Assessment. Frequently asked questions (FAQs) What were the drivers for changes to the assessment process in Foundation? The assessment process in Foundation has been changed to address the concerns raised in Foundation for Excellence, w hich w as c ommissioned by M edical Ed ucation England. Professor J ohn C ollins, w ho l ed t he re view o f t he Fo undation P rogramme, re commended that 20: The range of assessment tools and the number of times assessment must be repeated in the Foundation Programme should be reviewed, with a view to reducing these to the minimum required by 2013. The opportunity to avoid repetitive assessments, by improved transfer of information between undergraduate and schools, should be actively explored. How has the assessment process changed? There has been a r adical r evision a nd c larification o f the as sessment process w ithin foundation training. The following to ols ar e n o l onger us ed as formal as sessments i n t he F oundation Programme: � Mini-clinical evaluation exercise (mini-CEX) � Direct observation of procedural skills (DOPS) � Case-based discussion (CBD) � Developing the clinical teacher. Please r efer to t he supervised learning e vent ( SLE) g uidance for further details on t hese tools. Formal assessment will now occur a t t he e nd o f each p lacement a nd a t the e nd o f e ach year. T he clin ical s upervisor and t he educational su pervisor will p repare re ports o n t he foundation doctor’s performance in the workplace. The reports will be informed by review of the foundation doctor’s e-portfolio including consideration of completion of core procedures (F1), t eam as sessment of be haviour ( TAB) and feedback from th e p lacement s upervision group. What is the Placement Supervision Group? The Placement Supervision Group consists of trainers nominated in each placement by the named clin ical su pervisor. T heir observations a nd feedback will in form t he clin ical supervisor’s end of placement report. The makeup of the Placement Supervision Group will vary depending on the placement but could include: � Doctors more senior than F2, including at least one consultant or GP principal � Senior nurses (band 5 or above) � Al lied health professionals. The Placement Supervision Group is responsible for: � observing the foundation doctor’s performance in the workplace � providing feedback on practice to the foundation doctor � providing structured feedback to the named clinical supervisor � undertaking and facilitating supervised learning events (SLEs). What do assessments entail? Foundation doctors are expected to demonstrate achievement for each outcome described in the Foundation Programme Curriculum 2012 (the C urriculum). F oundation d octors ar e expected to r ecord t heir ac hievements and evidence o f l earning w ithin their e- portfolio. However they are not expected to demonstrate that they have met every single competence 5
listed below each outcome. The assessment process is not designed to rank performance against other foundation doctors. Are core procedures still part of F1 assessment? Yes. The GMC requires demonstration of competence in a series of procedures in order for a provisionally registered doctor with a licence to practise to be eligible for full registration. Assessment Frequency Core procedures Team assessment of behaviour (TAB) Clinical supervisor’s end of placement report Throughout F1 Educational supervisor’s end of placement report Once per placement Educational supervisor’s end of year report Once in first placement in both F1 and F2, optional repetition Once per placement Once per year All of these assessments should be recorded in the e-portfolio. The e-portfolio, as a whole, will b e scru tinised to i nform t he educational s upervisors’ ju dgements. I n p articular, t he educational supervisor will consider whether the foundation doctor has provided evidence of engagement and achievement for each outcome listed in the Curriculum. What do the different assessment tools do? Core procedures It is a GM C requirement that foundation doctors provide evidence within their e-portfolio of satisfactory performance of each core procedure at least once during F1. By the end of F1, the foundation d octor s hould be able t o c ompetently p erform and t each undergraduates these 15 procedures. The core procedures from F1 do not need to be repeated in F2, but evidence of the F1 sign off i s r equired for s uccessful c ompletion of t he Foundation P rogramme. It s hould also be recognised t hat wit h p ractice, t he d octor will b e e xpected t o d emonstrate co ntinuing improvement o f skills in wh ichever p rocedure t hey p erform, wit hin t he s piral c urriculum framework. Team assessment of behaviour (TAB) This is a type of Multi-Source Feedback, previously known as 360 degree assessment. Prior to inviting raters to contribute to the TAB process, foundation doctors must complete a self-assessment of behaviour (self-TAB). This includes reflection of their own performance. TAB c omprises c ollated v iews from a r ange o f m ulti-professional c olleagues. T he s ame sections are used in both the self-assessment and the rater-completed forms � TAB must take place at least once a year. Deaneries/foundation schools have the option of increasing the frequency 6
