(a) In case the entity is registered under the Companies Act, 1956, andthe name of company has been changed, the following documentsshould be obtained from the Client:i) Letter requesting for change of name signed by the authorizedsignatories.ii) Certified true copy of fresh certificate of incorporati<strong>on</strong>c<strong>on</strong>sequent up<strong>on</strong> change of name issued by the Registrar ofCompanies.iii) Certified true copy of Board Resoluti<strong>on</strong> for change of name.(b) In case of other corporate entities, where the name of corporate hasbeen changed, the following documents should be obtained fromthe Client:i) Letter requesting for change of name signed by the authorizedsignatories.ii) Certified true copy of Board Resoluti<strong>on</strong> for change of name.iii) Certificate of registrati<strong>on</strong> issued by SEBI / relevant StatutoryAuthorities, as applicable.(c) In case of amalgamati<strong>on</strong> / scheme of arrangement of the entityregistered under The Companies Act, 1956, the following procedureneeds to be followed:I) Obtain the following documents:i. Letter requesting for change of name signed by the authorizedsignatories specifying the details of the depository account.ii. Certified true copy of court order sancti<strong>on</strong>ing the scheme ofarrangement for merger or amalgamati<strong>on</strong>.iii. Certified true copy of Form 21 filed with Registrar ofCompanies (RoC) as required under secti<strong>on</strong> 394(3) of TheCompanies Act, 1956 al<strong>on</strong>gwith certified true copy of challanof payment made to RoC as generated from the website of49
