The name and address of theOffice that administers FERPA is: FamilyPolicy Compliance Office, U.S. Departmentof Education, 400 Maryland Avenue,SW, Washington, DC, 20202-8520.Directory InformationUMPI has designated certain informationcontained in the education records ofits students as directory information forthe purposes of FERPA. Such directory informationmay be publicly shared by theUniversity without the student’s consentunless the student has taken formal actionto restrict its release. Directory informationincludes: name, date of birth, address,phone number, class level, program ofstudy, dates of attendance, enrollment status(full or part-time), degrees and awardsreceived, most recent previous educationalinstitution attended, participation insports and activities, and appropriate personalathletic statistical data. Students canrequest that their directory informationnot be released at any time while they arean enrolled student at UMPI. Studentscan sign into the MaineStreet portal andselect Student Center, then select PrivacySetting from the drop-down menu. Selecteither “Do not release any directory information”or “Release all directory informationexcept address,” then click Save inorder to restrict release of Directory Information.Students may also request nondisclosureof directory information bycontacting the Office of Student Records.If a student chooses to have directory informationsuppressed, UMPI cannot assumeresponsibility to contact a studentfor subsequent permission to release thisinformation. Regardless of the effect on astudent, UMPI assumes no liability as a resultof honoring a student’s instructionsthat such information be withheld. Requestsfor non-disclosure will be honoredby UMPI until removed, in writing, bythe student or by the student updatingtheir privacy settings via the MaineStreetStudent Center.Maine State Immunization Law forPost Secondary EducationMaine State law requires that all stu dentswho are enrolled full-time or part-time in adegree program or who are taking at least12 credits through the Continuing EducationDepartment show proof of immunityto the following infectious diseases: measles(rubeola), rubella (German measles),mumps, tetanus and diphtheria. The Universityis not al lowed to permit any suchstudent to be enrolled or to attend classeswithout a certificate of immunization orother evi dence of immunity. The presentationof acceptable proof of immunizationor immunity must be made prior to a studentsattendance at class or participa tion inother activities conducted by the University.Certificate of ImmunizationStudents must present certification of immunizationor proof of immunity which indicatethe following:1. Diphtheria and tetanus vaccine givenwithin the past 10 years. If a tetanus shotonly has been given within the past 5 years,the student may be tem porarily exemptfrom the diphtheria requirement.2. Two doses of measles vaccine given afterthe first birthday with live vaccine. Vaccinesgiven after 1968 use live vaccine. If the vaccinewas given before 1968, the type of vaccinemust be indicated. “LIVE VACCINE” isacceptable. “UNSPECIFIED VACCINE” is unacceptableand the stu dent will need to bere-immunized or show proof of immunitythrough labora tory bloodwork called a titre.3. Two doses of rubella vaccine given afterthe first birthday,4. Two doses of mumps vaccine given afterthe first birthday.Proof of ImmunityA photocopy of the following is ac ceptableas proof of immunity: school immunization/healthrecord signed by school nurse orphysician; immunization record from yourphysician, nurse practitioner, or physi ciansassistant or other professional health careprovider; U.S. Military immunizationrecord; or, laboratory evidence of immunity(titres) to rubeola, rubella, and mumps ifimmunization records are not available.All records must be signed by the phy sicianor nurse who administered the vaccine.Please do not submit original records as immunizationrecords will be kept on file andnot returned. Send copies of records to theAdmissions Office, 181 Main Street, PresqueIsle ME 04769. 207.768.9532.Re-immunization/ExemptionsIf you do not have the required im munizationsor do not have official re cords,have your health care provider immunize orre-immunize you prior to coming to campus.Students with medi cal exemptionsfrom vaccination must present a writtenreason for exemption signed by their healthcare provider. Exempted students shall beexcluded from campus during adisease outbreak,should one occur.Right to ReviseUMPI reserves the right to revise,amend, or change items set forth in thispublication. Accordingly, readers of thispublication should inquire about revisions,amendments, or changes whichhave been made since the date of printing.The University reserves the right to cancelcourse offerings, to set minimum andmaximum size of classes, to change designatedinstructors in courses, and to makedecisions affecting the academic standingof anyone participating in a course or programoffered by UMPI. a c a d e m i c i n f o r m a t i o nRegistrationcontinuedAcademic AdvisingFollowing initial registration, matriculatedstudents are generally assigned to facultyfor