ANWENDUNG<br />
Für <strong>de</strong>n schnellen Nachweis <strong>de</strong>s Schwangerschaftshormons <strong>hCG</strong> im Urin und im Serum.<br />
Dieser Test liefert ein optisches qualitatives Ergebnis und ist für <strong>de</strong>n professionellen Einsatz<br />
entwickelt wor<strong>de</strong>n.<br />
ZUSAMMENFASSUNG<br />
Humanes Choriongonadotropin (<strong>hCG</strong>) ist ein Glycoprotein Hormon, das bereits kurz nach<br />
<strong>de</strong>r Befruchtung von <strong>de</strong>r Plazenta gebil<strong>de</strong>t wird. Im Verlaufe einer normalen Schwangerschaft<br />
kann <strong>hCG</strong> schon sieben Tage nach <strong>de</strong>r Empfängnis im Urin o<strong>de</strong>r im Serum nachgewiesen<br />
wer<strong>de</strong>n (14). Die <strong>hCG</strong>-Konzentration steigt sehr rasch an, erreicht mit <strong>de</strong>m ersten<br />
Ausbleiben <strong>de</strong>r Regelblutung häufig einen Wert von über 100 mIU/mL (2-5) und gipfelt<br />
schließlich in <strong>de</strong>r 10. bis 12. Schwangerschaftswoche in einem Bereich von 30000 - 100000<br />
mIU/mL. Das frühzeitige Auftauchen <strong>de</strong>s <strong>hCG</strong> nach <strong>de</strong>r Befruchtung und <strong>de</strong>r darauf folgen<strong>de</strong><br />
Konzentrationsanstieg im Frühstadium <strong>de</strong>r Schwangerschaft machen es zu einem hervorragend<br />
geeigneten Indikator für <strong>de</strong>n frühen Nachweis einer Schwangerschaft.<br />
Der BioNexia ® <strong>hCG</strong> Combo ist für <strong>de</strong>n schnellen Nachweis <strong>de</strong>s Schwangerschaftshormons<br />
<strong>hCG</strong> in Urin- o<strong>de</strong>r Serumproben mit einer Sensitivität von 20 mIU/mL entwickelt wor<strong>de</strong>n.<br />
Der Test bedient sich monoklonaler Antikörper um <strong>hCG</strong>-Konzentration im Urin o<strong>de</strong>r Serum<br />
zu ermitteln. Der BioNexia ® <strong>hCG</strong> Combo zeigt keine Kreuzreaktivität zu strukturell verwandten<br />
Hormonen (FSH, LH, TSH) bis weit über <strong>de</strong>n zu erwarten<strong>de</strong>n maximalen physiologischen<br />
Konzentrationen.<br />
TESTPRINZIP<br />
Der BioNexia ® <strong>hCG</strong> Combo ist ein chromatographischer immunchemischer Test (Chromatographic<br />
Immunoassay, CIA) für <strong>de</strong>n schnellen qualitativen Nachweis von <strong>hCG</strong> in Urin- und<br />
Serumproben. An die Membran sind in <strong>de</strong>r Testregion Antikörper gegen die α-Kette <strong>de</strong>s<br />
<strong>hCG</strong> und in <strong>de</strong>r Kontrollregion Ziege-Anti-Kaninchen-Antikörper gebun<strong>de</strong>n.<br />
Während <strong>de</strong>s Tests reagiert die Probe zunächst mit gold-gekoppelten monoklonalen Antikörpern<br />
gegen die β-Kette <strong>de</strong>s <strong>hCG</strong>. Anschließend wer<strong>de</strong>n alle Partikel durch die Kapillarkräfte<br />
mit <strong>de</strong>r Flüssigkeit über die Membran transportiert.<br />
Bei einem positiven Ergebnis, d. h. bei Anwesenheit von <strong>hCG</strong>, bil<strong>de</strong>t sich ein Antikörper<strong>hCG</strong>-Antikörper-Gold-Aggregat<br />
und damit eine farbige Ban<strong>de</strong> in <strong>de</strong>r Testregion. Erscheint<br />
keine farbige Ban<strong>de</strong> in <strong>de</strong>r Testregion, ist das Testergebnis negativ. In <strong>de</strong>r Kontrollregion<br />
sollte in je<strong>de</strong>m Fall eine farbige Ban<strong>de</strong> zu sehen sein. Sie zeigt die erfolgreiche Durchführung<br />
<strong>de</strong>s Tests an.<br />
AUFBAU DER TESTKASSETTE<br />
Das Plastikgehäuse <strong>de</strong>r Testkassette enthält <strong>de</strong>n Teststreifen mit <strong>de</strong>r Membran und <strong>de</strong>n Antikörpern.<br />
Am rechtem En<strong>de</strong> <strong>de</strong>r Testkassette befin<strong>de</strong>t sich die run<strong>de</strong> Probennahmeöffnung<br />
und im linken Teil das rechteckige Reaktionsfeldfenster. Im Reaktionsfeld <strong>de</strong>s Teststreifens<br />
befin<strong>de</strong>t sich eine Testregion und die Kontrollregion. Die Beschriftung neben <strong>de</strong>m Fenster<br />
markiert die jeweilige Test- (T) und Kontrollregion (C).<br />

LAGERUNG UND HALTBARKEIT<br />
Der Test sollte während <strong>de</strong>r angegebenen Haltbarkeitszeit in <strong>de</strong>r Originalverpackung gekühlt<br />
bei 2-8°C o<strong>de</strong>r bei Raumtemperatur (nicht über 30°C) aufbewahrt wer<strong>de</strong>n.<br />

· Test umgehend nach Öffnung <strong>de</strong>r Folienverpackung verwen<strong>de</strong>n.<br />
· Bitte beachten Sie die angegebenen Auswertungszeiten (3 bis 5 Minuten).<br />
· Test nur unter <strong>de</strong>n angegebenen Temperaturen lagern und transportieren.<br />
PROBENAHME UND PROBENLAGERUNG<br />
Urinproben:<br />
· Die Urinprobe sollte in einem sauberen trockenen Gefäß aufgefangen wer<strong>de</strong>n. Es können<br />
Proben zu je<strong>de</strong>r Tageszeit gesammelt wer<strong>de</strong>n; jedoch enthält <strong>de</strong>r Morgenurin im Allgemeinen<br />
die höchste Hormonkonzentration.<br />
· Vor Durchführung <strong>de</strong>s Versuches müssen Probe und Test auf Zimmertemperatur gebracht<br />
wer<strong>de</strong>n. Proben mit einer sichtbaren Trübung sollten zentrifugiert, filtriert o<strong>de</strong>r bis zum<br />
Absetzen <strong>de</strong>r Trübung stehen gelassen wer<strong>de</strong>n. Nur <strong>de</strong>r klare Überstand sollte für <strong>de</strong>n<br />
Test verwen<strong>de</strong>t wer<strong>de</strong>n.<br />
· Urinproben können bei 2-8°C maximal 48 Stun<strong>de</strong>n bis zur Durchführung <strong>de</strong>s Versuches<br />
aufbewahrt wer<strong>de</strong>n. Für spätere Benutzungen ist es nötig, die Proben bei min<strong>de</strong>stens<br />
-20°C zwischenzeitlich zu lagern.<br />
Serumproben:<br />
· Sammeln Sie venöses Blut aseptisch in ein sauberes heparinisiertes o<strong>de</strong>r EDTA enthaltenes<br />
Gefäß.<br />
· Trennen Sie Serum o<strong>de</strong>r Plasma so schnell wie möglich vom Blut, um die Hämolyse zu vermei<strong>de</strong>n.<br />
Verwen<strong>de</strong>n Sie nur klare und nicht hämolysierte Proben. Die Proben, die feste Partikel<br />
enthalten, sollten vor <strong>de</strong>m Testen durch Zentrifugieren nochmals gereinigt wer<strong>de</strong>n.<br />
· Die Testdurchführung sollte unverzüglich nach <strong>de</strong>r Probenentnahme durchgeführt wer<strong>de</strong>n.<br />
Bewahren Sie Proben nicht zu lange bei Raumtemperatur auf. Serum und Plasma<br />
können bei 2-8°C bis zu 3 Tagen aufbewahrt wer<strong>de</strong>n. Für eine längere Lagerung sollten<br />
die Proben bei unter -20°C aufbewahrt wer<strong>de</strong>n.<br />
· Bringen Sie die Proben vor Testdurchführung auf Raumtemperatur. Gefrorene Proben<br />
müssen vor Testdurchführung vollständig aufgetaut und gut gemischt wer<strong>de</strong>n. Proben<br />
sollten nicht wie<strong>de</strong>rholt eingefroren und aufgetaut wer<strong>de</strong>n.<br />
QUALITÄTSKONTROLLE DURCH DEN BENUTZER:<br />
Der Test enthält eine interne Kontrolle. Das Erscheinen einer farbigen Ban<strong>de</strong> in <strong>de</strong>r Kontrollregion<br />
(C) bestätigt die korrekte Durchführung <strong>de</strong>s Tests und die Reaktionsfähigkeit <strong>de</strong>r<br />
Reagenzien.<br />
In einem sorgfältig arbeiten<strong>de</strong>n Laborbetrieb sollten jedoch zusätzlich externe Kontrollen<br />
durchgeführt wer<strong>de</strong>n.<br />
TESTDURCHFÜHRUNG:<br />
1. Bitte beachten Sie die Hinweise zur Probenahme. Bringen Sie die Testkassette<br />
(im verschlossenen Beutel!) und die Urin-/Serumprobe bzw. Kontrolle auf Zimmertemperatur<br />
(15 - 30°C).<br />
2. Entnehmen Sie die Kassette und versehen Sie sie mit einer Kennzeichnung<br />
<strong>de</strong>r eingesetzten Probe.<br />
3. Tragen Sie mit <strong>de</strong>r Pipette etwa 0,12 mL Probenflüssigkeit<br />
(markiertes Volumen o<strong>de</strong>r 3 Tropfen) in die run<strong>de</strong><br />
Probenöffnung auf. Halten Sie bitte dabei die Pipette<br />
senkrecht und verwen<strong>de</strong>n Sie für je<strong>de</strong> Probe eine neue<br />
Pipette.<br />
4. Warten Sie auf das Erscheinen von farbigen Ban<strong>de</strong>n.<br />
Ein positives Ergebnis kann bei hoher <strong>hCG</strong>-Konzentration<br />
schon nach 40 Sekun<strong>de</strong>n vorliegen. In <strong>de</strong>r Regel ist die Ablesezeit 3 bis 5 Minuten.<br />
Jedoch sollte für die Bestätigung eines negativen Ergebnisses die volle Reaktionszeit<br />
von 5 Minuten abgewartet wer<strong>de</strong>n. Warten Sie nicht länger als 5 Minuten.<br />
5. Falls das Testergebnis nach 15 Minuten interpretiert wird, liegt die Sensitivität bei 10<br />
mIU <strong>hCG</strong>/mL. Dies erhöht das Risiko von anscheinend falsch-positiven Ergebnissen be-<br />

dingt durch natürliche Aborte, bestimmte Tumorerkrankungen, erhöhte physiologische<br />
<strong>hCG</strong>-Konzentrationen nicht-schwangerer Frauen und <strong>hCG</strong>-haltige Medikamente. In <strong>de</strong>r<br />
Routine raten wir daher nicht zu einer Testinterpretation nach 5 Minuten Reaktionszeit.<br />
AUSWERTUNG<br />
NEGATIV (nicht schwanger):<br />
Im Ergebnisfenster erscheint nur eine rote Ban<strong>de</strong>, die Kontrolllinie (C). In <strong>de</strong>r Testregion (T)<br />
ist keine Ban<strong>de</strong> zu sehen.<br />

