INTRODUCTION

The School Report Card (SRC) will provide key statement on the outcomes expect

from schools and the balance of priorities between them, ensuring more intelligent accountability

across schools’ full range of responsibilities.

The SRC will report on outcomes across the breadth of school performance: Pupil

attainment, progress, and well-being; a school’s success in reducing the impact of disadvantage;

and parents’ and pupils’ views of the school and the support they are receiving.

The SRC is a comprehensive yet concise reporting tool. It has three parts: 1) school

profile; 2) performance indicators measuring aspects of access, quality, and governance;

and 3) status of school projects. In consideration of the unique needs and context of

each school, other information aside from those indicated in these guidelines may be included

according to what the school thinks is necessary to share to its stakeholders.

School Report Card is not for planning and accountability; it is a communication

and advocacy tool intended for internal and external stakeholders. The member of the

School Planning Team is composed of the Team Leader, Teacher Representative, the

GPTA President and her counterpart, Officers of the Supreme Pupil Government

Organization (SPGO), Disaster Risk Reduction and Management Council, School Child

Protection Committee, Documentation, Encoder and other stakeholders convene to discuss

important matters regarding how to run the school for the next three years.

Furthermore, the data on the SRC are bases for the School Improvement Plan.

Kalayaan Elementary School have a very cooperative and supportive

stakeholders. Petron, Gems Heart, Spring Board Foundation, British International School

of Manila, Sunshine Kids, Johnsons & Johnsons, KES Multi-purpose Cooperative, School

GPTA and Barangay 201 Court. They’ll give them assistance to our pupils. Donated of

bags and school supplies, gift-giving Christmas package every Christmas, groceries and

feeding program for the selected beneficiaries.

Another project of the school is the “Gulayan sa Paaralan” that teach the learners

on how to grow vegetables in their own backyards.

The Schools Division Office (SDO) conducted a variety of trainings and seminars

which gave the teachers an upgrade in their professional growth.

The LGUs had also made remarkable accomplishments for the school year like

painting of school building, ALS, Remedial Reading, different school equipments,

medals, salary of utilities, security guards, driver, school clerk, school

nurse, COLA, groceries, school supplies and allowance of the players and

coaches in the Sports Fest Palaro.

Brigada for Every Child Reader Program

Our school gives emphasis on the importance of reading being a core

skill. The school aims for zero non-reader, but the data revealed in the

reading level of proficiency that there are still many pupils that fall into C 7

to C6 or Classified as Letter reader and Syllable reader.

It appears that the number of enrolees have significantly

decreased for 2020-2021 up to present due to several causes like

financial problems, child labor, health concerns poor study habits,

lack of motivation and lack of interest to learn

The health and nutritional status of the pupils is determined through weighing

and getting the height of every learner. The measuring of height and weight is done

during the first month of the school year and another one is being done before the

school year ends.

There are 2,105 pupils in the primary grade. About 77 of the primary grade

learners are obese, 186 are overweight, 1,625 are in the normal status, 83 are wasted

and 580pupils are severely wasted.

In the intermediate level, out of 1,571 pupils, there are 56, learners who are

obese, 184 are overweight, 1,198 are in the normal status, 74 are wasted, and 24 are

severely wasted.

The school has a project of Gulayan sa Paaralan, which is a big help in the

school’s feeding program. Foundations like SUNSHINE and PETRON are also continuously

supporting the feeding program of the school and they’re doing all these

efforts for those learners that fall outside the normal health status.

There are enough

seats for all learners.

Summative Test was given twice a year.

that there is a decrease by 0.52% in the SY 2020

compared to the previous school year which has a

0.78% increase.

A number of pupils from the

different grade levels were not promoted

due to certain reasons like family problem,

parental involvement in the childs learning,

inattentiveness and lack of interest to learn.
