INSTRUCTIONS FOR SUPPLEMENTAL PERMITINFORMATION FORM (SPIF)This form applies to TPDES permit applications.The SPIF must be completed as a separate document. The TCEQ will mail a copy of the SPIF toeach agency as required by the TCEQ Memorandum of Agreement with the EPA. If any of theitems are not completely addressed and/or further information is needed, you will be contactedto provide the information before the permit is issued. INSTRUCTIONS FOR INDUSTRIAL TECHNICALREPORT 1.0The following information is required for all applications for renewals,amendments, and new TPDES permits and TLAPs. Please read the instructionscarefully.Please provide detailed technical information as needed. If more than one outfall is included inthe application, provide applicable information for each outfall. If an item does not apply to yourfacility, write N/A to indicate that you have considered it. Attach separate reports or additionalsheets as needed. Cross-reference all attachments to the question in the Technical Report. Youare not required to submit worksheets that do not apply to your application.1. FACILITY/SITE INFORMATIONa. Provide a brief narrative description of the general nature of your business and the type ofindustrial and commercial activity at the plant, including what specific products aremanufactured or produced and what services are provided.b. List, in descending order of significance, up to four 4-digit Standard IndustrialClassification (SIC) codes that best describe your facility in terms of the principal productsor services you produce or provide.List, in descending order of significance, up to four North American Industry ClassificationSystem (NAICS) codes which best describe the activities in which you are engaged.c. Provide a detailed description of the processes at the facility which generate wastewater.The description should include information such as any modifications to your processwater/stormwater handling facilities, the start-up or shutdown of any process or treatmentunits, any wastewater recycle projects, or any changes in production throughput.d. Provide a list of raw materials, major intermediates, and products handled at your facilitythat may be reasonably expected to be present in effluent which is either discharged ordisposed of via the authorizations requested in this application. Provide correspondingChemical Abstract Service Registration Numbers (CASRNs). Be specific and avoid tradenames. For commercial (non-manufacturing) facilities, provide a list of chemicals used onsitewhich could impact effluent quality. Attach additional pages if necessary.Note: If a material meeting the description above is confidential information as defined by30 TAC §1.5(d), the requirements of this provision may be satisfied by identifying theexistence of the material and providing non-confidential information about the material.The applicant must submit adequate information about the material, as determined by theExecutive Director, for the Executive Director to complete the technical review of theapplication.e.