POSITIV (schwanger):<br />
Es zeigen sich zwei farbige Ban<strong>de</strong>n. Zusätzlich zu <strong>de</strong>r Ban<strong>de</strong> in <strong>de</strong>r Kontrollregion (C) ist<br />
eine zweite Ban<strong>de</strong> in <strong>de</strong>r Testregion (T) zu sehen. Die <strong>hCG</strong>-Konzentration entspricht <strong>de</strong>r<br />
einer Schwangerschaft.<br />

UNGÜLTIG:<br />
Das Fehlen <strong>de</strong>r roten Ban<strong>de</strong> in <strong>de</strong>r Kontrollregion (C) o<strong>de</strong>r das vollkommene Fehlen von<br />
roten Ban<strong>de</strong>n <strong>de</strong>utet auf einen Fehler in <strong>de</strong>r Durchführung <strong>de</strong>s Tests o<strong>de</strong>r <strong>de</strong>n Verfall <strong>de</strong>r<br />
Reagenzien hin. Erscheint auch nach <strong>de</strong>m wie<strong>de</strong>rholten Auftropfen von Probenflüssigkeit<br />
keine rote Ban<strong>de</strong>, verwerfen Sie bitte diesen Test; er ist ungültig.<br />

Achtung: Die Farbintensität in <strong>de</strong>r Testregion ist proportional zur <strong>hCG</strong>-Konzentration im<br />
Urin/Serum. Dennoch ist <strong>de</strong>r Test nur qualitativ verwendbar.<br />
ANMERKUNGEN ZUR AUSWERTUNG<br />
Bei sehr verdünnten Proben kann es sein, dass die <strong>hCG</strong>-Konzentration nicht hoch genug ist.<br />
Bei einem negativen Testergebnis trotz Verdachtes auf eine Schwangerschaft sollte 48-72<br />
Stun<strong>de</strong>n später noch einmal eine Probe mit Morgenurin getestet wer<strong>de</strong>n.<br />
Bereits kurz nach <strong>de</strong>r Einnistung in <strong>de</strong>r Gebärmutter sind sehr geringe <strong>hCG</strong>-Konzentrationen<br />
(weniger als 50 mIU/mL) im Urin o<strong>de</strong>r Serum vorhan<strong>de</strong>n. Da jedoch eine beträchtliche<br />
Anzahl von Schwangerschaften in <strong>de</strong>n ersten 3 Monaten aus natürlichen Grün<strong>de</strong>n en<strong>de</strong>n,<br />
ist es ratsam, sich ein schwach positives Testergebnis mit Hilfe von an<strong>de</strong>ren klinischen Tests<br />
bestätigen zu lassen.<br />
Eine Reihe von Umstän<strong>de</strong>n (z.B. Tumorerkrankung) können, selbst wenn die Patientin nicht<br />
schwanger ist, eine <strong>hCG</strong>-Konzentration bis über 10 mIU/mL hervorrufen. Deshalb sollte bei<br />
Anwesenheit von <strong>hCG</strong> im Urin o<strong>de</strong>r im Serum, das durch <strong>de</strong>n BioNexia ® <strong>hCG</strong> Combo ermittelt<br />
wur<strong>de</strong>, keine Schwangerschaft diagnostiziert wer<strong>de</strong>n, bis die eben genannten Möglichkeiten<br />
nicht klar ausgeschlossen wur<strong>de</strong>n.<br />
Im Übrigen sollte wie bei allen diagnostischen Tests auch hier die Diagnose nicht auf Grund<br />
<strong>de</strong>s Ergebnisses eines einzelnen Testes gestellt wer<strong>de</strong>n. Die Diagnose sollte durch <strong>de</strong>n Arzt<br />
nach Auswertung aller relevanten Befun<strong>de</strong> aus <strong>de</strong>r klinischen und <strong>de</strong>r Laboruntersuchung<br />
gestellt wer<strong>de</strong>n.<br />
ERWARTETE WERTE<br />
Bei gesun<strong>de</strong>n Männern und gesun<strong>de</strong>n nicht schwangeren Frauen kann mit <strong>de</strong>m BioNexia ®<br />
<strong>hCG</strong> Combo kein <strong>hCG</strong> nachgewiesen wer<strong>de</strong>n.<br />
Die <strong>hCG</strong>-Konzentration im Urin o<strong>de</strong>r Serum von schwangeren Frauen verdoppelt sich nach<br />
<strong>de</strong>r Einnistung <strong>de</strong>r befruchteten Eizelle in <strong>de</strong>r Gebärmutter etwa alle 2,5 Tage, und erreicht,<br />
2-3 Monate nach <strong>de</strong>r letzten Menstruation (etwa 10. SSW), einen Höchststand von bis zu<br />
200000 mIU/mL. Der BioNexia ® <strong>hCG</strong> Combo hat eine Sensitivität von 20 mIU/mL nach 5<br />
Minuten für Urin und Serum und kann schon am ersten Tag nach Ausbleiben <strong>de</strong>r Menstruation<br />
<strong>hCG</strong> ermitteln. Eine Konzentration von 25 mIU/mL o<strong>de</strong>r höher ist 7-10 Tage nach<br />
<strong>de</strong>r Empfängnis vorhan<strong>de</strong>n o<strong>de</strong>r 4-5 Tage vor <strong>de</strong>m Ausbleiben <strong>de</strong>r Menstruation. Eine sehr<br />
schwache Linie in <strong>de</strong>r Testregion sollte nicht gleich als „Schwanger“ diagnostiziert wer<strong>de</strong>n.<br />
Es wird empfohlen nach 48-72 Stun<strong>de</strong>n eine weitere Urin-/Serumprobe zu testen. Auch bei<br />
negativen Testergebnissen, bei <strong>de</strong>nen angenommen wird, dass die Patientin schwanger ist,<br />
sollte nach 48-72 Stun<strong>de</strong>n <strong>de</strong>r Test mit einer neuen Probe wie<strong>de</strong>rholt wer<strong>de</strong>n.
QUALITÄTSEIGENSCHAFTEN DES TESTS<br />
Der BioNexia ® <strong>hCG</strong> Combo kann nach 3 bis 5 Minuten nach Zugabe <strong>de</strong>r Probe ausgewertet<br />
wer<strong>de</strong>n.<br />
SENSITIVITÄT<br />
Die analytische Sensitivität <strong>de</strong>s BioNexia ® <strong>hCG</strong> Combo ist 20 mIU/mL (basierend auf <strong>de</strong>r<br />
4. IRP von HCG). Die Sensitivität wur<strong>de</strong> durch wie<strong>de</strong>rholtes Testen von Proben mit einer<br />
Konzentration von 20 mIU/mL über mehrere Wochen hinweg ermittelt. Der BioNexia ® <strong>hCG</strong><br />
Combo zeigt keinen Prozon-Effekt bis zu einer maximalen physiologischen Konzentration<br />
(600 IU/mL). Das heißt, <strong>de</strong>r Gültigkeitsbereich ist von 20 mIU/mL bis 600 IU/mL.<br />
SPEZIFITÄT<br />
Die Spezifität <strong>de</strong>s BioNexia ® <strong>hCG</strong> Combo wur<strong>de</strong> mit Hilfe von Kreuzreaktivitätsstudien mit<br />
bekannten Mengen von LH (300 mIU/mL), FSH (1000 mIU/mL) und TSH (1 mIU/mL) ermittelt.<br />
Bis zu diesen Konzentrationen zeigte sich keine Kreuzreaktivität.<br />
VERGLEICHSSTUDIE<br />
URIN: In zwei Studien wur<strong>de</strong>n 130 positive und 178 negative Urinproben mit <strong>de</strong>m BioNexia ®<br />
<strong>hCG</strong> Combo und einem an<strong>de</strong>ren kommerziell erhältlichen <strong>hCG</strong> Schnelltest getestet. Bei<strong>de</strong><br />
Studien stimmten 100% überein.<br />

SUMMARY<br />
Human Chorionic Gonadotropin (<strong>hCG</strong>) is a glycoprotein hormone secreted by the <strong>de</strong>veloping<br />
placenta shortly after fertilization. In normal pregnancy, <strong>hCG</strong> can be <strong>de</strong>tected in the<br />
urine or serum of pregnant women as early as seven days after conception (14). The concentration<br />
of <strong>hCG</strong> rises very rapidly, frequently exceeding a value of 100 mIU/mL (2-5) by<br />
the first missed menstrual period, and finally peaking in the 30000-100000 mIU/mL range<br />
about 10-12 weeks into pregnancy. The appearance of <strong>hCG</strong> soon after fertilization and its<br />
subsequent rise in concentration during early gestational growth make it an excellent marker<br />
for the early <strong>de</strong>tection of pregnancy.<br />
BioNexia ® <strong>hCG</strong> Combo has been <strong>de</strong>veloped for the rapid <strong>de</strong>tection of the pregnancy hormone<br />
<strong>hCG</strong> in urine or serum samples with a sensitivity of 20 mIU/mL. The test kit uses monoclonal<br />
antibodies to <strong>de</strong>tect the concentration of <strong>hCG</strong> in urine or serum. BioNexia ® <strong>hCG</strong><br />
Combo shows no cross-reactivity with structurally related hormones (FSH, LH, TSH) in normal<br />
maximum physiological concentrations.<br />
TEST PRINCIPLE<br />
BioNexia ® <strong>hCG</strong> Combo is a chromatographic immunoassay (CIA) for the rapid qualitative<br />
<strong>de</strong>tection of <strong>hCG</strong> in urine and serum samples. The membrane of the test is pre-coated with<br />
anti-alpha <strong>hCG</strong> capture antibody on the test region and goat-anti-rabbit antibody on the<br />
control region.<br />
During the test, the sample reacts initially with colloidal gold particles coated with anti-beta<br />
<strong>hCG</strong> monoclonal antibody. Subsequently, all the particles move along with the liquid over<br />
the membrane by capillary action.<br />
In the case of a positive result, i.e. in the presence of <strong>hCG</strong>, an antibody-hcG-antibody-colloidal<br />
gold particle complex and thus a colored band is formed in the test region. If no colored<br />
band appears in the test region, the test result is negative. In any case, a colored band<br />
should always be visible in the control region. The appearance of the colored band in the<br />
control region is an indication of the proper test performance.<br />
STRUCTURE OF THE TEST CASSETTE<br />
The plastic casing of the test cassette contains the test strip with the membrane and the antibodies.<br />
The round sample window is located at the right end and the rectangular reaction<br />
field window is located at the left end of the test cassette. A test region and a control region<br />
are located in the reaction field of the test strip. The inscription next to the window indicates<br />
the test region (T) and the control region (C).<br />

