10. SIGNATURE PAGECertificationEach entity applying for the permit is required to sign the certification statement. Thecertification must bear an original signature of a person meeting the signatory requirementsspecified under 30 TAC §305.44.If you have any questions or need additional information concerning the signatory requirementsdiscussed above, please contact the TCEQ’s Environmental Law Division at (512) 239-0600.30 TAC §305.44 - Signatories to Applications(a) All applications shall be signed as follows.1. For a corporation, the application shall be signed by a responsible corporate officer. Forpurposes of this paragraph, a responsible corporate officer means a president, secretary,treasurer, or vice-president of the corporation in charge of a principal business function,or any other person who performs similar policy or decision-making functions for thecorporation; or the manager of one or more manufacturing, production, or operatingfacilities employing more than 250 persons or having gross annual sales or expendituresexceeding $25 million (in second-quarter 1980 dollars), if authority to sign documentshas been assigned or delegated to the manager in accordance with corporate procedures.Corporate procedures governing authority to sign permit or post-closure orderapplications may provide for assignment or delegation to applicable corporate positionsrather than to specific individuals.2. For a partnership or sole proprietorship, the application shall be signed by a generalpartner or the proprietor, respectively.3. For a municipality, state, federal, or other public agency, the application shall be signedby either a principal executive officer or a ranking elected official. INSTRUCTIONS FOR INDUSTRIAL ADMINISTRATIVEREPORT 1.1The following information is required only for new permit applications and majoramendment applications.The following information is not required for renewal and minor amendmentapplications.1. AFFECTED LANDOWNER INFORMATIONa. Landowner Map ComponentsThe following information is required for the affected landowner list and other interestedparties. Please use the format described below. Examples of landowner maps have beenprovided for review and assistance. Affected landowner information is critical to theprocessing of the application and any errors may cause significant delays in processing theapplication.The landowners list is used by the TCEQ to notify affected landowners of the application by mail.These individuals, as well as others, may provide comments on the application or request acontested case hearing on the application.1. All applicants shall submit a map that clearly shows the following:• the applicant’s property boundaries;• the location of the treatment facility within the applicant’s property; and• the property boundaries of landowners surrounding the applicant’s property (Note: Ifthe application is a major amendment for a lignite mine, the map shall include theproperty boundaries of all landowners adjacent to the boundaries of the newfacility(ponds).)2. For applications to discharge treated effluent to waters in the state, in addition to thelandowners in Item 1 above, the map must clearly show the following:• the discharge point;• the highlighted discharge route for one mile downstream from discharge point;• the property boundaries of all landowners surrounding the discharge point and onboth sides of the discharge route for one full stream mile downstream of the dischargepoint; and• If the discharge point is to a lake, bay estuary, or affected by tides, the propertyboundaries of landowners along the shoreline for a ½-mile radius from dischargepoint.3.
