THE CITY OF PITT MEADOWSHIGHWAY and TRAFFIC BYLAW No. 2260Consolidated VersionPage 43Section 4This permit shall be valid and subsisting from the:day of , 20 ,and shall expire on the:day of , 20 ,during the hours of:,and at all times during the currency thereof, shall be subject to cancellation if theholder thereof shall neglect, fail, or refuse to observe and comply with all therequirements of the City of Pitt Meadows Highway and Traffic (Bylaw No. 2260),and is issued subject to the following conditions:1. That all necessary plans and specifications of any works involved havebeen deposited with the Director and Development Services and havebeen approved by him/her.2. That this permit shall be valid only for the specific works or other uses of amunicipal highway stated herein. All alterations and addition must becovered by a separate permit.3. That the construction and maintenance of any works under this permitshall be carried out and completed to the satisfaction of the Director ofOperations and Development Services.4. That any person appointed by the Director of Operations andDevelopment Services for that purpose shall have free access at all timesto al parts of any works constructed under this permit for the purpose ofinspecting the same.5. That while reasonable care will be taken on the part of the City of PittMeadows to avoid damage to any private works constructed under thispermit while carrying out the construction or maintenance of any publicwork on any highway, the City of Pitt Meadows accepts no responsibility ofany kind for such damage if the same should result from such public work.6. That before opening up any highway or interfering with any public workunder this permit, notice in writing of intention to do so shall be given tothe Director of Operations and Development Services, not less than seven(7) days before the work is commenced.#55701v3

That the permit applicant shall hold and save harmless the City of PittMeadows from and against all claims, damages, and lien claims of everykind, arising out of or in any way connected with any works or other thingsfor which this permit is issued.8. That this permit applicant shall obtain and maintain during the term of thepermit a comprehensive general liability insurance policy providingcoverage of not less than two hundred thousand (2 000 000) dollars,naming the City of Pitt Meadows as an additional named insured andproviding that the said policy shall not be canceled, lapsed or materiallyaltered without thirty (30) days notice in writing to the City of PittMeadows. A copy of such policy shall be delivered to the City of PittMeadows prior to the issue of this permit.9. Other conditions :Section 5Special conditions, if applicable, are as follows:1. Your company is responsible to clean / sweep / flush Cityhighways as follows:a. Road / Avenue / Street fromRoad / Avenue / Street toRoad / Avenue / Streetis to be cleaned on a daily basis.b. Road / Avenue / Street fromRoad / Avenue / Street toRoad / Avenue / Streetis to be flushed every Friday.#55701v3
