6Application RequirementsIntended Use PlanThe SPCs must complete SectionsA & B of the application.Section A is a one-pageexecutive summary,which includes thefollowing information:• list of the organizationsto receive <strong>PATH</strong> funds• service areas• services the Federal <strong>PATH</strong>funds will support• number of clientsto be servedSection B consists ofstate-level and territoryinformation including:• the state’s operationaldefinitions of»»an individual whois homeless»»imminent risk ofhomelessness»»serious mental illness»»co-occurring disorder(serious mentalillness and substanceuse disorder)• estimated number ofindividuals who arehomeless with seriousmental illnesses by eachregion or geographicarea for the entirestate or territory• description of allocationof <strong>PATH</strong> funds• description of how <strong>PATH</strong>funding allocationsare consistent with thestate’s comprehensivemental health plan• information on whethermental health, SubstanceUse block grant funds,and/or general revenuespecifically serve peoplewho are homeless withmental illnesses• description of provisionof programmatic andfiscal oversight• information on whetherthe state provides, paysfor, or supports trainingfor <strong>PATH</strong> funded staff• description of thesource of requiredmatching non-Federalcontributions andassurances of availabilityof contributions• description of theprocess for publicnotice of the availabilityof <strong>PATH</strong> fundingThe Intended Use Plan (IUP) is the local providers’ requestfor <strong>PATH</strong> funding from the state or territory. The purposeof the IUP is for local providers to describe their intendedplan to deliver <strong>PATH</strong>-eligible services with the fundingreceived. The SPC’s role is to decide whether the requestmeets the priority for the state, the targeted needs, andthe grant eligibility guidelines. Several of the itemsrequested in the IUP require guidance from the RFA orthe Appendices.