30°C) in its original packing for the duration of the shelf-life.<br />

SAMPLING AND SAMPLE STORAGE<br />
Urine samples:<br />
· The urine sample should be collected in a clean, dry container. Samples collected at any<br />
time of the day may be used. However, a first morning urine sample is preferred since it<br />
generally contains the highest concentration of <strong>hCG</strong>.<br />
· The sample and the test must be brought to room temperature before performing the<br />
test. Urine samples exhibiting visible precipitates should be centrifuged, filtered or allowed<br />
to settle. The sample must be taken only from the clear upper phase.<br />
· Urine samples may be stored at 2-8°C up to 48 hours before the performance of the test.<br />
For subsequent usages, it is necessary to store the samples at a temperature of at least<br />
-20°C.<br />
Serum samples:<br />
· Collect venous blood aseptically into a clean, container containing heparin or EDTA.<br />
· Separate the serum or plasma from the blood as fast as possible in or<strong>de</strong>r to avoid hemolysis.<br />
Use only clear, non-hemolyzed samples. Samples containing solid particles should be<br />
cleaned once again by centrifuging before testing.<br />
· The test should be performed immediately after the sampling.<br />
· Do not store samples for excessively longer periods of time at room temperature. Serum<br />
and plasma can be stored for up to 3 days at 2-8°C. For a long-term storage, samples<br />
should be stored at -20°C.<br />
· Bring the samples to room temperature before testing. Frozen samples must be thawed<br />
completely and mixed thoroughly before performing the test. Avoid repeated freezing<br />
and thawing of samples.<br />
QUALITY CONTROL BY THE USER:<br />
The test contains an internal control. The appearance of a colored band in the control region<br />
(C) confirms the correct handling of the test and the ability of the reagents to respond.<br />
However, good laboratory practice recommends the usage of additional external quality<br />
controls.<br />
TESTDURCHFÜHRUNG:<br />
1. Please observe the instructions for sampling. Bring the test cassette (in its sealed<br />
pouch) and the urine/serum sample or control to room temperature (15 - 30°C).<br />
2. Remove the cassette and provi<strong>de</strong> it with an i<strong>de</strong>ntification mark of the inserted<br />
sample.<br />
3. Dispense about 0.12 mL of sample liquid (marked volume or 3 drops) into<br />
the round sample window with the pipette. Hold the pipette vertically and<br />
use a new pipette for each sample.<br />
4. Wait for the appearance of colored bands. A positive<br />
result can be present at a high concentration of <strong>hCG</strong><br />
as early as after 40 seconds. Usually, the reading time<br />
is 3 to 5 minutes. However, it is recommen<strong>de</strong>d to wait<br />
for the complete response time of 5 minutes for confirming<br />
a negative result. Do not wait for more than 5<br />
minutes.<br />
5. If the test result is interpreted after 15 minutes, the sensitivity is at 10 mIU <strong>hCG</strong>/mL. This<br />
increases the risk of apparent false-positive results caused by natural abortions, certain<br />
tumor diseases, increased physiological concentrations of <strong>hCG</strong> among non-pregnant women<br />
and medicines containing <strong>hCG</strong>. We therefore usually do not advise a test interpretation<br />
after a response time of 5 minutes.<br />

INTERPRETATION OF THE RESULTS<br />
NEGATIVE (not pregnant):<br />
Only one red band - the control band (C) appears in the result window. No band is visible<br />
in the test region (T).<br />

Two colored bands are visible. A second band can be seen in the test region (T) in addition<br />
to the band in the control region (C). The concentration of <strong>hCG</strong> corresponds to that existing<br />
during a pregnancy.<br />

INVALID:<br />
The absence of the red band in the control region (C) or the complete absence of red bands<br />
points to an error in performing the test or the <strong>de</strong>terioration of reagents. If a red band does<br />
not appear even after repeated dropwise application of sample liquid, please discard this<br />
test kit; it is invalid.<br />

Please note: The color intensity in the test region is proportional to the concentration of <strong>hCG</strong><br />
in urine /serum. Nevertheless, the test can be used only qualitatively.<br />
NOTES ON THE INTERPRETATION OF RESULTS<br />
Very dilute urine samples may not contain a sufficiently high concentration of <strong>hCG</strong>. If the test<br />
result is negative and pregnancy is still suspected, a first morning urine sample should be<br />
collected 48-72 hours later and tested.<br />
Very low levels of <strong>hCG</strong> (less than 50 mIU/mL) are present in urine and serum shortly after<br />
implantation in the uterus. However, because a significant number of pregnancies terminate<br />
in the first three months for natural reasons, a test result that is weakly positive should be<br />
confirmed with the help of other clinical tests.<br />
A number of conditions (e.g. tumor disease) cause a concentration of <strong>hCG</strong> of more than 10<br />
mlU/mL even though the patient is not pregnant. Therefore, unless these conditions have<br />
been ruled out clearly, the presence of <strong>hCG</strong> in urine or serum <strong>de</strong>tected by BioNexia® <strong>hCG</strong><br />
Combo should not be used to diagnose pregnancy.<br />
As with all diagnostic tests, a <strong>de</strong>finitive diagnosis should not be based on the results of a<br />
single test. A confirmed pregnancy diagnosis should only be ma<strong>de</strong> by a physician after all the<br />
relevant clinical and laboratory findings have been evaluated.<br />
EXPECTED VALUES<br />
Healthy men and women do not have <strong>de</strong>tectable levels of <strong>hCG</strong> by BioNexia ® <strong>hCG</strong> Combo.<br />
For pregnant women, the concentration of <strong>hCG</strong> in urine or serum doubles approximately<br />
every 2.5 days after the implantation of the fertilized ovum in the uterus and reaches a<br />
maximum level of up to 200000 mIU/mL 2-3 months after the last menstrual period (around<br />
the 10th week of pregnancy). BioNexia ® <strong>hCG</strong> Combo has a sensitivity of 20 mIU/mL after 5<br />
minutes for urine and serum and can <strong>de</strong>tect the presence of <strong>hCG</strong> as early as on the very first<br />
day since the missed period. A concentration of 25 mIU/mL or higher is present 7-10 days<br />
after conception or 4-5 days before the missed period. A very weak band in the test region<br />
should not be diagnosed immediately as “pregnant“. It is recommen<strong>de</strong>d to test another<br />
urine/serum sample after 48-72 hours. Also in the case of negative test results, in which it<br />
is assumed that the patient is pregnant, it is recommen<strong>de</strong>d to repeat the test with a new<br />
sample after 48-72 hours.
QUALITY PROPERTIES OF THE TEST<br />
BioNexia ® <strong>hCG</strong> Combo can be interpreted 3 to 5 minutes after adding the sample.<br />
SENSITIVITY<br />
The analytical sensitivity of BioNexia ® <strong>hCG</strong> Combo is 20 mIU/mL (based on the 4. IRP of<br />
HCG). The sensitivity was <strong>de</strong>termined by repeated testing of samples having a concentration<br />
of 20 mIU/mL over a period of several weeks. BioNexia ® <strong>hCG</strong> Combo shows no proton<br />
effect up to a maximum physiological concentration (600 IU/mL). This means that the range<br />
of validity is 20 mIU/mL to 600 IU/mL.<br />
SPECIFICITY<br />
The specificity of BioNexia ® <strong>hCG</strong> Combo was <strong>de</strong>termined with the help of cross-reactivity<br />
studies with known quantities of LH (300 mIU/mL), FSH (1000 mIU/mL) and TSH (1 mIU/mL).<br />
No cross-reactivity was observed at these concentrations.<br />
COMPARATIVE STUDY<br />
URINE: 130 positive and 178 negative urine samples were tested in two studies using<br />
BioNexia ® <strong>hCG</strong> Combo and another commercially available <strong>hCG</strong> rapid test. The results obtained<br />
in both the studies indicated 100% agreement.<br />

APPLICATION<br />
Pour la détermination rapi<strong>de</strong> <strong>de</strong> l‘hormone <strong>de</strong> grossesse <strong>hCG</strong> dans l‘urine et dans le sérum.<br />
Ce test donne un résultat qualitatif optique et a été conçu pour l’usage professionnel.<br />
RÉSUMÉ<br />
La Gonadotrophine chorionique (<strong>hCG</strong>) est une hormone glycoprotéinique qui se forme déjà<br />
peu après la fécondation placentaire. Au cours d‘une grossesse normale, on peut détecter<br />
l’<strong>hCG</strong> déjà sept jours après la conception dans l‘urine ou dans le sérum (14). La concentration<br />
en <strong>hCG</strong> augmente rapi<strong>de</strong>ment, en atteignant fréquemment avec la première absence<br />
<strong>de</strong> la saignée menstruelle une valeur <strong>de</strong> plus <strong>de</strong> 100 mIU/mL (2-5) et culmine enfin entre les<br />
semaines <strong>de</strong> grossesse 10 et 12 dans une plage comprise entre 30000 et 100000 mIU/mL.<br />
L‘apparition précoce <strong>de</strong> l’<strong>hCG</strong> après la fécondation et la hausse <strong>de</strong> concentration qui suit<br />
le sta<strong>de</strong> précoce <strong>de</strong> la grossesse en font un indicateur approprié remarquable pour le test<br />
précoce d‘une grossesse.<br />
Le BioNexia ® <strong>hCG</strong> Combo a été développé pour le test rapi<strong>de</strong> <strong>de</strong> l‘hormone <strong>de</strong> grossesse<br />
<strong>hCG</strong> dans <strong>de</strong>s échantillons <strong>de</strong> sérum ou d‘urine avec une sensibilité <strong>de</strong> 20 mIU/mL. Le test<br />
utilise l’anticorps monoclonal pour déterminer la concentration en <strong>hCG</strong> dans l‘urine ou dans<br />
le sérum. Le BioNexia ® <strong>hCG</strong> Combo ne montre pas <strong>de</strong> réactivité croisée aux hormones<br />
utilisées structurellement (FSH, LH, TSH) dans les limites <strong>de</strong>s concentrations physiologiques<br />
maximales prévues.<br />
PRINCIPE DU TEST<br />
Le BioNexia ® <strong>hCG</strong> Combo est un test immunochimique chromatographique (Chromatographic<br />
Immunoassay, CIA) pour la détermination qualitative rapi<strong>de</strong> <strong>de</strong> l’<strong>hCG</strong> dans <strong>de</strong>s<br />
échantillons <strong>de</strong> sérum et d‘urine. Des anticorps anti-chaîne HCG α sont liés au niveau <strong>de</strong> la<br />
membrane dans le domaine <strong>de</strong> test et <strong>de</strong>s anticorps <strong>de</strong> chèvre anti-lapin sont liés dans la<br />
région <strong>de</strong> contrôle.<br />
Pendant le test, l‘échantillon réagit d‘abord avec <strong>de</strong>s anticorps monoclonaux couplés or à la<br />
chaîne <strong>hCG</strong> β. Ensuite, toutes les particules sont transportées par les forces capillaires avec<br />
le liqui<strong>de</strong> vers la membrane.<br />
Avec un résultat positif, c.-à-d. par la présence d’<strong>hCG</strong>, se forme un agrégat d’anticorps<br />
<strong>hCG</strong>-anticorps-or ainsi qu’une ban<strong>de</strong> colorée dans le domaine <strong>de</strong> test. Si aucune ban<strong>de</strong><br />
colorée n’apparaît dans la le domaine <strong>de</strong> test, c’est que le résultat est négatif. Dans tous les<br />
cas, une ban<strong>de</strong> colorée doit être vue dans le domaine <strong>de</strong> contrôle. En effet, elle indique que<br />
l’exécution du test a réussi.<br />
STRUCTURE DE LA CASSETTE DE TEST<br />
Le boîtier en plastique <strong>de</strong> la cassette <strong>de</strong> test contient la ban<strong>de</strong>lette avec la membrane et les<br />
anticorps. A l’extrémité droite <strong>de</strong> la cassette <strong>de</strong> test se trouve, l‘ouverture <strong>de</strong> prélèvement et<br />
à gauche la fenêtre rectangulaire du champ <strong>de</strong> réaction. Un domaine <strong>de</strong> test et un domaine<br />
<strong>de</strong> contrôle se trouvent dans le champ <strong>de</strong> réaction <strong>de</strong> la ban<strong>de</strong>lette <strong>de</strong> test. L‘inscription à<br />

