18<strong>PATH</strong> Data Report ClarificationsOn the first page of the report, makesure to list the contact informationof the person filling out thereport. This person is not necessarily the<strong>PATH</strong>-funded staff or provider contact.1. In A1 of Table A, list the total annualamount for services dedicated to persons whoare homeless and have serious mental illness(including <strong>PATH</strong>, matching, and non-<strong>PATH</strong> funds).2. In A3 of Table A, list matching fundsfrom state, local, or other resources to supportthe provision of <strong>PATH</strong> services needs toinclude all match resources. Do not includeFederal <strong>PATH</strong> funds in this amount.3. The values reported in A2and A3 may not add up to A1 if thereare other homeless services funds, inaddition to <strong>PATH</strong>, also reported in A1.4. All questions in the C and D tablesof the report include only persons enrolled(B3). Answers in Table C must be nomore than the number of persons enrolled(B3) and answers in Table D must add upto the number of persons enrolled (B3).5. Definitions of services are available onthe <strong>PATH</strong> website (http://pathprogram.samhsa.gov) and in the Annual Report Provider Guide.6. The Community Mental HealthServices question (Cd) in the AvailableServices section of the report needs toinclude the number of clients successfullylinked to mental health services as a resultof the <strong>PATH</strong> program, even if the mentalhealth services are not <strong>PATH</strong> funded.7. To determine how to report theService Type Code (i.e., 1–4) forcommunity mental health services,use the following:(1) if the <strong>PATH</strong> worker’s time on this activityis 100 percent <strong>PATH</strong> funded, then the serviceis 100 percent <strong>PATH</strong> funded and(2) if the <strong>PATH</strong> worker’s time on thisactivity is not 100 percent <strong>PATH</strong> funded,then it is partially <strong>PATH</strong> funded.8. The Voluntary Outcome Measures(Ck1–Ck5) are voluntary, but if providers do notreport, they must click the “Not Reporting” box.9. The state level defines “shortterm-” (D7b) and “long term(D7c) shelter”, but usually shortterm means thirty days or less and longterm means more than thirty days.10. The report is not completeuntil validation. Once validated,providers cannot make changes to thereport, unless the SPC reopens it.11. After clicking “Validate” at the endof the report, a pop-up window willopen if the program identifies anyerrors.
