Techwill attempt to diagnose and fix problem via the phone. If problem cannot be fixed, tech will issue a returnauthorization number which customer writes on the outside of the box. Tech will give customer the shippinginstructions. HME will repair the unit(s) and promptly return to customer at no charge.EWA #Extended Warranty Agreement Information(Fill out order form on reverse)Base stations: Offers extended warranty for the following HME base stations: BS121, BS200,BS850, MB100, MB300, MB300ES. Coverage excludes external accessory items (antennas,adaptors, batteries, headsets, and power supplies).<strong>Com</strong>municators, wireless speaker station: Offers extended warranty for the following HMEcommunicators: BP200, BP300, BP850, WH200, WH300, WS200/CL200. Coverage forWS200 Wireless Speaker Station includes one CL200 Call Light Adapter; serial number ofcovered CL200 must be listed. Coverage excludes external accessory items (antennas,adaptors, batteries, headsets, and power supplies).Battery charger: Offers extended warranty for AC40A Battery Charger. Coverage excludesexternal accessory items (adaptors batteries, and power supplies).Equipment Warranty Agreement (EWA) Terms and ConditionsPerUnitPrice$249/year$49/year$99/year1. This agreement ensures that the covered equipment will operate in accordance with HME’s specifications for the covered period. During thisperiod, HME will service or repair the equipment that failed to operate within HME‘s specifications due to normal use and operation.Equipment abuse and Acts of God are not covered, including power surges. Equipment deemed to be Beyond Economical Repair (BER) willresult in the equipment being returned to customer and the EWA for that item cancelled. The customer will be refunded the amount paid forthe EWA less any expenses that may have occurred for prior repairs.2. This agreement extends the mail­in factory warranty for a period of one year starting on the date that the original warranty expires. If theoriginal warranty has already expired, the agreement is effective for a period of one year starting on the date that payment is received. (If youare unsure when your original warranty expires, you may look it up at www.hme.com/AWWebLive/warranty/pro_serlookup.asp )3. The customer pays the freight to HME and HME pays return freight via the same freight method.4. This EWA covers the equipment specified. It does not cover antennas, adapters, batteries, cables, connectors, earmuffs, headsets, pouches,or power supplies.5. Any upgrades that are considered to be bug fixes or FCC mandated adjustments will be performed at no charge. Upgrades that addenhanced features are not included as part of this agreement.6. Agreements are billed in full at time of purchase. Customer may pay via credit card, cash in advance (CIA), or N/30 open account. For openaccount orders, returned items under contract will not be repaired until the invoice has been paid in full.7. HME shall not be liable for any delay or failure to perform its obligations due to Acts of God, strikes, emergencies, regulatory or othergovernment action. HME reserves the right to modify or cancel the EWA program due to shortages or other causes beyond HME’sreasonable control. Should this occur, the customer will be notified of cancellation. In the event of changes, the customer will have theopportunity to accept or cancel the EWA.8. This EWA has been approved and accepted by the customer named above.
