The complete unit name, street address, city, state, <strong>and</strong> ZIP code areentered.(4) <strong>Training</strong> Center Code – The training center code for the unit.(5) <strong>Unit</strong> Identification Code – The <strong>Unit</strong> Identification Code of the unit.(6) Finance Processing Code – The Finance Processing Code of the unit.(7) For month of – The month <strong>and</strong> year for which DA Form 1379 was printed.(8) Name (col a) – ARPERCEN will print in alphabetical sequence soldier’s name (Last, First, <strong>and</strong> Middle Initial) up to 18 characters.(9) Grade (col b) – ARPERCEN will enter soldier’s rank. ROTC/SMP cadets will be entered as CT5, CT6, or CT7 as appropriate. OfficerC<strong>and</strong>idates will be entered as OC5, OC6, or OC7 as appropriate. Warrant Officer C<strong>and</strong>idates will be entered as WC5, WC6, or WC7 asppropriate.(10) Social Security Number (Col c) – ARPERCEN will enter soldier’s social security number with hyphens as shown.(11) Attached Personnel – A subdivision of the form listing the individuals who are attached to the unit for pay <strong>and</strong> administration.(12) UIC/FPC Assigned – The <strong>Unit</strong> Processing Code <strong>and</strong> Finance Processing Code of the unit to which the attached personnel are assigned.Correction Instructions(1) To correct name, line out old name <strong>and</strong> enter correct name.
