Create a 'dummy' questionnaire with:<br />

● 'lead<strong>in</strong>g' or bias questions<br />

● odd numbers of responses <strong>in</strong> MCQs<br />

● 'scale' questions with <strong>to</strong>o many (or not enough) options.<br />

● language that is <strong>in</strong>accessible<br />

● not enough space provided for an <strong>in</strong> depth response <strong>to</strong> open questions<br />

● 'Heads I w<strong>in</strong>, tails you lose' type multiple choice options<br />

● More than one checkbox options for a Q ask<strong>in</strong>g for "one of <strong>the</strong> follow<strong>in</strong>g" (or <strong>the</strong> reverse)<br />

● 'Choose from a list', without an 'o<strong>the</strong>r' option<br />

● <strong>to</strong>o many (or not enough) questions<br />

● 'Choose <strong>the</strong> range' with some options not covered, e.g.:<br />
