Nearly three quarters <strong>of</strong> a million people<br />

will suffer a stroke this year. And<br />

although most will survive, stroke is the leading<br />

cause <strong>of</strong> adult disability in this country.<br />

Recovery <strong>of</strong>ten is a lifelong and deeply<br />

personal challenge.<br />

“There are lots <strong>of</strong> st<strong>at</strong>istics and studies, but<br />

no one is truly an ‘average’ stroke p<strong>at</strong>ient,”<br />

says Lynne Nicolson, MD, our chief <strong>of</strong><br />

physical medicine and rehabilit<strong>at</strong>ion <strong>at</strong><br />

<strong>Sunnyview</strong> <strong>Rehabilit<strong>at</strong>ion</strong> Hospital. “The<br />

various recovery factors are so highly individual.<br />

Each new p<strong>at</strong>ient has slightly different<br />

physical or cognitive deficits, personal<br />

needs and priorities.”<br />

This is why <strong>Sunnyview</strong>’s inp<strong>at</strong>ient and<br />

outp<strong>at</strong>ient stroke rehabilit<strong>at</strong>ion programs<br />

are so comprehensive in scope and interdisciplinary<br />

in approach … tailored to each<br />

p<strong>at</strong>ient’s needs.<br />

<strong>Stroke</strong> rehabilit<strong>at</strong>ion is a “center <strong>of</strong> excellence”<br />

<strong>at</strong> <strong>Sunnyview</strong>. For more than 40<br />

years, we’ve been the region’s premier<br />

provider <strong>of</strong> comprehensive services.<br />

<strong>Sunnyview</strong> p<strong>at</strong>ients consistently return to<br />

their homes, instead <strong>of</strong> nursing homes,<br />

<strong>at</strong> r<strong>at</strong>es far exceeding regional or n<strong>at</strong>ional<br />

averages. We can think <strong>of</strong> no finer example<br />

than a remarkable man named Angelo —<br />

affection<strong>at</strong>ely known as “the mayor<br />

<strong>of</strong> <strong>Sunnyview</strong>.”<br />

Playing his cards right<br />

Every time 81-year-old Angelo Salamone<br />

plays a game <strong>of</strong> poker these days, he wins.<br />

The secret is not how Angelo plays … but<br />

why. Because for him, poker is more than<br />

a way to have fun and stay in touch with<br />

buddies. It’s therapy.<br />

Angelo first came to <strong>Sunnyview</strong> in 1987<br />

after a debilit<strong>at</strong>ing stroke. He was 60. “The<br />

nurses and therapists and social workers were<br />

so wonderful … I really can’t tell you how<br />

wonderful,” says his wife Rose. “They do everything<br />

here with love.” After three months<br />

<strong>of</strong> inp<strong>at</strong>ient therapy, with steady improvements<br />

in his speech, mobility and motor<br />

skills, Angelo was able to return home.<br />

But within weeks he and Rose were back,<br />

as new members <strong>of</strong> our Post-<strong>Stroke</strong><br />

Support Group.<br />

“The post-stroke group is real outp<strong>at</strong>ient<br />

therapy … the only difference is we get<br />

to play.” Th<strong>at</strong>’s how Connie Todt, CTRS,<br />

director <strong>of</strong> recre<strong>at</strong>ional therapy, explains<br />

one vital aspect <strong>of</strong> the post-stroke program.<br />

Each week, Connie and her <strong>Sunnyview</strong><br />

colleagues cre<strong>at</strong>e a range <strong>of</strong> therapeutic and<br />

social activities: group outings, numerical or<br />

word games — even Karaoke sing-alongs<br />

and Wii video games. One goal is for group<br />

members to exercise their physical and cognitive<br />

abilities. But equally important<br />

<strong>Sunnyview</strong> is the only stroke specialty program in the region<br />

accredited by the Commission on the Accredit<strong>at</strong>ion <strong>of</strong><br />