PRELEVEMENT ET CONSERVATION DES ECHANTILLONS<br />
Échantillons urine :<br />
· L‘échantillon d’urine doit être collecté dans un récipient sec propre. Des échantillons<br />
peuvent être collectés à tout moment <strong>de</strong> la journée ; toutefois, l‘urine <strong>de</strong> matin contient<br />
en général la concentration en hormone la plus élevée.<br />
· Avant d’effectuer toute tentative, l‘échantillon et le test doivent être amenés à une température<br />
ambiante<br />
· Les échantillons qui présentent une opacification visible doivent être centrifugés, filtrés<br />
ou mis <strong>de</strong> côté jusqu‘à ce que l‘opacification disparaisse. Seulement la projection claire<br />
doit être utilisée pour le test.<br />
· Des échantillons d’urine peuvent être conservés jusqu’à 48 heures après le test à une<br />
température comprise entre 2-8°C.<br />
· Pour toute utilisation ultérieure, il est nécessaire <strong>de</strong> conserver temporairement les échantillons<br />
à une température d’au moins -20°C.<br />
Échantillons <strong>de</strong> sérum :<br />
· Collecter le sang veineux <strong>de</strong> façon aseptique dans un récipient propre héparinisé ou<br />
contenant <strong>de</strong> l’EDTA.<br />
· Séparer dès que possible le sérum ou le plasma sanguin, <strong>de</strong> l‘hémolyse.<br />
· N‘utiliser que <strong>de</strong>s échantillons clairs et non hémodialysés. Les échantillons qui contiennent<br />
<strong>de</strong>s particules fermes <strong>de</strong>voir être nettoyés <strong>de</strong> nouveau avant le test par centrifuge age.<br />
· L’exécution du test doit se faire immédiatement après l‘échantillonnage.<br />
· Ne pas gar<strong>de</strong>r les échantillons trop longtemps à température ambiante. Le sérum et le<br />
plasma peuvent être conservés à une température comprise entre 2 et 8°C jusqu‘à 3<br />
jours. Pour prolonger la conservation, les échantillons doivent être conservés à une température<br />
<strong>de</strong> -20°C.<br />
· Amener les échantillons à la température ambiante avant l’exécution du test. Les échantillons<br />
congelés doivent être dégelés complètement et bien mélanges avant d’exécuter<br />
le test. Les échantillons doivent être congelés et dégelés <strong>de</strong> nouveau.<br />
CONTRÔLE DE QUALITE DE LA PART DE L’UTILISATEUR :<br />
Le test contient un contrôle interne. L’apparition d‘une ban<strong>de</strong> colorée dans la région <strong>de</strong><br />
contrôle (c) confirme l’exécution correcte du test et la réactivité <strong>de</strong>s réactifs.<br />
Toutefois, <strong>de</strong>s contrôles externes supplémentaires doivent être effectués dans un laboratoire<br />
qui travaille soigneusement.<br />
EXECUTION D’UN TEST :<br />
1. Veuillez considérer les indications ci-après pour l‘échantillonnage. Amener la<br />
cassette <strong>de</strong> test (dans le sachet scelle !) et l‘échantillon d’urine et/ou <strong>de</strong> sérum<br />
ou encore <strong>de</strong> contrôle à température ambiante (15 - 30°C).<br />
2. Extraire la cassette et prévoir l’i<strong>de</strong>ntification d’un échantillon inséré.<br />
3. Verser avec la pipette environ 0,12 mL <strong>de</strong> liqui<strong>de</strong><br />
d‘échantillon (volume marqué ou 3 gouttes) dans<br />
l‘ouverture d‘échantillon ron<strong>de</strong>. Pour ce faire, tenir la<br />
pipette à la verticale en utilisant une nouvelle pipette<br />
vous pour chaque échantillon.<br />
4. Atten<strong>de</strong>z l’apparition <strong>de</strong>s ban<strong>de</strong>s colorées. Un résultat<br />
positif peut déjà être présent en cas <strong>de</strong> concentration<br />
en <strong>hCG</strong> très élevée au bout <strong>de</strong> 40 secon<strong>de</strong>s. Généralement,<br />
le temps <strong>de</strong> lecture varie va <strong>de</strong> 3 à 5 minutes. Toutefois, pour la confirmation d‘un<br />
résultat négatif, le temps <strong>de</strong> réaction total <strong>de</strong> 5 minutes doit être attendu. N‘atten<strong>de</strong>z pas<br />
plus <strong>de</strong> 5 minutes.<br />

5. Si le résultat <strong>de</strong>s tests est interprété après 15 minutes, la sensibilité s‘élève à 10 mIU<br />
<strong>hCG</strong>/mL. Cela augmente le risque d’avoir <strong>de</strong>s résultats apparemment faux-positifs en<br />
raison <strong>de</strong> faux-couches, <strong>de</strong> certaines affections tumorales et <strong>de</strong> concentrations élevées en<br />
<strong>hCG</strong> chez les femmes non enceintes et <strong>de</strong> médicaments contenant l’<strong>hCG</strong>. Dans la routine,<br />
nous déconseillons d’interpréter un test après un temps <strong>de</strong> réaction <strong>de</strong> 5 minutes.<br />
EVALUATION<br />

Dans la fenêtre <strong>de</strong> résultat, seule une ban<strong>de</strong> rouge apparaît : la ligne <strong>de</strong> contrôle (c). Dans<br />
le domaine <strong>de</strong> test (T), aucune ban<strong>de</strong> n‘est visible.<br />

Deux ban<strong>de</strong>s colorées apparaissent. En plus du volume dans le domaine <strong>de</strong> contrôle (c) une<br />

<strong>hCG</strong> correspond aux valeurs indiquant une grossesse.<br />

INVALIDE :<br />
L‘absence <strong>de</strong> la ban<strong>de</strong> rouge dans la région <strong>de</strong> contrôle (c) ou l‘absence totale <strong>de</strong> ban<strong>de</strong><br />
rouge indique qu’une erreur <strong>de</strong> test est survenue ou que les réactifs sont défaillants. Si<br />
aucune ban<strong>de</strong> rouge n’apparaît après un goutte à goutte répétée du liqui<strong>de</strong> d‘échantillon,<br />
le test doit être refusé car il n’est pas valable.<br />

Attention : L‘intensité <strong>de</strong> couleurs dans le domaine <strong>de</strong> test est proportionnelle à la concentration<br />
en <strong>hCG</strong> dans l‘urine/sérum. Cependant, le test ne peut être utilisé que qualitativement.<br />
REMARQUES A PROPOS DE L‘ÉVALUATION<br />
Avec <strong>de</strong>s échantillons très dilués, il se peut que la concentration en <strong>hCG</strong> ne soit pas suffisamment<br />
élevée. Avec un résultat <strong>de</strong>s tests négatif malgré un soupçon <strong>de</strong> grossesse, un autre<br />
échantillon doit être testé avec l’urine du matin après 48- 72 heures.<br />
Déjà peu après l‘implantation dans l‘utérus, une concentration en <strong>hCG</strong> très faible (moins <strong>de</strong><br />
50 mIU/mL) est présente dans l‘urine ou dans le sérum. Compte tenu du nombre considérable<br />
<strong>de</strong> grossesses qui prennent fin au cours <strong>de</strong>s 3 premiers mois pour <strong>de</strong>s raisons naturelles,<br />
il est toutefois conseillé <strong>de</strong> faire confirmer un résultat <strong>de</strong>s tests faiblement positif à l‘ai<strong>de</strong><br />
d‘autres tests cliniques.<br />
Une série <strong>de</strong> circonstances (par exemple, une affection tumorale) peut provoquer, même si<br />
la patiente n‘est pas enceinte, une concentration en <strong>hCG</strong> <strong>de</strong> plus <strong>de</strong> 10 mIU/mL. Par conséquent,<br />
avec la présence d’<strong>hCG</strong> dans l‘urine ou dans le sérum, déterminée par BioNexia ®<br />
<strong>hCG</strong> Combo, aucune grossesse ne peut être diagnostiquée avec précision, tant que les<br />
possibilités citées n‘ont pas été exclues clairement.<br />
En outre, comme avec tous les tests diagnostics, il est déconseillé d‘établir un diagnostic sur<br />
la base du résultat d‘un essai particulier. Le diagnostic <strong>de</strong>vrait être établi par le mé<strong>de</strong>cin suite<br />
à l’évaluation <strong>de</strong> tous les résultats cliniques et <strong>de</strong>s tests <strong>de</strong> laboratoire pertinents.<br />
VALEURS ATTENDUES<br />
Chez les hommes sains et les femmes non enceintes et saines, aucune <strong>hCG</strong> ne peut être<br />
déterminée avec le BioNexia ® <strong>hCG</strong> Combo.<br />
La concentration en <strong>hCG</strong> dans l‘urine ou le sérum <strong>de</strong>s femmes enceintes double après<br />
l‘implantation <strong>de</strong> l’ovule fécondé dans l‘utérus par exemple tous les 2,5 jours, et atteint, 2-3<br />
mois après les <strong>de</strong>rnières règles (environ 10. SSW), un niveau maximum 200v000 mIU/mL.<br />
Le BioNexia ® <strong>hCG</strong> Combo a une sensibilité <strong>de</strong> 20 mIU/mL après 5 minutes pour urine et<br />
le sérum et peut déterminer la présence <strong>de</strong> l‘<strong>hCG</strong> dès le premier jour après <strong>de</strong> retard <strong>de</strong>s<br />
règles. Une concentration <strong>de</strong> 25 mIU/mL ou plus élevée peut être déterminée 7-10 jours<br />
après la conception ou 4-5 jours avant l‘absence <strong>de</strong> règles. Une ligne très faible dans la le<br />
domaine <strong>de</strong> test ne doit pas être diagnostiquée immédiatement comme „enceinte“. Il est<br />
recommandé <strong>de</strong> tester un autre échantillon d’urine ou <strong>de</strong> sérum après 48-72 heures. Même<br />
en cas <strong>de</strong> résultats négatifs, avec lesquels l’on détermine que la patiente est enceinte, le test<br />
doit être répété sur un nouvel échantillon après 48-72 heures.
CARACTERISTIQUES QUALITATIVES DU TEST<br />
Le BioNexia ® <strong>hCG</strong> Combo peut être évalué 3 à 5 minutes après l‘ajout <strong>de</strong> l‘échantillon.<br />
SENSIBILITE<br />
La sensitivité analytique du BioNexia ® <strong>hCG</strong> Combo est <strong>de</strong> 20 mIU/mL (sur la base <strong>de</strong>s 4. IRP<br />
<strong>de</strong> l’HCG). La sensitivité a été déterminée par <strong>de</strong>s tests répétés sur <strong>de</strong>s échantillons avec<br />
une concentration <strong>de</strong> 20 mIU/mL étalées sur plusieurs semaines. Le BioNexia ® <strong>hCG</strong> Combo<br />
ne présente aucun effet prozone jusqu‘à une concentration physiologique maximale (600<br />
IU/mL). Autrement dit, la plage <strong>de</strong> validité va <strong>de</strong> 20 mIU/mL à 600 IU/mL.<br />
SPECIFICITE<br />
La spécificité du BioNexia ® <strong>hCG</strong> Combo a été déterminée par <strong>de</strong>s étu<strong>de</strong>s <strong>de</strong> réactivité croisée<br />
avec <strong>de</strong>s quantités connues <strong>de</strong> LH (300 mIU/mL), FSH (1000 mIU/mL) et TSH (1 mIU/mL).<br />
Aucune réactivité <strong>de</strong> croix n‘est apparue jusqu’à ces concentrations.<br />

URINE : 130 échantillons positifs et 178 échantillons négatifs d’urine ont été testés avec le<br />

