(a) In case the entity is registered under the Companies Act, 1956, andthe name of company has been changed, the following documentsshould be obtained from the Client:i) Letter requesting for change of name signed by the authorizedsignatories.ii) Certified true copy of fresh certificate of incorporationconsequent upon change of name issued by the Registrar ofCompanies.iii) Certified true copy of Board Resolution for change of name.(b) In case of other corporate entities, where the name of corporate hasbeen changed, the following documents should be obtained fromthe Client:i) Letter requesting for change of name signed by the authorizedsignatories.ii) Certified true copy of Board Resolution for change of name.iii) Certificate of registration issued by SEBI / relevant StatutoryAuthorities, as applicable.(c) In case of amalgamation / scheme of arrangement of the entityregistered under The Companies Act, 1956, the following procedureneeds to be followed:I) Obtain the following documents:i. Letter requesting for change of name signed by the authorizedsignatories specifying the details of the depository account.ii. Certified true copy of court order sanctioning the scheme ofarrangement for merger or amalgamation.iii. Certified true copy of Form 21 filed with Registrar ofCompanies (RoC) as required under section 394(3) of TheCompanies Act, 1956 alongwith certified true copy of challanof payment made to RoC as generated from the website of49