<strong>Rehabilit<strong>at</strong>ion</strong> Facilities (CARF).<br />

is a lively social environment. This is where<br />

p<strong>at</strong>ients and their families can c<strong>at</strong>ch up<br />

on news, compare notes, get recommend<strong>at</strong>ions,<br />

commiser<strong>at</strong>e and share their every-<br />

day experiences.<br />

“We’re so lucky to have this program,”<br />

says Rose. “Angelo looks forward to it<br />

so much. He’s in his glory there.”<br />

The Post-<strong>Stroke</strong> Support Group is another<br />

innov<strong>at</strong>ive way <strong>Sunnyview</strong> continues<br />

caring for p<strong>at</strong>ients and their families for<br />

months and <strong>of</strong>ten years after they return<br />

home from rehabilit<strong>at</strong>ion.<br />

Angelo bounces back<br />

But one day last year, as Angelo was walking<br />

into his weekly poker game <strong>at</strong> <strong>Sunnyview</strong>,<br />

he suffered a minor stroke — 20 years after<br />

his first one. Within minutes, he was rushed<br />

to the emergency department <strong>at</strong> nearby Ellis<br />

Welcome<br />

The year 2008 began with a victory for<br />

p<strong>at</strong>ients who need access to inp<strong>at</strong>ient rehabilit<strong>at</strong>ion.<br />

Medicare’s 75 percent rule, which had been<br />

scheduled for full implement<strong>at</strong>ion in January<br />

2009, has been a looming challenge for rehabilit<strong>at</strong>ion<br />

hospitals across the country ... thre<strong>at</strong>ening<br />

to prevent access to critically important healthcare<br />

for tens <strong>of</strong> thousands <strong>of</strong> p<strong>at</strong>ients in need <strong>of</strong> intensive<br />

rehabilit<strong>at</strong>ion.<br />

The regul<strong>at</strong>ion stipul<strong>at</strong>ed th<strong>at</strong> 75 percent <strong>of</strong> an<br />

inp<strong>at</strong>ient rehabilit<strong>at</strong>ion hospital’s p<strong>at</strong>ients must<br />

fall within a narrow listing <strong>of</strong> only 13 diagnostic<br />

c<strong>at</strong>egories. If a hospital was to fall below 75 percent, the hospital could lose its design<strong>at</strong>ion<br />

as a rehabilit<strong>at</strong>ion hospital and face significant reductions in reimbursement.<br />

Legisl<strong>at</strong>ion was introduced in both the U.S. House <strong>of</strong> Represent<strong>at</strong>ive and the Sen<strong>at</strong>e<br />

to permanently establish 75 percent rule relief <strong>at</strong> 60 percent ... legisl<strong>at</strong>ion th<strong>at</strong> Congress<br />

passed into law in December. I am proud to say th<strong>at</strong> we worked diligently for<br />

this legisl<strong>at</strong>ion, in conjunction with the American Medical <strong>Rehabilit<strong>at</strong>ion</strong> Providers<br />

Associ<strong>at</strong>ion (AMRPA), the American Hospital Associ<strong>at</strong>ion (AHA) and the <strong>Health</strong>care<br />

Associ<strong>at</strong>ion <strong>of</strong> New York St<strong>at</strong>e (HANYS). Thanks go to these organiz<strong>at</strong>ions for<br />

their hard work in helping get this legisl<strong>at</strong>ion passed, as well to our U.S. legisl<strong>at</strong>ors,<br />

especially Congresswoman Nita M. Lowey, who introduced the legisl<strong>at</strong>ion, and<br />

Sen<strong>at</strong>or Charles E. Schumer who was a strong proponent <strong>of</strong> 75 percent rule relief.<br />

While this was a tremendous victory, it comes <strong>at</strong> a cost, since we will see no Medicare<br />

r<strong>at</strong>e increases for 18 months. We still face enormous challenges in the field <strong>of</strong><br />

inp<strong>at</strong>ient rehabilit<strong>at</strong>ion. I look forward to continuing to work with AMRPA, AHA<br />

and HANYS as we advoc<strong>at</strong>e for wh<strong>at</strong> people with disabilities need in their medical<br />

rehabilit<strong>at</strong>ion recovery.<br />

On a positive note, we have exciting things happening <strong>at</strong> <strong>Sunnyview</strong> this year. With<br />

construction <strong>of</strong> Eddy Village Green on the Cohoes campus this fall, Eddy Cohoes<br />