UTILIZACIÓN<br />
Para la <strong>de</strong>tección rápida <strong>de</strong> la hormona <strong>de</strong>l embarazo <strong>hCG</strong> en la orina y el suero. Este test<br />
brinda un resultado óptico y cualitativo y ha sido <strong>de</strong>sarrollado para uso profesional.<br />
ZUSAMMENFASSUNG<br />
La hormona gonadotropina coriónica humana (<strong>hCG</strong>) es una hormona glucoproteica formada<br />
por la placenta, incluso poco <strong>de</strong>spués <strong>de</strong> la fecundación. Durante el transcurso <strong>de</strong> un<br />
embarazo normal, la hormona <strong>hCG</strong> ya pue<strong>de</strong> <strong>de</strong>tectarse siete días <strong>de</strong>spués <strong>de</strong> ocurrida la<br />
concepción, sea en orina o en suero (14). La concentración <strong>de</strong> <strong>hCG</strong> aumenta con rapi<strong>de</strong>z,<br />
alcanzando frecuentemente un valor mayor a 100 mIU/mL (2-5) con la primera falta <strong>de</strong> menstruación,<br />
hasta culminar finalmente en un área <strong>de</strong> 30000 - 100000 mIU/mL, entre la 10° y la<br />
12° semana <strong>de</strong> embarazo. La prematura aparición <strong>de</strong> la <strong>hCG</strong> luego <strong>de</strong> la fecundación, con el<br />
consecuente aumento <strong>de</strong> concentración en el estadio temprano <strong>de</strong>l embarazo, lo convierten<br />
en un excelente e idóneo indicador para la <strong>de</strong>tección precoz <strong>de</strong> un embarazo.<br />
BioNexia ® <strong>hCG</strong> Combo está <strong>de</strong>sarrollado para la <strong>de</strong>tección rápida <strong>de</strong> la hormona <strong>de</strong>l embarazo<br />
<strong>hCG</strong> en muestras <strong>de</strong> orina o suero, con una sensibilidad <strong>de</strong> 20 mIU/mL. El test se<br />
sirve <strong>de</strong> anticuerpos monoclonales, a fin <strong>de</strong> <strong>de</strong>terminar la concentración <strong>de</strong> <strong>hCG</strong> en la orina<br />
o el suero. BioNexia ® <strong>hCG</strong> Combo no presenta reactividad cruzada alguna con hormonas<br />
estructuralmente relacionadas (FSH, LH, TSH), hasta mucho más allá <strong>de</strong> las máximas concentraciones<br />
fisiológicas esperadas.<br />
PRINCIPIO DEL TEST<br />
BioNexia® <strong>hCG</strong> Combo es un inmunoensayo cromatográfico (Chromatographic Immunoassay,<br />
CIA) para <strong>de</strong>tectar rápida y cualitativamente la <strong>hCG</strong> en muestras <strong>de</strong> orina y suero. En la<br />
región <strong>de</strong> test, se hallan ligados a la membrana anticuerpos contra la ca<strong>de</strong>na α (alfa) <strong>de</strong> <strong>hCG</strong><br />
y en la región <strong>de</strong> control, anticuerpos <strong>de</strong> cabra anti-conejo.<br />
Durante el procedimiento <strong>de</strong> test, la muestra reacciona primero con los anticuerpos monoclonales<br />
conjugados con oro, contra la ca<strong>de</strong>na β (beta) <strong>de</strong> <strong>hCG</strong>. Seguidamente, se transportan<br />
todas las partículas mediante la fuerza capilar y con el líquido, a lo largo <strong>de</strong> la membrana.<br />
En caso <strong>de</strong> resultado positivo, vale <strong>de</strong>cir <strong>de</strong> presencia <strong>de</strong> <strong>hCG</strong>, se formará un agregado<br />
<strong>de</strong> oro-anticuerpo-<strong>hCG</strong>-anticuerpo y con ello, una banda <strong>de</strong> color en la región <strong>de</strong> test. Si<br />
no aparece ninguna banda <strong>de</strong> color en la región <strong>de</strong> test, entonces el resultado <strong>de</strong>l test es<br />
negativo. En todo caso, en la región <strong>de</strong> control <strong>de</strong>bería hacerse visible una banda <strong>de</strong> color,<br />
que indica la efectiva realización <strong>de</strong>l test.<br />
ESTRUCTURA DEL CASETE DE TEST<br />
La cajita plástica <strong>de</strong>l casete <strong>de</strong> test contiene la tira reactiva con la membrana y los anticuerpos.<br />
En el extremo <strong>de</strong>recho <strong>de</strong>l casete <strong>de</strong> test, se encuentra la ranura circular para muestra y<br />
en la parte izquierda, la ventana rectangular <strong>de</strong>l campo <strong>de</strong> reacción. En el campo <strong>de</strong> reacción<br />
<strong>de</strong> la tira reactiva, se hallan una región <strong>de</strong> test y la región <strong>de</strong> control. La inscripción situada al<br />
lado <strong>de</strong> la ventana señala las correspondientes regiones <strong>de</strong> test (T) y <strong>de</strong> control (C).<br />

CONSERVACIÓN Y DURABILIDAD<br />

OBSERVACIONES<br />

· No utilizar el test, cuando el envoltorio plástico esté dañado.<br />
· Respete la cantidad <strong>de</strong> muestra necesaria.<br />
· Lleve las muestras <strong>de</strong> orina o suero a temperatura ambiente (15-30°C).<br />
· No introducir muestras en el campo <strong>de</strong> reacción (campo <strong>de</strong> resultados).<br />
· No tocar el campo <strong>de</strong> reacción, a fin <strong>de</strong> evitar contaminaciones.<br />
· Se aconseja utilizar un recipiente colector individual para cada muestra, a fin <strong>de</strong> evitar<br />
contaminaciones cruzadas.<br />
· Por ello, <strong>de</strong>ben tomarse las precauciones necesarias para su manipulación y eliminación.<br />
· Utilizar el test inmediatamente <strong>de</strong>spués <strong>de</strong> abierto el envoltorio plástico.<br />
· Respete el tiempo <strong>de</strong> reacción indicado (3 a 5 minutos).<br />
· Conservar y transportar el test sólo bajo las condiciones <strong>de</strong> temperatura indicadas.<br />
TOMA Y CONSERVACIÓN DE LAS MUESTRAS<br />
Muestras <strong>de</strong> orina:<br />
· La orina <strong>de</strong>be recolectarse en un recipiente limpio y seco. Se pue<strong>de</strong>n recoger muestras a<br />
cualquier hora <strong>de</strong>l día, pero por lo general, la orina matutina contiene la mayor concentración<br />
hormonal.<br />
· Antes <strong>de</strong> realizar la prueba, la muestra y el test <strong>de</strong>ben llevarse a temperatura ambiente.<br />
Si las muestras fueran visiblemente turbias, se recomienda centrifugarlas, filtrarlas o bien<br />
<strong>de</strong>jarlas en reposo, hasta que se precipite la turbi<strong>de</strong>z. Para el test, sólo <strong>de</strong>be utilizarse el<br />
supernatante transparente.<br />
· Las muestras <strong>de</strong> orina pue<strong>de</strong>n conservarse a 2-8°C y durante 48 horas como máximo,<br />
antes <strong>de</strong> la realización <strong>de</strong> la prueba. En caso <strong>de</strong> una utilización posterior <strong>de</strong> las muestras,<br />
es necesario conservarlas a una temperatura <strong>de</strong> por los menos -20°C.<br />
Muestras <strong>de</strong> suero:<br />
· Recoja asépticamente sangre venosa en un recipiente limpio y heparinizado o con EDTA.<br />
· Separe el suero <strong>de</strong>l plasma <strong>de</strong> la sangre, tan rápido como sea posible, a fin <strong>de</strong> evitar la<br />
hemólisis. Utilice sólo muestras claras y sin hemolizar. Si las muestras contienen partículas<br />
sólidas, es necesario centrifugarlas para limpiarlas nuevamente, antes <strong>de</strong> efectuar la prueba.<br />
· El test <strong>de</strong>be realizarse inmediatamente <strong>de</strong>spués <strong>de</strong> la toma <strong>de</strong> la muestra.<br />
· No conserve las muestras <strong>de</strong>masiado tiempo a temperatura ambiente. El suero y el plasma<br />
pue<strong>de</strong>n conservarse hasta 3 días a 2-8°C. En caso <strong>de</strong> un almacenamiento más prolongado,<br />
<strong>de</strong>ben conservarse a una temperatura inferior a -20°C.<br />
· Lleve las muestras a temperatura ambiente, antes <strong>de</strong> realizar el test. Las muestras congeladas<br />
<strong>de</strong>ben <strong>de</strong>scongelarse completamente y mezclarse bien, antes <strong>de</strong> la realización <strong>de</strong>l<br />
test. No <strong>de</strong>ben congelarse y <strong>de</strong>scongelarse las muestras repetidas veces.<br />
CONTROL DE CALIDAD A TRAVÉS DEL USUARIO:<br />
El test contiene un control interno. La aparición <strong>de</strong> una banda <strong>de</strong> color en la región <strong>de</strong> control<br />
(C) confirma la realización correcta <strong>de</strong>l test y la capacidad <strong>de</strong> reacción <strong>de</strong> los reactivos.<br />
Sin embargo, para efectuar un procedimiento <strong>de</strong> laboratorio cuidadoso y esmerado, <strong>de</strong>berían<br />
realizarse controles externos adicionales.<br />
REALIZACIÓN DEL TEST:<br />
1. Respete las indicaciones para la toma <strong>de</strong> muestras. Lleve el casete (¡con el envoltorio<br />
cerrado!) y la muestra <strong>de</strong> orina o suero o el control, a temperatura ambiente (15<br />
- 30°C).<br />
2. Retire el casete y rotúlelo con una <strong>de</strong>scripción <strong>de</strong> la muestra utilizada.<br />
3. Con una pipeta, introduzca aprox. 0,12 mL <strong>de</strong>l líquido <strong>de</strong> muestra (volumen<br />
marcado ó 3 gotas) en la ranura circular para muestra. Sostenga para ello<br />
la pipeta verticalmente y para cada muestra, utilice una<br />
nueva pipeta.<br />
4. Espere hasta que aparezcan las bandas <strong>de</strong> color. En<br />
caso <strong>de</strong> una elevada concentración <strong>de</strong> <strong>hCG</strong>, el resultado<br />
positivo ya será indicado luego <strong>de</strong> 40 segundos. Por<br />
lo general, el tiempo <strong>de</strong> lectura es <strong>de</strong> 3 a 5 minutos. Sin<br />
embargo, para confirmar un resultado positivo, <strong>de</strong>be<br />
aguardarse el tiempo total <strong>de</strong> reacción <strong>de</strong> 5 minutos.<br />
No espere más <strong>de</strong> 5 minutos.<br />
5. En caso <strong>de</strong> que el resultado <strong>de</strong>l test se interprete luego <strong>de</strong> 15 minutos, la sensibilidad<br />
se hallará en los 10 mIU <strong>hCG</strong>/mL. Esto aumenta el riesgo <strong>de</strong> probables resultados falsos<br />
negativos, causados por abortos naturales, <strong>de</strong>terminadas enfermeda<strong>de</strong>s tumorales, elevadas<br />
concentraciones fisiológicas <strong>de</strong> <strong>hCG</strong> en mujeres no embarazadas y medicamentos<br />

que contienen <strong>hCG</strong>. Por ello, en la rutina diaria no aconsejamos hacer una interpretación<br />
<strong>de</strong>l test, una vez transcurridos los 5 minutos <strong>de</strong> tiempo <strong>de</strong> reacción.<br />
INTERPRETACIÓN<br />
NEGATIVO (no embarazada):<br />
En la ventana <strong>de</strong> resultados aparece sólo una banda roja, la línea <strong>de</strong> control (C). En la región<br />
<strong>de</strong> test (T) no se hace visible ninguna banda.<br />

POSITIV (embarazada):<br />
Se presentan dos bandas <strong>de</strong> color. Adicionalmente a la banda en la región <strong>de</strong> control (C),<br />
aparece una segunda banda en la región <strong>de</strong> test (T). La concentración <strong>de</strong> <strong>hCG</strong> correspon<strong>de</strong><br />
a la <strong>de</strong> un embarazo.<br />

