We alsoprovide services to the only inhabited island in Northern Ireland, Rathlin.The population of (Causeway locality) is approximately 100,000.Causeway Hospital is situated in Coleraine near the North Coast of NorthernIreland in an area of outstanding natural beauty.Causeway Hospital covers a rural coastal area from Castlerock to Cushendalland reaches inland to the towns of Garvagh, Kilrea and Rasharkin. Referralsare increased by the high transient tourist population and the number ofstudents, who reside in the area during the academic year.Mid-Ulster Hospital is situated in Magherafelt, a small town, approximately 35miles from Belfast. Local education facilities are excellent and recreationalfacilities are available for all interests.Altnagelvin Hospital is the closest hospital to Causeway and is 30 miles distant– 1 hours drive. Antrim Hospital, the other acute facility in the Northern Health1

These include the provision of enhanceduse of Day Procedure facilities, provision of protected elective beds andwaiting list initiatives.2


Therefore, it is an exciting and rewarding opportunity for an enthusiasticcandidate to contribute to a forward thinking Trust in a modern, well-equippedenvironment.There is close liaison between Consultant Surgeons and the GastroenterologyDepartment regarding ERCPs, management of Inflammatory Bowel Disease,acute upper GI bleeding etc.Mid-Ulster Hospital: In September 2009 a decision was announced by theHealth Minister that in-patient surgery (more complex surgery requiringovernight stay from Mid-Ulster Hospital) was to move to Causeway Hospitalfrom November 2009. Day Surgery and Endoscopy continues at the Mid-Ulster Hospital, with an increase in the number of Day Case operationsundertaken there.Causeway Hospital opened at end May 2001. It is a state-of-the-art facility,which includes 240 beds, an outstanding Day Procedure Unit and excellentOut-Patient facilities. There is access to a range of clinical support services onsite, including CT Scanner with 24 hour Consultant Radiologist cover, PACsSystem, 24 hour, fully accredited (CPA – UK) Ltd Laboratory services areoperational on the Causeway Hospital site and there is access to MedicalPhotography support. The SBATS (small-bore air transport system) facilitiesspeed the communication between clinical departments including Laboratoryand Pharmacy. A Telemedicine System is currently available to facilitatesharing of information with other health care sites. There is access to Out-Patient Rehabilitation Services in the Fort Centre. The bed complementincludes:General Surgery (including Emergency Paediatric Surgery and Urology)Day Procedure UnitGeneral Medicine (including Dermatology)Coronary CareIntensive Care/High DependencyElderly RehabilitationPaediatric MedicineObstetricsGynaecologyAcute PsychiatryMid-Ulster Hospital, Magherafelt will be a base for Day Surgery/EndoscopyUnit which this post-holder and colleagues will use for surgical sessions ascontained in their job plan.Robinson Memorial Hospital is situated in Ballymoney and includes GPbeds, Palliative Care and a modern, well-equipped PhysiotherapyDepartment.Dalriada Hospital in Ballycastle provides In-Patient care for MS respite andGP patients.3