<strong>Rehabilit<strong>at</strong>ion</strong> Center will be transitioning its rehabilit<strong>at</strong>ion beds to <strong>Sunnyview</strong>. We<br />

will be welcoming staff from Eddy Cohoes, as well as many new p<strong>at</strong>ients who will<br />

experience <strong>Sunnyview</strong> for the first time. We are proud <strong>of</strong> the outstanding rehabilit<strong>at</strong>ive<br />

care th<strong>at</strong> we provide and look forward to providing th<strong>at</strong> care to even more<br />

residents in our service area <strong>of</strong> upst<strong>at</strong>e New York and New England in the coming<br />

months and years.<br />

physical therapy (2002) from Quinnipiac College in CT. Kristen<br />

has five years <strong>of</strong> extensive experience working with neurologically<br />

impaired p<strong>at</strong>ients and currently works on the inp<strong>at</strong>ient brain injury<br />

program. She has additional training in neurologic developmental<br />

tre<strong>at</strong>ment (NDT), constraint induced movement therapy, and<br />

the StimMaster functional electric stimul<strong>at</strong>ion bike for spinal<br />

cord injured p<strong>at</strong>ients.<br />

The <strong>Sunnyview</strong> <strong>Rehabilit<strong>at</strong>ion</strong> Hospital Found<strong>at</strong>ion is currently in<br />

the process <strong>of</strong> raising funds to bring the Bioness H200 and Bioness<br />

L300 systems to <strong>Sunnyview</strong> (see article below). The devices will be<br />

<strong>Sunnyview</strong> Looks to New Bioness System<br />

In 2004, a group <strong>of</strong> visionary leaders established Bioness Inc.<br />

to help individuals with neurological impairments regain their<br />

independence. Bioness <strong>of</strong>fers two award-winning medical devices<br />

designed to benefit people with stroke, multiple sclerosis, traum<strong>at</strong>ic<br />

brain injury, cerebral palsy and spinal cord injury. These noninvasive<br />

devices, based on functional electrical stimul<strong>at</strong>ion (FES), help<br />

people regain mobility and independence, and improve quality<br />

<strong>of</strong> life and productivity.<br />

The two devices include:<br />

• the Bioness H200 Hand <strong>Rehabilit<strong>at</strong>ion</strong> System helps those with<br />

upper limb impairments. Utilizing mild electrical stimul<strong>at</strong>ion,<br />

the system helps improve the way an individual’s arm works,<br />

used for both lower extremity rehabilit<strong>at</strong>ion in conjunction with<br />

traditional physical therapy approaches, as well as upper extremity<br />

rehabilit<strong>at</strong>ion to improve quality <strong>of</strong> life. The program will target inp<strong>at</strong>ient<br />

and outp<strong>at</strong>ient clients tre<strong>at</strong>ed by <strong>Sunnyview</strong> and will bring<br />

st<strong>at</strong>e-<strong>of</strong>-the-art therapy to help in the functional improvement <strong>of</strong><br />

our p<strong>at</strong>ients.<br />

Funds from the scholarship will allow Becky and Kristen to <strong>at</strong>tend<br />

an extensive Bioness training program.<br />

Credit Union Don<strong>at</strong>ion<br />

<strong>Sunnyview</strong> <strong>Rehabilit<strong>at</strong>ion</strong> Hospital recently purchased two<br />

Nintendo Wii systems, televisions and stands, as well as the<br />

Bruininks-Oseretsky Test <strong>of</strong> Motor Pr<strong>of</strong>iciency (BOT2), thanks<br />

to the generosity the “Credit Union Care for Kids Campaign 2007,”<br />

a st<strong>at</strong>ewide community involvement program <strong>of</strong> the New York<br />