NO VÁLIDO:<br />
La ausencia <strong>de</strong> la banda roja en la región <strong>de</strong> control (C) o la ausencia total <strong>de</strong> bandas rojas<br />
indica un error en la realización <strong>de</strong>l test o la caducidad <strong>de</strong> los reactivos. Si aún luego <strong>de</strong><br />
introducir repetidamente líquido <strong>de</strong> muestra, no apareciera ninguna banda roja, <strong>de</strong>seche<br />
este test, ya que no es válido.<br />

Atención: La intensidad <strong>de</strong>l color en la región <strong>de</strong> test es proporcional a la concentración <strong>de</strong><br />
<strong>hCG</strong> en la orina o el suero. No obstante, el test es sólo cualitativamente utilizable.<br />
ADVERTENCIAS SOBRE LA INTERPRETACIÓN<br />
Si las muestras están muy diluidas, pue<strong>de</strong> ocurrir que la concentración <strong>de</strong> <strong>hCG</strong> no sea suficientemente<br />
elevada. En caso <strong>de</strong> resultado negativo, a pesar <strong>de</strong> existir una presunción <strong>de</strong><br />
embarazo, se aconseja volver a examinar una muestra con orina matutina y luego <strong>de</strong> 48-72<br />
horas.<br />
Ya poco <strong>de</strong>spués <strong>de</strong> la anidación uterina, las concentraciones <strong>de</strong> <strong>hCG</strong> presentes en la orina o<br />
el suero son muy bajas (menos <strong>de</strong> 50 mIU/mL). Sin embargo, <strong>de</strong>bido a que un consi<strong>de</strong>rable<br />
número <strong>de</strong> embarazos se interrumpen en los primeros 3 meses por causas naturales, es recomendable<br />
confirmar un resultado positivo débil por medio <strong>de</strong> otros test clínicos.<br />
Una serie <strong>de</strong> circunstancias (p. ej. una enfermedad tumoral) pue<strong>de</strong>n generar una concentración<br />
<strong>de</strong> <strong>hCG</strong> <strong>de</strong> más <strong>de</strong> 10 mIU/mL, aun cuando la paciente no está embarazada. Por lo<br />
tanto, en caso <strong>de</strong> presencia <strong>de</strong> <strong>hCG</strong> en la orina o el suero <strong>de</strong>terminada con BioNexia® <strong>hCG</strong><br />
Combo, no <strong>de</strong>bería diagnosticarse un embarazo, hasta tanto se excluyan claramente las<br />
posibilida<strong>de</strong>s arriba mencionadas.<br />
Asimismo y como en todos los test diagnósticos, no es conveniente efectuar un diagnóstico<br />
basándose en el resultado <strong>de</strong> un único test. El diagnóstico <strong>de</strong>bería ser efectuado por<br />
un médico, <strong>de</strong> acuerdo a la evaluación <strong>de</strong> los diferentes estudios clínicos y exámenes <strong>de</strong><br />
laboratorio.<br />
VALORES ESPERADOS<br />
Con BioNexia® <strong>hCG</strong> Combo, no se pue<strong>de</strong> <strong>de</strong>tectar la presencia <strong>de</strong> <strong>hCG</strong> en hombres sanos<br />
y mujeres no embarazadas.<br />
La concentración <strong>de</strong> <strong>hCG</strong> en la orina o el suero <strong>de</strong> mujeres embarazadas se duplica luego<br />
<strong>de</strong> la anidación <strong>de</strong>l óvulo fecundado en el útero aprox. cada 2,5 días, alcanzando un nivel<br />
máximo <strong>de</strong> hasta 200000 mIU/mL 2-3 meses <strong>de</strong>spués <strong>de</strong> la última menstruación (aprox. 10.<br />
SSW). BioNexia® <strong>hCG</strong> Combo tiene una sensibilidad <strong>de</strong> 20 mIU/mL luego <strong>de</strong> 5 minutos,<br />
para orina y suero, y pue<strong>de</strong> <strong>de</strong>terminar la <strong>hCG</strong> ya al día siguiente <strong>de</strong>l primer día <strong>de</strong> falta <strong>de</strong><br />
menstruación. De 7 a 10 días <strong>de</strong>spués <strong>de</strong> la concepción o <strong>de</strong> 4 a 5 días antes <strong>de</strong> la falta <strong>de</strong><br />
menstruación, la concentración presente es <strong>de</strong> 25 mIU/mL o mayor. Una línea muy débil en<br />
la región <strong>de</strong> test no <strong>de</strong>be diagnosticarse inmediatamente como “embarazada”. Se recomienda<br />
volver a examinar una muestra <strong>de</strong> orina o suero luego <strong>de</strong> 48-72 horas. También en caso<br />
<strong>de</strong> resultados <strong>de</strong> test negativos y en casos en que se supone que la paciente está embarazada,<br />
es recomendable repetir el test luego <strong>de</strong> 48-72 horas con una nueva muestra.
CARACTERÍSTICAS DE CALIDAD DEL TEST<br />
BioNexia® <strong>hCG</strong> Combo pue<strong>de</strong> interpretarse luego <strong>de</strong> transcurridos 3 a 5 minutos <strong>de</strong> introducida<br />
la muestra.<br />
SENSIBILIDAD<br />
La sensibilidad analítica <strong>de</strong> BioNexia® <strong>hCG</strong> Combo es <strong>de</strong> 20 mIU/mL (basada en 4. <strong>de</strong> IRP<br />
<strong>de</strong> HCG). La sensibilidad fue <strong>de</strong>terminada mediante reiterados test, <strong>de</strong> muestras con una<br />
concentración <strong>de</strong> 20 mIU/mL y a lo largo <strong>de</strong> varias semanas. BioNexia® <strong>hCG</strong> Combo no presenta<br />
efecto Prozono, hasta una concentración fisiológica máxima (600 IU/mL). Esto significa<br />
que el campo <strong>de</strong> vali<strong>de</strong>z es <strong>de</strong> entre 20 mIU/mL a 600 IU/mL.<br />
ESPECIFICIDAD<br />
La especificidad <strong>de</strong> BioNexia® <strong>hCG</strong> Combo fue <strong>de</strong>terminada mediante estudios <strong>de</strong> reactividad<br />
cruzada, con cantida<strong>de</strong>s conocidas <strong>de</strong> LH (300 mIU/mL), FSH (1000 mIU/mL) y TSH (1<br />
mIU/mL). Hasta dichos niveles <strong>de</strong> concentración, no se presentó reactividad cruzada alguna.<br />
ESTUDIO COMPARATIVO<br />
ORINA: en dos estudios, se examinaron 130 muestras <strong>de</strong> orina positivas y 178 negativas, con<br />
BioNexia® <strong>hCG</strong> Combo y con otro test rápido <strong>de</strong> <strong>hCG</strong> <strong>de</strong> venta comercial. Los dos estudios<br />
coincidieron en un 100%.<br />

USO<br />
Per la <strong>de</strong>terminazione rapida <strong>de</strong>ll’ormone <strong>de</strong>lla gravidanza <strong>hCG</strong> nell’urina e nel siero. Questo<br />
test fornisce un risultato qualitativo a lettura visiva ed è stato concepito per l‘uso professionale.<br />
RIASSUNTO<br />
La gonadotropina corionica umana (<strong>hCG</strong>) è un ormone glicoproteico che viene secreto dalla<br />
placenta già poco dopo la fecondazione. Nel <strong>de</strong>corso di una normale gravidanza, l’<strong>hCG</strong><br />
può essere rilevata nell’urina o nel siero già sette giorni dopo il concepimento (14). La concentrazione<br />
di <strong>hCG</strong> aumenta molto rapidamente, spesso raggiunge un valore di oltre 100<br />
mIU/mL (2-5) durante la prima interruzione <strong>de</strong>l ciclo mestruale e infine tocca un range di<br />
30000 - 100000 mIU/mL dalla 10° alla 12° settimana di gravidanza. La comparsa <strong>de</strong>ll’<strong>hCG</strong><br />
poco dopo il concepimento e il conseguente aumento <strong>de</strong>lla sua concentrazione nei primi<br />
mesi di gestazione ne fanno un indicatore straordinariamente adatto per la <strong>de</strong>terminazione<br />
precoce di una gravidanza.<br />
BioNexia ® <strong>hCG</strong> Combo è stato concepito per la <strong>de</strong>terminazione rapida <strong>de</strong>ll’ormone <strong>de</strong>lla<br />
gravidanza <strong>hCG</strong> nei campioni di urina o di siero con una sensibilità di 20 mIU/mL. Il test<br />
utilizza anticorpi monoclonali per rilevare la concentrazione <strong>de</strong>ll’<strong>hCG</strong> nell’urina o nel siero.<br />
BioNexia ® <strong>hCG</strong> Combo non presenta alcuna reattività crociata con ormoni di struttura analoga<br />
(FSH, LH, TSH) fino a ben oltre le concentrazioni fisiologiche massime attese.<br />
PRINCIPIO DEL TEST<br />
BioNexia ® <strong>hCG</strong> Combo è un immunodosaggio cromatografico (Chromatographic Immunoassay,<br />
CIA) per la <strong>de</strong>terminazione qualitativa rapida <strong>de</strong>ll’<strong>hCG</strong> nei campioni di urina e di siero.<br />
Sulla membrana sono legati anticorpi anti alfa-<strong>hCG</strong> nella zona <strong>de</strong>l test e anticorpi caprini<br />
anti-coniglio nella zona di controllo.<br />
Durante il test, il campione reagisce inizialmente con gli anticorpi monoclonali anti beta-<strong>hCG</strong><br />
coniugati con oro colloidale. Successivamente, tutte le particelle vengono trasportate con il<br />
liquido per azione capillare sulla membrana.<br />
In caso di risultato positivo, vale a dire in presenza di <strong>hCG</strong>, si forma un complesso anticorpo<strong>hCG</strong>-anticorpo<br />
coniugato con oro colloidale e quindi una banda colorata nella zona <strong>de</strong>l test.<br />
Se non compare alcuna banda colorata nella zona <strong>de</strong>l test, il risultato <strong>de</strong>l test è negativo.<br />
Nella zona di controllo <strong>de</strong>ve poter essere in ogni caso visibile una banda colorata. Questa<br />
indica l’esecuzione corretta <strong>de</strong>l test.<br />
STRUTTURA DEL DISPOSITIVO PER IL TEST<br />
L’alloggiamento di plastica <strong>de</strong>l dispositivo per il test contiene strisce reattive con la membrana<br />
e gli anticorpi. Sull’estremità <strong>de</strong>stra <strong>de</strong>l dispositivo si trova un pozzetto circolare per<br />
i campioni, mentre sulla parte sinistra è presente la finestra rettangolare <strong>de</strong>l campo di reazione.<br />
Nel campo di reazione <strong>de</strong>lla striscia reattiva si osservano una zona <strong>de</strong>l test e una<br />
zona di controllo. Le lettere a fianco <strong>de</strong>lla finestra indicano la rispettiva zona <strong>de</strong>l test (T) e<br />

· Osservare i tempi di interpretazione <strong>de</strong>i risultati indicati (da 3 a 5 minuti).<br />

