(Employee paid benefit-offered to employees with weekly base hours
of 17.5 or greater)

longer perform “Activities of Daily Living” as a result of old age,
chronic illness, or serious injury (i.e. Alzheimer’s)
– Choose from a variety of long-term care plan options to suit your
individual lifestyle
– Confidential consultations are provided to participants to
determine the best plan option
• Purchase “fully-portable” individual policies at discounted group
premiums.
• Empes, spouses, or partners can sign up for or stop participation at
anytime
– Benefit from ‘Simplified Underwriting’ (i.e. fewer health
questions) if you apply within 90 days of hire date or benefits
eligibility date
• Premiums will be deducted from payroll on a ‘post-tax’ basis
