� It is suggested that both F1 and F2 TAB is taken in the last month of the first
placement during the year. If there are significant concerns about any foundation
doctor, TAB should be repeated. Deaneries have the option of altering the timing
of TAB to satisfy local needs
� For eac h as sessment, t he foundation d octor a nd t he e ducational s upervisor
should agree 1 5 r aters/assessors. A m inimum of 10 r eturns are r equired. N o
other foundation doctor can be a rater.
The required mix of raters/assessors must include at least two of each of the following:
� Doctors more senior than F2, including at least one consultant or GP principal
� Senior nurses (band 5 or above)
� Al lied health professionals
� Other team members including ward clerks, secretaries and auxiliary staff.
Following T AB, t he foundation doctor s hould r eflect o n a ny s ections i n w hich there i s
variance be tween t heir s elf r ating a nd t hat of t he as sessors. T he doctor s hould di scuss
significant discrepancies with their educational supervisor.
End of placement reports
There are two end of placement reports:
a) clinical supervisor’s end of placement report
b) educational supervisor’s end of placement report
The c linical s upervisor’s en d o f pl acement r eport d escribes th e foundation doctor’s
performance i n t he w orkplace. T he e ducational s upervisor’s r eport i ncorporates the
information contained in the clinical supervisor’s report and includes information from the eportfolio.
If th e e ducational and c linical s upervisors ar e one an d t he s ame, t hen t he educational
supervisor will b e r esponsible for t he s ections t hat a re u sually c overed i n t he clin ical
supervisor’s report.
a) Clinical supervisor’s end of placement report
Towards the end of each placement, the foundation doctor should meet with their clinical
supervisor to complete a summative assessment of their overall performance and progress
in the placement.
The clinical supervisor’s report should comment specifically on:
� Any noteworthy aspect of performance
� Any concerns regarding performance
� Comments on participation in the agreed educational process
� Evidence of professional development as a result of feedback and reflection.
The c linical s upervisor s hould seek a nd r ecord evidence from the Placement Supervision
Group to c orroborate each o f t he a bove. T he names of th ose c ontributing e vidence on
performance is recorded in the report.
The outcome of the final assessment discussion should be agreed by both the foundation
doctor a nd t he c linical s upervisor an d r ecorded i n th e e- portfolio as part of t he c linical
supervisor’s end of placement report.
b) Educational supervisor’s end of placement report
The educational s upervisor’s e nd of placement r eport r equires r eview o f t he c linical
supervisor’s report along with evidence provided within the e-portfolio and any other source.
