Primary hepatic neoplasms, constituting less than two percent of all pediatric tumors, are the major indication for resection [2, 4, 5, 14]. Approximately three quarters of all liver lesions are malignant. Epithelial tumors, hepatoblastoma and hepatocarcinoma, are the most common and account for 90% of hepatic malignancies. One third of childhood liver tumors are benign. They include vascular malformations, adenomas, focal nodular hyperplasia, mesenchymal hamartomas and various Address for correspondence Hepatoblastoma, hepatocarcinoma and, much less often, focal nodular hyperplasia and mesenchymal hamartoma are the major indications for liver resection. This paper presents the outcome for children treated surgically because of liver tumors. Between January 1999 and July 2006, 13 partial hepatic resections were performed in the Department of Paediatric Surgery Children's Hospital, Jagiellonian University. Indications were hepatoblastoma in 8 patients (62%), focal nodular hyperplasia in 2 (15%), mesenchymal hamartoma in 2 (15%), and metastatic nephroblastoma in 1 (8%). In 6 patients (46%) right hemihepatectomy was performed, in 2 (15%) extended right hemihepatectomy, in 1 (8%) – right hemihepatectomy with lateral part of the left lobe, in 3 patients (23%) left hemihepatectomy and in 1 (8%) posterio-lateral portion of the right lobe was resected. There were no perioperative deaths. Eleven patients, including all of the patients with benign lesions, continue to live and develop normally without signs of illness. One child with hepatoblastoma died three months after surgery, there is one recurrence of hepatoblastoma. Six patients after treatment of hepatoblastoma are disease free with median follow up of 56 months. Surgery is the treatment of choice in resectable hepatic lesions. In the children with malignant tumors the chemotherapy, followed by surgery gives a big chance of survival. Taking into account the low risk of complications of liver resections and good outcomes associated therewith, we consider hepatic resection a better choice for children with mesenchymal hamartoma and progressing FNH than the risk of malignant transformation and progression possibly requiring hepatic transplantation in the future. Nowadays thanks to progress in understanding of hepatic anatomy, achievements in pre and postoperative multi drug chemotherapy, survival of children with hepatic malignant tumors has improved [2, 4, 12, 14]. Hepatic resection still remains a challenge for pediatric surgeons due to location and risk of surgical complications. However, in specialized centers good results of hepatic resections encourage to operate not only malignant but also potentially dangerous benign lesions such as mesenchymal hamartoma and selected cases of focal nodular hyperplasia (FNH) [15]. This article presents our experience with partial hepatic resection in thirteen consecutive patients during seven years period.
