PSYCHOSOCIAL EVALUATIONS FOR ALCOHOL<br />
AND OTHER SUBSTANCE ABUSE (2-hour)<br />
State of <strong>Florida</strong> Approved<br />
Purpose: To identify risk factors associated with alcohol or other drug use.<br />
Approved by the state of <strong>Florida</strong>.<br />
MAY BE COURT ORDERED.<br />
Goal: To provide a thorough face-to-face psychosocial evaluation to determine harmful<br />
involvement with alcohol or other drugs, assess risk factors and facilitate a<br />
referral to community services based upon need, and if Court ordered, assist with<br />
reporting requirements.<br />
Designed to coordinate with paperwork required by out of state drivers license<br />
programs for license clearance.<br />
Evaluation Content:<br />
� Client completes a state and/or nationally certified self-reporting instrument.<br />
� One (1) hour evaluation by a DHSMV certified evaluator.<br />
� Results are reported to referring agency, i.e. Courts, Probation, Employer, or Employee<br />
Assistance Program (EAP).<br />
Registration:<br />
� Enrollment is completed in person at one of our Brevard, Orange, Osceola or Seminole<br />
County offices.<br />
� The evaluation appointment is selected during the initial office visit.<br />
