EXHIBIT 11: (FORM OF AGREEMENT) TO BE SIGNED BETWEEN THE SUCESSFULBIDDER & DEPARTMENT OF HEALTH & FAMILY WELFAREThis agreement is made on the ______ day _________ 2011 between _________________(Name and address of Authorised Officer, Department of Health & Family Welfare)hereinafter called ‘the Department of the one part and _____________ (Name & Address ofSucessful Bidder) hereinafter called “the Operator” of the other part, under which theoperator shall collect, receive, store, transport, treat and scientifically dispose the biomedicalWaste from the Medical Colleges, Dental Colleges, Regional/District Hospital, Civil Hospitals,Community Health Centers and First Referral Units located in the feeder districts includingprivate colleges, hospitals and clinics situated in the above mentioned places who choose toopt, strictly as per the Bio-Medical Waste (Management & Handing) Rules, 1998 as amendedtime to time and other Acts, Rules and guidelines issued by the Government of India,Government of Himachal Pradesh, the Central Pollution Control Board and H.P State PollutionControl Board will use its best endeavors to provide above mentioned services to theHospital during the period from (dd/mm/yyyy to dd/mm/yyyy).NOW THIS AGREEMENT WITNESSETH as follows:-1. In this Agreement words and expression shall have the same meanings as are respectivelyassigned to them in the Terms & conditions of contract hereinafter referred to.2. The following documents shall be deemed to form and be read and constructed as part ofthis Agreement, viz :(a) Letter of acceptance / award of contract(b) All Terms and Conditions mentioned in Tender Document(c) Notice inviting Tender(d) Commercial Bid(e) Addendums, if any(f) Any other documents forming part of the contract.3.