St<strong>at</strong>e Credit Union League (NYSCUL).<br />

and can help p<strong>at</strong>ients get the most out <strong>of</strong> therapy, and reduce<br />

complic<strong>at</strong>ions including muscle spasms;<br />

• the Bioness L300 Foot Drop System assists those with foot<br />

drop regain a more normal gait. The advanced foot drop system<br />

uses mild stimul<strong>at</strong>ion to help an individual lift his/her foot,<br />

so th<strong>at</strong> he/she can walk more safely and easily. The L300 may<br />

even elimin<strong>at</strong>e the need to wear a rigid, heavy orthosis.<br />

needed equipment. Every item from the<br />

wish list th<strong>at</strong> the <strong>Sunnyview</strong> Hospital<br />

Found<strong>at</strong>ion is able to fill is one less strain<br />

on our not-for-pr<strong>of</strong>it budget.<br />

Cardiopulmonary $28,000<br />

To complete the $150,000 purchase <strong>of</strong><br />

medical telemetry for our cardiopulmonary<br />

p<strong>at</strong>ients. Obtaining st<strong>at</strong>e-<strong>of</strong>-the-art telemetry<br />

will enable <strong>Sunnyview</strong> to continue<br />

to provide prompt rehabilit<strong>at</strong>ion services<br />

to individuals who are <strong>at</strong> risk for cardiac<br />

arrhythmias on any unit <strong>of</strong> the hospital.<br />

Without this capability, individuals would<br />

be restricted to certain units and equipment.<br />

Physical Therapy/<br />

Occup<strong>at</strong>ional Therapy $100,000<br />

To enable the departments to purchase the<br />

Bioness system, a st<strong>at</strong>e-<strong>of</strong>-the-art functional<br />

electrical stimul<strong>at</strong>ion (FES) device used<br />

to tre<strong>at</strong> a number <strong>of</strong> conditions including<br />

stroke, traum<strong>at</strong>ic brain injury, multiple<br />

sclerosis and incomplete spinal cord injury.<br />

Neurovascular $11,480<br />

The use <strong>of</strong> a portable ultrasound bladder<br />

scanner is becoming more common in<br />

the rehabilit<strong>at</strong>ion setting as a non-invasive<br />

method <strong>of</strong> estim<strong>at</strong>ing bladder volume. Bladder<br />

scanners are used to screen p<strong>at</strong>ients for<br />

post-void residual volumes and to determine<br />

the need for intermittent c<strong>at</strong>heteriz<strong>at</strong>ion<br />

based on volume r<strong>at</strong>her than time. Benefits<br />

include a decreased number <strong>of</strong> invasive<br />

c<strong>at</strong>heteriz<strong>at</strong>ions, thus decreasing the potential<br />

for urinary tract infections and urethral<br />

trauma, and increasing p<strong>at</strong>ient s<strong>at</strong>isfaction<br />

and comfort. The scanner would be used<br />

by approxim<strong>at</strong>ely 520 p<strong>at</strong>ients per year and<br />

could be used up to four times a day per<br />

p<strong>at</strong>ient. The cost associ<strong>at</strong>ed includes the<br />

cart for portability.<br />

Hospital Wide $3,625 each (45 needed)<br />

<strong>Sunnyview</strong> has used low position beds successfully<br />

to minimize the potential for injury<br />

with p<strong>at</strong>ients who are <strong>at</strong> a risk <strong>of</strong> falling.<br />

Increasing the number <strong>of</strong> beds will allow<br />

<strong>Sunnyview</strong> to meet peak demand when<br />

there is a large number <strong>of</strong> <strong>at</strong>-risk inp<strong>at</strong>ients.<br />

Orthopedics $1,625 (for two)<br />

The Rancho Multi-Link Mobile Arm<br />

Support system allows p<strong>at</strong>ients with spinal<br />

cord injury, traum<strong>at</strong>ic brain injury, multiple<br />

sclerosis and stroke with significant arm<br />

weakness to self feed, perform light hygiene<br />

tasks and light recre<strong>at</strong>ional tasks.<br />

Audiology $4,500<br />

The Audiology OAE (Otoaccoustic Emissions)<br />

Screener is used for infant hearing<br />

screenings. Otoaccoustic emissions are very<br />

small sounds produced by a healthy human<br />

cochlea. The cochlea takes an incoming<br />

acoustic signal and sends inform<strong>at</strong>ion to<br />

the brain. In doing so, it produces distortion<br />

th<strong>at</strong> can be measured objectively. Our<br />

current equipment is outd<strong>at</strong>ed and can no<br />

longer be supported.<br />

Nursing $15,600<br />

To purchase IV pumps and poles which<br />

will safely and accur<strong>at</strong>ely infuse medic<strong>at</strong>ions<br />

to p<strong>at</strong>ients.<br />

Annual Fund Contributors<br />

Wellness Center $2,200<br />

To purchase a LifeSource blood pressure<br />

monitor. This monitor will allow Wellness<br />

Center members to take their own blood<br />

pressure <strong>at</strong> any time.<br />

Speech $2,598<br />

Augment<strong>at</strong>ive communic<strong>at</strong>ions has become<br />

an integral component <strong>of</strong> services we <strong>of</strong>fer<br />

