strong> response letter.i. Daily Discharge of 5 Million Gallons per Day or MoreFor permits that have a permitted average flow of 5 million gallons per day (MGD), or forapplications requesting an increase in permitted average flow to 5 MGD or greater, provide <strong>the</strong>name(s) of each county or counties within 100 statute miles downstream of <strong>the</strong> point(s) ofdischarge.j. Disposal Site LocationFor TLAPs, if <strong>the</strong> existing permit includes an accurate description, indicate so bychecking “yes” on <strong>the</strong> application form. If “no”, or a new site, provide a locationdescription of <strong>the</strong> effluent disposal site (e.g., irrigation, subsurface drip irrigation, evaporation).Do not provide directions to <strong>the</strong> disposal site. The location description must use easilyidentifiable landmarks found on <strong>the</strong> USGS map submitted as an attachment to <strong>the</strong> application.The description must include <strong>the</strong> distance in feet or miles from road intersections. If, however,<strong>the</strong> application is for a new facility or <strong>the</strong> description is inaccurate, provide an accurate locationdescription. Two examples of acceptable location descriptions are: 1) The effluent disposal site islocated 2,600 feet southwest of <strong>the</strong> intersection of State Highway 20 and Farm-to-Market Road1200; 2) The effluent disposal site is located 1.2 miles east of <strong>the</strong> intersection of Farm-to-MarketRoad 345 and County Road 10.NOTE: a change in location or increase in acreage requires a major amendment.k.
