Methodological approach, starting from screening the patient until post operative follow up is mandatory.This article discuses about the importance of collecting the data and it’s retrieval at that time of need. Riaz BaigE-mail : riazcmfs@yahoo.comThis collected data is known as Case history, it is avery important step as it is the primary lead to thesequence, planning the treatment and predicting theprognosis.After collection of the data, it should serve itspurpose of guiding the doctor throughout every step,proceeding further until recall appointments, andnot to be ignored as a piece of paper or an archivefile that might never be dusted again.Another key component of the case history is a consentform which should serve its purpose of keenly jottingthe acceptance of the treatment and its outcomeby the patient in full consciousness. 3 It should alsopossess the advantage of serving as a legal document.Hence it should be on paper and be also supportedwith the patient's signature and one other signature ofthe accompanying person (the witness). 4SUPPLEMENTARY DATA:(Photographs and Models)"A picture is equivalent to million words" is not justan old saying but a practical fact that can help toeliminate all of subjective opinions and comments,For Example: A patient who was recently treated formalocclusion can turn up within a few days sayingthat he/she is not very happy with the new profileand it’s unpleasant after the treatment. 5At times like these Photographs and models not onlyhelp you to educate the patient to understand betterand also helps to protect yourself from avoidableallegations.Apart from the above mentioned advantages, themaintenance of a fully descriptive file containingphotographs and models also help in situations offorensic identifications.Indian Journal of Multidisciplinary Dentistry, Vol. 2, <strong>Issue</strong> 4, <strong>Aug</strong>ust-<strong>Oct</strong>ober 2012 589