RACCOLTA E CONSERVAZIONE DEI CAMPIONI<br />
Campioni di urina:<br />
· Il campione di urina <strong>de</strong>ve essere raccolto in un recipiente pulito e asciutto. I campioni<br />
possono essere raccolti in qualsiasi momento <strong>de</strong>lla giornata; tuttavia, la prima urina <strong>de</strong>l<br />
mattino contiene in genere la maggiore concentrazione di ormone.<br />
· Prima <strong>de</strong>ll’esecuzione <strong>de</strong>l test, campione e dispositivo per il test <strong>de</strong>vono essere portati<br />
a temperatura ambiente. I campioni che presentano precipitazioni visibili <strong>de</strong>vono essere<br />
centrifugati, filtrati o lasciati riposare fino al <strong>de</strong>posito <strong>de</strong>lle precipitazioni. Per il test utilizzare<br />
solo il surnatante limpido.<br />
· I campioni di urina possono essere conservati a 2-8°C per un massimo di 48 ore fino<br />
all’esecuzione <strong>de</strong>l test. Per utilizzi successivi, nel frattempo i campioni <strong>de</strong>vono essere<br />
conservati ad almeno -20°C.<br />
Campioni di siero:<br />
· Raccogliere il sangue venoso in modo asettico in un recipiente pulito eparinizzato o contenente<br />
EDTA.<br />
· Separare il siero o il plasma dal sangue il più rapidamente possibile per evitare l’emolisi.<br />
Utilizzare esclusivamente campioni limpidi e non emolizzati. I campioni che contengono<br />
particelle soli<strong>de</strong> <strong>de</strong>vono essere di nuovo centrifugati prima <strong>de</strong>ll’esecuzione <strong>de</strong>l test.<br />
· Il test <strong>de</strong>ve essere eseguito immediatamente dopo il prelievo <strong>de</strong>l campione. Non conservare<br />
i campioni a temperatura ambiente troppo a lungo. Il siero e il plasma possono<br />
essere conservati a 2-8°C per fino a 3 giorni. I campioni <strong>de</strong>vono essere congelati ad una<br />
temperatura inferiore a -20°C per una conservazione prolungata.<br />
· Prima <strong>de</strong>ll’esecuzione <strong>de</strong>l test, portare i campioni a temperatura ambiente. I campioni<br />
congelati <strong>de</strong>vono essere completamente scongelati e ben miscelati prima <strong>de</strong>ll’esecuzione<br />
<strong>de</strong>l test. I campioni non <strong>de</strong>vono essere ricongelati e scongelati.<br />
CONTROLLO DI QUALITÀ DA PARTE DELL’UTENTE:<br />
Il test contiene un controllo interno. La comparsa di una banda colorata nella zona di controllo<br />
(C) conferma l’esecuzione corretta <strong>de</strong>l test e la capacità di reazione <strong>de</strong>i reagenti.<br />
In un buon laboratorio dovrebbero tuttavia essere eseguiti anche controlli esterni supplementari.<br />
ESECUZIONE DEL TEST:<br />
1. Osservare le indicazioni per la raccolta <strong>de</strong>i campioni. Portare il dispositivo per il<br />
test (nell’involucro chiuso!) e il campione di urina/siero e/o il controllo a temperatura<br />
ambiente (15 – 30°C).<br />
2. Estrarre il dispositivo per il test e applicarvi un’etichetta di i<strong>de</strong>ntificazione<br />
<strong>de</strong>l campione utilizzato.<br />
3. Con una pipetta applicare circa 0,12 mL <strong>de</strong>l campione<br />
liquido (volume contrassegnato o 3 gocce) nel pozzetto<br />
circolare. A questo proposito, tenere la pipetta in verticale<br />
e utilizzare una pipetta nuova per ogni campione.<br />
4. Atten<strong>de</strong>re che compaiano ban<strong>de</strong> colorate. Già dopo<br />
40 secondi può essere presente un risultato positivo in<br />
caso di elevata concentrazione di <strong>hCG</strong>. Generalmente il<br />
tempo di lettura ammonta a 3-5 minuti. Tuttavia, per la conferma di un risultato negativo<br />
occorre atten<strong>de</strong>re il tempo di reazione completo di 5 minuti. Non atten<strong>de</strong>re oltre 5 minuti.<br />
5. Se il risultato <strong>de</strong>l test viene interpretato dopo 15 minuti, la sensibilità è pari a 10 mIU<br />
<strong>hCG</strong>/mL. Questo aumenta il rischio di risultati palesemente falsi positivi dovuti ad aborti<br />
naturali, <strong>de</strong>terminate patologie tumorali, concentrazioni di <strong>hCG</strong> fisiologiche aumentate di<br />
donne non in stato di gravidanza e farmaci contenenti <strong>hCG</strong>. Normalmente sconsigliamo<br />
pertanto di interpretare il test dopo 5 minuti di tempo di reazione.<br />

INTERPRETAZIONE DEI RISULTATI<br />
NEGATIVO (non in gravidanza):<br />
Nella finestra <strong>de</strong>l risultato compare solo una banda rossa, la linea di controllo (C). Nella zona<br />
<strong>de</strong>l test (T) non è visibile alcuna banda.<br />

POSITIVO (in gravidanza):<br />
Compaiono due ban<strong>de</strong> colorate. Oltre alla banda nella zona di controllo (C) è visibile una<br />
seconda banda nella zona <strong>de</strong>l test (T). La concentrazione di <strong>hCG</strong> corrispon<strong>de</strong> a quello di<br />
uno stato di gravidanza.<br />

NON VALIDO:<br />
L’assenza <strong>de</strong>lla banda rossa nella zona di controllo (C) o l’assenza totale di ban<strong>de</strong> rosse<br />
indica un errore nell’esecuzione <strong>de</strong>l test o che i reagenti sono scaduti. Se anche dopo la<br />
ripetuta applicazione <strong>de</strong>l campione non compare alcuna banda rossa, gettare il dispositivo<br />
in quanto non valido.<br />

Attenzione: L’intensità <strong>de</strong>l colore nella zona <strong>de</strong>l test è proporzionale alla concentrazione di<br />
<strong>hCG</strong> nell’urina/nel siero. Tuttavia, il test è utilizzabile solo per una <strong>de</strong>terminazione qualitativa.<br />
NOTE SULL’INTERPRETAZIONE DEI RISULTATI<br />
In caso di campioni molto diluiti può acca<strong>de</strong>re che la concentrazione di <strong>hCG</strong> non sia sufficientemente<br />
alta. Un risultato negativo nonostante una sospetta gravidanza <strong>de</strong>ve essere<br />
confermato con un nuovo test di un campione di urine <strong>de</strong>l mattino 48-72 ore dopo.<br />
Già poco dopo l’annidamento nell’utero sono presenti concentrazioni di <strong>hCG</strong> molto ridotte<br />
(meno di 50 mIU/mL) nell’urina o nel siero. Tuttavia, poiché un numero consi<strong>de</strong>revole di<br />
gravidanze termina nei primi 3 mesi per ragioni naturali, è consigliabile farsi confermare un<br />
<strong>de</strong>bole risultato positivo mediante altri test clinici.<br />
Una serie di condizioni (ad es. una patologia tumorale) possono causare una concentrazione<br />
elevata di <strong>hCG</strong> fino a oltre 10 mIU/mL, anche se la paziente non è in gravidanza. Pertanto,<br />
in presenza di <strong>hCG</strong> nell’urina o nel siero rilevata mediante il test BioNexia ® <strong>hCG</strong> Combo<br />
non dovrà essere diagnosticata alcuna gravidanza fino all’esclusione <strong>de</strong>lle possibilità summenzionate.<br />
Inoltre, come per tutti i test diagnostici, anche in questo caso la diagnosi non <strong>de</strong>ve essere<br />
formulata sulla base <strong>de</strong>l risultato di un singolo test. La diagnosi <strong>de</strong>ve essere formulata dal<br />
medico dopo la valutazione di tutti i risultati rilevanti <strong>de</strong>lla visita medica e <strong>de</strong>lle analisi di<br />
laboratorio.<br />
VALORI ATTESI<br />
Negli uomini sani e nelle donne sane non in gravidanza non è possibile rilevare alcuna <strong>hCG</strong><br />
con BioNexia ® <strong>hCG</strong> Combo.<br />
La concentrazione di <strong>hCG</strong> nell’urina o nel siero <strong>de</strong>lle donne in gravidanza raddoppia circa<br />
ogni 2,5 giorni dopo l’annidamento <strong>de</strong>lla cellula uovo fecondata nell’utero e raggiunge un<br />

(ca. nella 10° settimana di gravidanza). BioNexia ® <strong>hCG</strong> Combo ha una sensibilità di 20 mIU/<br />
mL dopo 5 minuti per urina e siero ed è in grado di rilevare l’<strong>hCG</strong> già il primo giorno dopo<br />
<strong>de</strong>ll’interruzione <strong>de</strong>l ciclo mestruale. Una concentrazione pari o superiore a 25 mIU/mL è<br />
presente 7-10 giorni dopo il concepimento o 4-5 giorni prima <strong>de</strong>ll’interruzione <strong>de</strong>l ciclo mestruale.<br />
Una linea molto <strong>de</strong>bole nella zona <strong>de</strong>l test non <strong>de</strong>ve essere diagnosticata come uno<br />
“stato di gravidanza“. Si consiglia di ripetere il test su un altro campione di urina/siero dopo<br />
48-72 ore. Anche in caso di risultati negativi per pazienti di cui si suppone una gravidanza, è<br />
necessario ripetere il test con un nuovo campione dopo 48-72 ore.
CARATTERISTICHE QUALITATIVE DEL TEST<br />
BioNexia ® <strong>hCG</strong> Combo può essere valutato dopo 3-5 minuti dopo l’aggiunta <strong>de</strong>l campione.<br />
SENSIBILITÀ<br />
La sensibilità analitica di BioNexia ® <strong>hCG</strong> Combo è 20 mIU/mL (calibrata secondo la 4° IRP di<br />
HCG). La sensibilità è stata rilevata mediante ripetuti test su campioni con una concentrazione<br />
di 20 mIU/mL per diverse settimane. BioNexia ® <strong>hCG</strong> Combo non presenta alcun fenomeno<br />
di zona fino ad una concentrazione massima fisiologica (600 IU/mL). Questo significa<br />
che il range di validità va da 20 mIU/mL a 600 IU/mL.<br />
SPECIFICITÀ<br />
La specificità di BioNexia ® <strong>hCG</strong> Combo è stata rilevata mediante studi di reattività crociata<br />
con quantità note di LH (300 mIU/mL), FSH (1000 mIU/mL) e TSH (1 mIU/mL). Fino a queste<br />
concentrazioni non è stata riscontrata alcuna reattività crociata.<br />
STUDIO DI CORRELAZIONE<br />
URINA: in due studi sono stati testati 130 campioni di urina positivi e 178 negativi con<br />
BioNexia ® <strong>hCG</strong> Combo e un altro test rapido per l’<strong>hCG</strong> reperibile in commercio. Entrambi<br />