<strong>at</strong> <strong>Sunnyview</strong>. Often, our p<strong>at</strong>ients have acquired<br />

a medical disease or condition th<strong>at</strong> has<br />

limited the effectiveness <strong>of</strong> their spoken and<br />

written communic<strong>at</strong>ion. We wish to purchase<br />

both low and high technology systems th<strong>at</strong><br />

will facilit<strong>at</strong>e our p<strong>at</strong>ients being able to communic<strong>at</strong>e<br />

on either a verbal or written level.<br />

Nursing – Cardiopulmonary $4,300 (for two)<br />

The D<strong>at</strong>ex Ohmeda Pulse Oximeter 3900<br />

provides accur<strong>at</strong>e readings for oxygen levels<br />

in the blood. It has a continuous print out<br />

capability for document<strong>at</strong>ion and provides<br />

the ability to store readings in memory<br />

which is useful when performing continuous<br />

pulse oximetry readings for sleep studies.<br />

The equipment allows sleep studies to be<br />

performed which serves as document<strong>at</strong>ion<br />

to justify oxygen needs for home coverage.<br />

It also identifies sleep apnea episodes allowing<br />

us to provide the best possible tre<strong>at</strong>ment<br />

to p<strong>at</strong>ients requiring interventions for<br />

this diagnosis.<br />

<strong>Sunnyview</strong> Junior Volunteers Attend<br />

Disney Youth Leadership Conference<br />

Anyone who’s been involved with<br />

<strong>Sunnyview</strong> knows the important<br />

role th<strong>at</strong> junior volunteers play in helping<br />

various hospital departments.<br />

“I am always looking for ways to enhance<br />

our junior volunteer program,” said Joan<br />

Laviolette, <strong>Sunnyview</strong>’s director <strong>of</strong> volunteer<br />

services. After much research, she concluded<br />

th<strong>at</strong> the Disney Youth Leadership Program<br />

held in Orlando, FL, would <strong>of</strong>fer the teens<br />

a development opportunity th<strong>at</strong> would<br />

help them grow as volunteers, students<br />

and young citizens.<br />

“Many <strong>of</strong> our junior volunteers excel in<br />

school and community activities,” she<br />

added. “This program not only teaches<br />

leadership skills, it also provides hands-<br />

on experience in customer service ...<br />

Disney-style!”<br />

The program focuses on different aspects<br />

<strong>of</strong> leadership, including understanding<br />

conflict, cooper<strong>at</strong>ion and interdependence<br />

among individuals, groups and institutions.<br />

in an experiential component to practice the<br />

concepts they have learned.<br />

<strong>Sunnyview</strong> junior volunteers were able to<br />

take part in the recent leadership program<br />

thanks to fundraisers and the support <strong>of</strong><br />

parents/guardians. Traveling to Disney<br />

World were: Eli Feldman, Marissa Debiase,<br />

Christian Forte, Jessica Mazzo, Carly Snow,<br />

Rebecca Pike, Philip Mascitelli, Noelle<br />

Vetro, Stephanie Todt and Krista Carroll.<br />

Ten parents and guardians were able to<br />

particip<strong>at</strong>e, along with Joan, K<strong>at</strong>hie Boyle,<br />

volunteer coordin<strong>at</strong>or, and Connie Todt,<br />

Physical Therapy Department Acquires Pressure Mapping Equipment<br />

In February, <strong>Sunnyview</strong> <strong>Rehabilit<strong>at</strong>ion</strong> Hospital became the first<br />

area acute rehabilit<strong>at</strong>ion facility to acquire a new pressure mapping<br />

system th<strong>at</strong> helps clinicians determine the suitability <strong>of</strong> a wheelchair<br />

cushion and/or to justify to insurance companies why certain<br />

wheelchair fe<strong>at</strong>ures would be most beneficial to specific p<strong>at</strong>ients.<br />

