14.10 Co-curricular Achievement Transcript (CAT)If you have successfully attended particular Computer Training Module(s) which is (are) not specifiedin your Programme Definitive Document as compulsory training requirement, the training can berecognized as your co-curricular achievements. These records will be periodically sent to SAO fortheir compilation of Co-curricular Achievement Transcript (CAT). You may approach SAO to ask foryour own CAT for reference or retention.15. TRAINING CERTIFICATIONUpon completion of your IC Training, you may apply for a certification on trainingattainment; Certificate for applicants who have successfully completed ALL ICTraining or Testimonial for applicants who have partially completed IC Training orlost of their original certificate, both of them contains a complete record ofsubject/module results of your training in IC. A fee of HK$ 50 will be charged.Payment shall be made in cash at the IC Student Enquiry Counter.If you are unable to come in person to collect the certificate, you may authorizeanother person in writing (with the HKID number of the authorized personprovided) to collect it on your behalf. A copy of your HKID card (orpassport for those who do not have an HKID card) must be attached toyour authorization letter.The IC is not responsible for the safe custody of any uncollectedparchments after the collection period. Uncollected parchments maybe destroyed after the stipulated collection period. IC reserves theright to withhold the issuance of a certification to a student who has unsettled matters with IC.16. USE OF IC FACILITIESStudents are strongly encouraged to take full advantage of the IC'sfacilities. There is a considerable amount of high tech equipmentwithin the IC Building, to at least part of which you will be given anintroduction during your training programme. Time constraintsinevitably mean, however, that you will seldom have theopportunity to explore the full potential of such equipment.Facility booking form ……IC Form IC-T01 Request for ICFacilities/Service can be accessedfrom IC homepage “StudentCorner” or from IC technicalworkshops. More information onA number of short training programmes (normally of one or twoIC’s support services can be founddays duration) which will enable you to become much more familiarin IC homepage also.with such modern equipment and technology are available tostudent groups on request. Such programmes are free of charge.Enquire about them with the workshop staff. The IC exists to serveyou and all reasonable requests from student groups for additional training in specific high techareas will be seriously considered.If you are involved in a Team Project or other non-modular training activity, or if you simply wish toavail yourself of IC facilities, you may apply for the use of IC machine tools and other equipment byfilling in Form IC-T01 which is available from the various workshops or the IC Student EnquiryCounter. 16.1 Use of FacilitiesWhich facility to use”If you are not sure of theappropriate workshop toapproach, you may contact theIndustrial Projects Unit (IPU) of IC.Location: Room w401jEnquiry Tel: 2766-7639Students who have completed the IC learning programme arestrongly encouraged to use our facilities and services to aid theirstudy work or research. Prior consultation with relevantworkshop staff is requested to ensure the facilities studentsrequested are appropriate and feasible to their work.For students of other institutions such as HKUST, CUHK, CityU,etc. who are not convenient to consult with the workshop inperson, they may send email to the workshop staff and makeenquiry on their request first.16.2 Booking of FacilitiesAdvance booking is required to ensure the availability of technical workshop, staff and facilities.Complete and return IC form IC-T01 to the specific technical workshop to arrange the booking.The technical staff may request additional information from students such as engineering drawing,material sample, etc. to ensure the feasibility of work requested. Students should haveappropriate skill or knowledge on the specific equipment/machinery’s operation.16.3 ChargesExcept for facilities/services using expensive tooling expensive tooling and consumables such aslaser cutting, EDM wires, RP resins, most of the services rendered us to students are FREE ofcharge.If a student brings his/her own materials for fabrication and manufacturing, we will not charge foryour materials. If a student uses our workshop materials, a charge will be imposed.If a student has not received our training on that particular facility/machine before, we can alsooffer a fabrication and manufacturing service. A charge will be imposed for such services.17. SERVICES FOR STUDENTS WITH DISABILITIESThe IC General Office (GO) coordinates the services for students with disability.