ZASTOSOWANIE<br />
Do szybkiego wykrywania hormonu ciąży <strong>hCG</strong> w moczu i w surowicy. Niniejszy test daje optyczny<br />
wynik jakościowy i został stworzony do zastosowań profesjonalnych.<br />
SKŁAD<br />
Ludzka gonadotropina kosmówkowa (<strong>hCG</strong>) jest hormonem glikoproteinowym, który wytwarzany<br />
jest przez łożysko już krótko po zapłodnieniu. W przebiegu normalnej ciąży <strong>hCG</strong> można wykryć<br />
w moczu lub surowicy już po siedmiu dniach od poczęcia (14). Koncentracja <strong>hCG</strong> bardzo szybko<br />
rośnie, przy pierwszym braku miesiączkowania często osiąga wartość ponad 100 mIU/mL (2-5) i<br />
wreszcie osiąga swój szczyt pomiędzy 10. a 12. tygodniem ciąży w zakresie 30000 - 100000 mIU/mL.<br />
Wczesne pojawienie się <strong>hCG</strong> po zapłodnieniu i przypadający po nim wzrost koncentracji we wczesnym<br />
stadium ciąży czynią go niezwykle skutecznym wskaźnikiem wczesnego wykrywania ciąży.<br />
BioNexia ® <strong>hCG</strong> Combo stworzony jest do szybkiego wykrywania hormonu ciąży <strong>hCG</strong> w próbkach<br />
moczu i surowicy z czułością 20 mIU/mL. Test wykorzystuje przeciwciała monoklonalne aby ustalić<br />
koncentrację <strong>hCG</strong> w moczu lub surowicy. BioNexia ® <strong>hCG</strong> Combo wykazuje brak reaktywności<br />
skrzyżowanej do strulturalnie spokrewnionych hormonów (FSH, LH, TSH) dalece ponad oczekiwaną<br />
maksymalną koncentrację fizjologiczną.<br />
ZASADA TESTU<br />
BioNexia ® <strong>hCG</strong> Combo jest chromatograficznym testem immunochemicznym (Chromatographic<br />
Immunoassay, CIA) do szybkiego jakościowego wykrywania <strong>hCG</strong> w próbkach moczu i surowicy. Na<br />
membranie w obszarze testowym powiązane są przeciwciała na łańcuchy α <strong>hCG</strong>, a w obszarze kontrolnym<br />
kozie przeciwciała przeciwkrólicze.<br />
Podczas testu próbka reaguje wpierw ze sprzężonymi ze złotem monoklonalnymi przeciwciałami<br />
na łańcuchy β <strong>hCG</strong>. Po tym wszystkie cząsteczki transportowane są dzięki siłom kapilarnym wraz z<br />
płynem poprzez membranę.<br />
Przy wyniku pozytywnym, tzn. przy obecności <strong>hCG</strong>, tworzy się zespół przciwciałoHCG-przeciwciałozłoto<br />
i tym samym kolorowy pasek w obszarze testowym. Jeśli w obszarze testowym nie pojawi się<br />
kolorowy pasek, wynik testu jest negatywny. W obszarze kontrolnym w każdym przypadku powinien<br />
być widoczny kolorowy pasek. Wskazuje on na pomyślne przeprowadzenie testu.<br />
KONSTRUKCJA KASETY TESTOWEJ<br />
Plastikowa obudowa kasety testowej zawiera pasek testu wraz z membraną i przeciwciałami. Na<br />
prawym końcu kasety testowej znajduje się okrągły otwór do pobierania próbek a w lewej części<br />
czworokątne okienko pola reakcji. W polu reakcji paska testowego znajduje się obszar testowy i<br />
obszar kontrolny. Opis obok okienka oznacza odpowiednio obszar testowy (T) i kontrolny (C).<br />

POBIERANIE I PRZECHOWYWANIE PRÓBEK<br />
Próbki moczu:<br />
· próbkę moczu należy pobrać do czystego suchego pojemnika. Próbki mogą być pobierane o<br />
każ<strong>de</strong>j porze dnia; jednakże rano mocz zasadniczo zawiera największą koncentrację hormonów.<br />
· przed dokonaniem kontroli próbka i test muszą być doprowadzone do temperatury pokojowej.<br />
Próbki z widocznym zmętnieniem należy odwirować, przefiltrować lub pozostawić do ustania się<br />
zmętnienia. Do testu stosować tylko klarowną zawartość.<br />
· próbki moczu do czasu przeprowadzenia testu można przechowywać w temperaturze 2-8°C<br />
maksymalnie 48 Stun<strong>de</strong>n. Do późniejszego użycia konieczne jest przechowywanie próbek w<br />
tym czasie w temperaturze przynajmniej -20°C.<br />
Próbki surowicy:<br />
· proszę aseptycznie pobrać krew żylną do pojemnika czystego heparynizowanego lub<br />
zawierającego EDTA.<br />
· oddzielić surowicę lub osoczę jak najszybciej od krwi, aby uniknąć hemolizy. Stosować tylko<br />
próbki czyste i nie hemolizowane. Próbki, które zawierają trwałe cząstki należy przed testem ponownie<br />
oczyścić przez odwirowanie.<br />
· przeprowadzenie testu powinno nastąpić niezwłocznie po pobraniu próbki.<br />
Przechowywać próbki nie za długo w temperaturze pomieszczenia, Surowica i osocze mogą być<br />
przechowywane w temperaturze 2-8°C do 3 dni. Dłużej przechowywane próbki powinny być<br />
magazynowane poniżej -20°C.<br />
· przed przeprowadzeniem testu doprowadzić próbki do temperatury otoczenia. Próbki<br />
zamarznięte muszą być przed testem całkowicie rozmrożone i dobrze zmieszane. Próbek nie powinno<br />
się ponownie zamrażać i odmrażać.<br />
KONTROLA JAKOŚCI PRZEZ UŻYTKOWNIKA:<br />
Test zawiera kontrolę wewnętrzną. Ukazanie się kolorowego paska w obszarze kontrolnym (C) potwierdza<br />
prawidłowe wykonanie testu i zdolność reakcyjną odczynników.<br />
W sarannie pracującym zakładzie laboratoryjnym należy jednakże przeprowadzić dodatkową<br />
zewnętrzną kontrolę.<br />
PRZEPROWADZENIE TESTU:<br />
1. Proszę przestrzegać instrukcji odnośnie pobierania próbek. Proszę doprowadzić kasetę<br />
testową (w zamkniętym woreczku!) i próbkę moczu/surowicy wzgl. kontrolę do temperatury<br />
pokojowej (15 - 30°C).<br />
2. Wyjąć kasetę i opatrzyć ją oznaczeniem zastosowanej próbki.<br />
3. Nanieść pipetą około 0,12 mL płynu próbki (ilość zaznaczona lub 3 krople) na<br />
okrągły otwór do próbki. Trzymać przy tym pipetę pionowo i<br />
stosować dla każ<strong>de</strong>j próbki nową pipetę.<br />
4. Odczekać do ukazania się kolorowych pasków. Pozytywny<br />
wynik przy dużej koncentracji <strong>hCG</strong> może pojawić się już<br />
po 40 sekundach. Z reguły czas odczytu wynosi od 3 do 5<br />
minut. Jednakże dla potwierdzenia wyniku negatywnego<br />
należy odczekać do pełnego czasu reakcji 5 minut. Nie<br />
czekać dłużej niż 5 minut.<br />
5. W przypadku gdy wynik testu interpretowany jest po 15<br />
minutach, czułość oscyluje na 10 mIU <strong>hCG</strong>/mL. Zwiększa to ryzyko ewentualnego błędu pozytywnego<br />
wyniku w uwarunkowaniu od naturalnych poronień, określonych guzów, zwiększonej<br />
fizjologicznej koncentracji <strong>hCG</strong> kobiet nieciężarnych i lekarstw zawierających <strong>hCG</strong>. Rutynowo z<br />
tego względu nie radzimy interpretować testu po 5 minutach czasu reakcji.<br />

Ukazują się dwa kolorowe paski. Dodatkowo do paska w obszarze kontrolnym (C) widoczny jest<br />
drugi pasek w obszarze testowym (T). Koncentracja <strong>hCG</strong> odpowiada tej w ciąży.<br />

TEST NIEWAŻNY:<br />
Brak czerwonego paska w obszarze kontrolnym (C) oraz całkowity brak czerwonych pasków<br />
wskazują na błąd w przeprowadzeniu testu lub przeterminowanie odczynników. Jeśli po ponownym<br />
wkropleniu płynu z próbki nie pojawi się ża<strong>de</strong>n czerwony pasek, proszę odrzucić ten test; jest<br />
on nieważny.<br />

UWAGA: Intensywność koloruw obszarze testowym jest proporcjonalna do koncentracji <strong>hCG</strong> w moczu/surowicy.<br />
Jednakże test stosuje się tylko jakościowo.<br />
UWAGI ODNOŚNIE OCENIANIA<br />
Przy bardzo rozrzedzonych próbkach może mieć miejsce fakt, że koncentracja <strong>hCG</strong> nie jest dostatecznie<br />
wysoka. Przy negatywnym wyniku testu pomimo po<strong>de</strong>jrzenia ciąży należy ponownie<br />
przeprowadzić test 48-72 godziny później na próbce porannego moczu.<br />
Już krótko po zagnieżdżeniu się w macicy istnieją niewielkie koncentracje <strong>hCG</strong> (mniej niż 50 mIU/<br />
mL) w moczu lub surowicy. Ponieważ jednak znaczna ilość ciąż kończy się z przyczyn naturalnych w<br />
pierwszych 3 miesiącach, zaleca się, słabo pozytywny wynik potwierdzać za pomocą inych testów<br />
klinicznych.<br />
Spora ilość okoliczności (np. guzy), nawet gdy pacjentki nie są w ciąży, może wywoływać<br />
koncenstrację <strong>hCG</strong> do wartości 10 mIU/mL. Z tego względu nie należy diagnozować ciąży przy<br />
obecności <strong>hCG</strong> w surowicy lub w moczu, która ustalona została za pomocą BioNexia ® <strong>hCG</strong> Combo,<br />
aż do czasu, gdy w oczywisty sposób nie wykluczy się podanych wyżej możliwości.<br />
Poza tym, tak jak i przy innych testach diagnostycznych, nie należy także i tutaj stawiać diagnozy na<br />
podstawie wyniku pojedynczego testu. Diagnoza winna zostać postawiona przez lekarza po ocenie<br />
wszystkich istotnych oznak z badania klinicznego i laboratoryjnego.<br />
WARTOŚCI OCZEKIWANE<br />
U zdrowych mężczyzn i u zdrowych nie ciężarnych kobiet za pomocą BioNexia ® <strong>hCG</strong> Combo można<br />
stwierdzić brak <strong>hCG</strong>.<br />
Koncentracja <strong>hCG</strong> w moczu lub surowicy kobiet ciężarnych po zagnieżdżeniu zapłodnionego jajeczka<br />
w macicy podwaja się co 2,5 dnia i 2-3 miesiące po ostatniej menstruacji (mniej więcej 10. tydzień<br />
ciąży) osiąga najwyższy poziom do 200000 mIU/mL. BioNexia ® <strong>hCG</strong> Combo ma czułość 20 mIU/<br />
mL po 5 minutach dla moczu i surowicy i można nim badać <strong>hCG</strong> już w pierwszym dniu brakującej<br />
menstruacji. Koncentracja 25 mIU/mL lub wyższa istnieje już 7-10 dni po poczęciu lub 4-5 dni przed<br />
wypadającą menstruacją. Bardzo słaby pasek w obszarze testowym nie powinien być od razu diagnozowany<br />
jako „ciężarna“. Zaleca się przetestowanie następnej próbki moczu/surowicy po 48-72<br />
godzinach. Nawet przy negatywnych wynikach testu, przy których po<strong>de</strong>jrzewa się, że pacjentka jest<br />
w ciąży, należy po 48-72 godzinach ponowić test z nową próbką.
WŁAŚCIWOŚCI JAKOŚCIOWE TESTU<br />
BioNexia ® <strong>hCG</strong> Combo można oceniać po 3 do 5 minutach po podaniu próbki.<br />
CZUŁOŚĆ<br />
Analityczna czułość BioNexia ® <strong>hCG</strong> Combo to 20 mIU/mL (bazująć na 4. IRP HCG). Czułość ustalano<br />
za pomocą powtarzanych testów próbek o koncentracji 20 mIU/mL w ciągu wielu tygodni. BioNexia<br />
® <strong>hCG</strong> Combo nie wykazuje żadnego efektu procentowego do maksymalnej wartości fizjologicznej<br />
koncentracji (600 IU/mL). To znaczy, obszar obowiązywania to od 20 mIU/mL do 600 IU/mL.<br />

Specyficzność BioNexia ® <strong>hCG</strong> Combo ustalono za pomocą studiów reaktywności skrzyżowanej ze<br />