According to P<strong>at</strong>ricia Valenza, PT, “pressure mapping utilizes sensors<br />

th<strong>at</strong> measure pressure <strong>at</strong> different parts <strong>of</strong> the sitting surface.”<br />

She notes th<strong>at</strong> a thin m<strong>at</strong>, measuring approxim<strong>at</strong>ely 18 inches by<br />

18 inches, contains a m<strong>at</strong>rix <strong>of</strong> small sensors. “When a p<strong>at</strong>ient sits<br />

on the m<strong>at</strong>, the sensors read pressure <strong>at</strong> individual loc<strong>at</strong>ions on the<br />

thighs and buttocks. This d<strong>at</strong>a is transferred to a computer where<br />

The cardiopulmonary department recently acquired the new,<br />

st<strong>at</strong>e-<strong>of</strong>-the-art GE ApexPro CH telemetry system, the most thorough<br />

and reliable wireless p<strong>at</strong>ient monitoring available today.<br />

“Telemetry monitors a p<strong>at</strong>ient’s heart rhythm, almost like a continuous<br />

EKG,” says Deb Dross, RN, nurse manager. She notes<br />

th<strong>at</strong> “the new system gre<strong>at</strong>ly increases quality <strong>of</strong> care for our<br />

p<strong>at</strong>ients, especially since it enables our nurses to monitor p<strong>at</strong>ients<br />

from two loc<strong>at</strong>ions, r<strong>at</strong>her than just one.” She explained th<strong>at</strong> with<br />

the new system, p<strong>at</strong>ients can move freely throughout the unit,<br />

as well onto other units within the hospital.<br />

In addition to p<strong>at</strong>ients with congestive heart failure and those<br />

with a recent heart <strong>at</strong>tack, the 19-bed unit cares for p<strong>at</strong>ients recovering<br />

from cardiac surgery. “As an inp<strong>at</strong>ient cardiac and pulmonary<br />

rehabilit<strong>at</strong>ion unit, the telemetry system helps us monitor our<br />

p<strong>at</strong>ients’ endurance during the rehabilit<strong>at</strong>ion process.”<br />

Don<strong>at</strong>ions to the <strong>Sunnyview</strong> Found<strong>at</strong>ion made purchase <strong>of</strong> the<br />

equipment possible.<br />

Hospital. And five days l<strong>at</strong>er, Angelo was back <strong>at</strong> <strong>Sunnyview</strong><br />

for three weeks <strong>of</strong> inp<strong>at</strong>ient rehabilit<strong>at</strong>ion.<br />

“The second stroke left him a little weaker,” says Connie, “but<br />

he bounced back strong. Our goal was to get Angelo back into<br />

the support group as quickly as possible. We knew how important<br />

th<strong>at</strong> was to him.”<br />

“Th<strong>at</strong> group helped tremendously,” says Rose. “Even when he<br />

was still doing inp<strong>at</strong>ient rehab, they would bring him downstairs<br />

we can analyze it and determine where pressure sores may occur,”<br />

says P<strong>at</strong>ricia.<br />

“We also use the system as a p<strong>at</strong>ient educ<strong>at</strong>ion tool ... p<strong>at</strong>ients can<br />

see on the computer screen how shifting body weight can allevi<strong>at</strong>e<br />

undue pressure on specific areas <strong>of</strong> the body, which in turn helps<br />

allevi<strong>at</strong>e the risk for pressure sores and skin breakdown,” she adds.<br />

P<strong>at</strong>ients appropri<strong>at</strong>e for pressure mapping are individuals who<br />

spend a good part <strong>of</strong> their day in a wheelchair — those with<br />

spinal cord injuries, multiple sclerosis, stroke or brain injury.<br />

The $7,000 Vista Medical FSA system was made possible<br />

through a gift to the <strong>Sunnyview</strong> Found<strong>at</strong>ion.<br />

The therapeutic recre<strong>at</strong>ion department <strong>of</strong>fers a series <strong>of</strong> clinics<br />

and classes for people with disabilities, providing opportunities<br />

to explore and particip<strong>at</strong>e in sports and leisure activities, learn<br />

new skills and gain self confidence.<br />