academic advising. Students mustmeet with their faculty advisors at leastonce each semester to register for the upcomingterm; however, students arestrongly encouraged to maintain closecontact with their advisors throughout thesemester. Faculty advisors are available toassist students with all matters relating totheir academic goals, but it is also importantfor students to recognize their role inthe advising relationship. Advisors cannothelp students who do not seek out theiradvice. While it is important for studentsto realize that they are ultimately responsiblefor the successful pursuit of their degreerequirements, the relationship theyestablish with their academic advisor canbe invaluable in that effort.The Advising CenterThe Advising Center, located in SouthHall, assists students and faculty with advisingand academic policy questions, ensuresthat matriculated students areassigned to a degree program advisor, andfacilitates changes of major as well as Leavesof Absence and Withdrawals from the university.The Center also coordinates placementtesting, advising, and registration fornew students.Declaring a MajorMany students choose to begin their academiccareer as undecided or undeclaredmajors. Students may be advised by a facultyadvisor in an area of interest while remainingofficially undeclared, or they canbe advised by the Director of Advising. Ineither case, undeclared students will be encouragedto explore their educational andcareer interests with the objective ofchoosing a major as soon as possible. Matriculatedstudents are expected to declarea major prior to earning 60 degree hours.Students approaching 60 credit hours whoare still undecided about their majorshould make an appointment with the Directorof Advising.Change of Major or AdvisorStudents seek information about changingtheir major or advisor for a variety ofreasons. The Advising Center can helpstudents who simply wish to explore theiroptions and will update the student’s officialrecord when the decision to changehas been made.Registration for MatriculatedStudentsEach fall and spring an advisement andregistration period is held for continuingstudents to select their courses for the followingsemester. During this period, studentsregister for courses in the order ofclass level (seniors first, juniors second,sophomores third, freshmen fourth). Studentsare responsible for monitoring andfor planning their schedule of courses accordingto the degree program requirementsin place at the time of matriculationinto their degree program.Registration For Non-MatriculatedStudentsIndividuals may take college courses forpersonal enrichment as non-degree studentsby registering and paying for thecourses without going through a formaladmission process.Non-degree students may enroll in upto 9 credit hours of coursework.Course NumberingCourses numbered between 100 and 299are lower level courses. Courses numbered300 and above are upper level courses.Course PrerequisitesPlease refer to each course in this cataloguefor specific prerequisites.Course Registration Changes✓ Course Add-Drop Period - During thefirst five class days students may addcourse(s); during the first seven class days studentsmay drop course(s) without notationon the record. Students who find that theirnames are not on the instructor’s official classroster should check immediately with theOffice of Student Records to make necessarycorrections in the registration records. Onceregistered, students may make changes totheir registrations electronically.✓ Course Withdrawal Period - After theCourse Add-Drop Period, students maywithdraw from courses and receive a notationof “W” on their record. The last date fora withdrawal without academic penalty willbe determined by the Vice President for AcademicAffairs and published in the academiccalendar. All students leaving a course withoutofficially withdrawing will receive a gradeof F or L in the course. This F or L grade willbe computed in the student’s grade point average.After the deadline passes for officialwithdrawal from a course, a letter grade of Fwill be assigned. A student may be pardonedonly on the grounds of serious illness orproven extenuating circumstances as determinedby the instructor.Course Waiver PolicyOn rare occasions students at UMPImay be granted waivers for requiredcourses based on extensive experience in agiven area. The student must submit supportingdocumentation detailing his/herprevious applicable experience to theChair of the College offering the course tobe waived. The Provost/Vice President ofAcademic Affairs has final approval authority.Auditing CoursesStudents auditing a course pay regulartuition and receive no credit for thecourse, but will have the notation AUrecorded on their transcripts. Auditcourses must be declared at registration.Pass/Fail OptionStudents may elect to take one courseper semester on a pass/fail basis. Coursesin the General Education Curriculum andcourses required in the student’s majorfield are excluded from this option.Students who are receiving VeteransBenefits should note that when taking thepass/fail option, a pass grade is required inorder to receive Veterans Benefits.