SurgicalDepartment:The Surgical Department includes 34 Emergency In-Patient beds (sharedwith Acute Medicine), including 9 single rooms, along with a 15 bed ElectiveSurgical Unit. 4 Paediatric Surgery beds are located in the PaediatricDepartment.There is access to Intensive Care and High Dependency beds (total of 4funded) in the Intensive Care Unit, which is located immediately beside theMain Operating Theatre Suite.Operating Theatre:There are 3 Theatres for In-Patients.The Theatre Suite is a spacious facility with separate accommodation for thereception of patients, anaesthetic rooms, and 6-bed recovery area. Thedepartment is equipped to a high standard with Kreuzer pendants, linkmonitoring to A&E, CCU and ICU, camera facilities for linking to the MDEC forteaching purposes, 3 complete Laparoscopic systems, Video Endoscopy, andImage Intensifier. There is clerical support provided inTheatre to produceoperation notes, which accompany the patient on return to the ward.There is formal co-ordination of the scheduling of Operating Sessions, DayProcedure Sessions, Anaesthetic cover and Out-Patient activity.The Trust has recently introduced a Theatre Management System.Day ProcedureUnit, CausewayHospital:This unit is an integral part of the hospital building and has direct accessbeside the main front entrance of the hospital. There are 22 places including6 beds for Laparoscopic Majors and 16 Day cases.Facilities includes:2 Operating Theatres – Used for General Surgery, ENT, Gynaecology,Dentistry and Diathermy for Podiatry; separate Endoscopy room used forprocedures including Gastroscopy, Colonoscopy, Bronchoscopy andColposcopy; dedicated recovery area; equipment includes Kreuzer Pendantsystems, 2 Laparoscopic Systems and Video Endoscopy; Urodynamicsequipment used by Consultant Urologist, Gynaecologists and IncontinenceAdvisor; excellent patient facilities and information, with discharge lettersbeing issued to patients before leaving the Unit.Day SurgeryUnit,Mid-UlsterHospital:Staffing:A 12 bedded unit has been operational from 9 th November 2009. Thisfacilitates day surgery for General Surgery and Gynae, ENT and OphthalmicSurgery. There is a separate Endoscopy with up-to-date facilities.There is one General Manager for Surgery and related specialties, supportedby a Department Manager in each of the 2 Surgical Wards and inTheatres/Day Procedure Unit, Causeway and Mid-Ulster as well as a range ofRegistered and Enrolled Nurses, Nursing Auxiliaries and Medical Technical4


Officers.Nursing staff based in Surgical Wards, Operating Theatres and DayProcedure/Surgery Unit participate in a range of in-service training andcontinuing professional development. There are Specialist/Link Nurses inInfection Control, Stoma Care / IBD, Incontinence and Nutrition.SurgicalWorkload, foryear endedOrthopaedics,Fractures/NeurosurgerySee Appendix B.The successful applicant will be expected to receive and manage all acutesurgical admissions. Those admissions involving specialties outside his/hertraining, for example neurosurgery or fractures can be referred to suitableunits within the province for appropriate treatment using well-establishedprotocols in place at the present time. The transfer of these patients willrequire an ability to stabilise the patient prior to transfer and deal with anygeneral surgical problem, if appropriate, prior to this transfer. The transfer canonly take place if considered safe to do so. If further treatment of the patientis required by subspecialties outside his/her training and transfer is unsafe,arrangements can be made for consultant input on site by arrangement withthe sub-specialist unit involved.Minor fractures are managed by visiting Consultant Orthopaedics fromAltnagelvin Hospital, providing weekly Out-Patient Fracture Clinics. Thesuccessful candidate will have no direct input in Fracture Clinics and will beexpected to refer any fracture directly to Orthopaedic Service.The existing Consultant Surgeons provide cover for all trauma.Office andSecretarialProvision:Nature of WorkUndertaken:There will be access to an office and secretarial support.Share in the provision of General Surgical services to the Northern Healthand Social Care Trust and in the proper functioning of the hospital.The work will comprise mainly of General Surgery both acute and elective.There will be sessional commitments to Theatre, Day Procedures / DaySurgery and Out-Patients.Participate in the General Surgery on-call rota, which would be a 1:6. TheConsultant Surgeons on-call is based on a “Surgeon of the Week Rota”.Play an active part in the functioning and administration of the Surgicalservice; contribute to the planning and future development of the serviceand the preparation of change in operational policy.Make effective and efficient use of beds and facilities to which he/she hasaccess.5


Have responsibility for continuing care of patients in his/her charge andundertake associated administrative duties.Participate in the selection of staff in Training Grades and non-consultantcareer grades, Induction, Appraisal interviews and play an active part in thedevelopment of post-graduate activities in Causeway. Take a lead role intraining and supervision of medical staff in the training grades and the nonconsultantcareer grades. Where appropriate, the appointee will be namedin contracts of employment of doctors in training grades as the personresponsible for overseeing their training and as the initial source of careeradvice to such doctors.Review of Log Books of students, training grades etc as required to meetthe standards of Royal College of Surgeons, Post Graduate Dean andFaculty of Medicine & Sciences (QUB).Support the ongoing development and training of staff.Participate in multi-disciplinary audit. Update clinical, managerialknowledge and skills. Participate in continuing medical education and inresearch programmes as appropriate and enrol in a recognised programmeof Continued Professional Development. The Trust will support StudyLeave as contained in the Terms and conditions of Service. All researchmust be managed in accordance with the requirements of the DHSSPSResearch Governance Framework. Consultants must comply with reportingrequirements, systems and duties of action plan put in place by theemploying organisation to deliver research governance. Consultants mustalso comply with the GMC guidance 'Good Practice in Research’, as fromtime to time amended.Attend meetings of the Surgical Division of the Northern Health & SocialServices Board.The post-holder will be encouraged to continue to develop his/her ownspecial interest and also an interest in general or paediatric surgery. Thiscould include consultation arrangements with another hospital/trust asappropriate.Develop departmental protocols in conjunction with other Clinicians andprofessionals within the Trust and from other health care providers. Thiswill require close co-operation with other specialties and hospitals to providea high standard and comprehensive range of services.Responsible for the continuing clinical and professional responsibility forpatients admitted under his or her care for a local population. It is also theduty of the Consultant to:-Keep patients (and/or their carers if appropriate) informed about theircondition;6


Involve patients (and/or their carers if appropriate) in decision making abouttheir treatment;Maintain professional standards and obligations as set out from time to timeby the General Medical Council (GMC) and comply in particular with theGMC’s guidance on ‘Good Medical Practice’ as amended or substitutedfrom time to time.Participate with colleagues in the Trust’s planning process.Work collaboratively with other Trusts in keeping with best practice, regionalstrategy and trust policy.Use Trust wide and departmental computer technology in line with theTrust’s Information Management and Technology strategy.Set, monitor and evaluate quality standards for General Surgery service andensure that all staff adhere to the philosophy and values contained withinthe quality strategyParticipate in the Clinical Governance process.Participate in the Appraisal process for Consultants.The post-holder will be required to agree a job plan with the Clinical Directorwithin three months of taking up the appointment. A provisional workprogramme is enclosed (Appendix A).The post-holder will be responsible for carrying out any work related to andreasonably related to the duties set out in their <strong>Job</strong> Plan as:-The keeping of records and the provision of reportsThe proper delegation of tasksMaintaining skills and knowledge.Through his/her own actions ensure adherence to the Principles of Equalityand Human Rights.Provide a high level of skill and expertise to children/families to ensure thesafeguarding of children within the legislative framework and Best PracticeStandards. Be responsible for acquiring appropriate training in childprotection and be familiar with Child Protection Standards, Policy andProcedure.Ensure the principle of Public Involvement is adhered to throughcommitment to involving service users, their carers and the wider public inall decision making that affects them as individuals or as a communitythrough a range of consultant processes.7

Regular meetings are held withmembership representative of Clinicians, Nurses and Management.There are excellent facilities for study and lectures etc in the MultidisciplinaryEducation Centre on the Causeway Hospital site.A comprehensive library containing up-to-date reference books, periodicalsand journals are provided in the Multi-disciplinary Education Centre.Computer facilities including Microsoft excel; PowerPoint and wordpackages and CD-ROM’s are available for use by Medical Staff. Access tothe Internet and databases such as CINAHL, COCHRANE and MEDLINEare available. Books may also be requested from the Library at Queen’sUniversity, Belfast via the Queens catalogue.8


Terms and Conditions of ServiceCurrent SalaryScale:Conditions ofEmployment:£74,504 - £100,446 per annum - HSS (TC8) 1/2010The appointment is on a whole-time or maximum part-time basis.Practitioners who for personal reasons are able to contract for alimited number of half days are also eligible to apply.The appointment will be subject to the Consultant Terms andConditions of Service (Northern Ireland) 2004.Study leave entitlement – 30 days in 3 years, which may include oneconference outside United Kingdom in 3 years for which standardgrant is payable.
