Summary<br />
Designed toprovide timely information on the patient,<br />
whether in hospital, at home or in the consulting room, the<br />
shared medical record holds apermanent store of medical<br />
and administrative information relevant to careofthe patient.<br />
It aims, firstly, toimprove the quality of care byfacilitating<br />
coordination and information exchanges among<br />
health professionals, patients’ relatives and the patient<br />
himself. Another very important element is monitoring of<br />
care practices or CPW (clinical pathways). Medlink is the<br />
first platform with which each patient’s CPW can be followed<br />
individually in real time. The multidisciplinary coordinator<br />
thus has apowerful monitoring and workflow<br />
tool in its hands.<br />
Implemented for several years in projects funded by public<br />
or private structures, one of the foundersof<strong>Medical</strong> Link<br />
Services, whose mission is to providesolutions for the collection<br />
and medico-social sharing of information in atown<br />
setting, isinaposition tofurnish testimony on the conditions<br />
for success, the constraints, and the results achieved<br />
by its customers. The model, developed over five years, is<br />
ablend of advanced technology, health professionals’ experience<br />
and modern organisational practices. It has<br />
proved its worth in the management of various health networks<br />
within public health structures (diabetology, obesity,<br />
palliative care, etc.) but also for the coordination of<br />
home care settings (hospital at home, home management<br />
of the elderly) or monitoring of publichealth projects in developing<br />
countries.<br />
E-Toile is amajor project of the Canton of Geneva and,<br />
through the FSASD and Sitex projects, <strong>Medical</strong> Link<br />
Services forms one of its main contributors for electronic<br />
medical files in atown setting.<br />
Objectifs<br />
La genèse du projet est de mettre en œuvre une plate-forme<br />
universelle de collecte et de partage d’information pourles<br />
activités médico-sociales en ville. Partant du constat que<br />
les acteurs de la médecine de ville se référaient tous àdes<br />
logiques différentes de collecte et de partage de l’information,<br />
et quel’essentiel de cette collecte se faisait sur papier,<br />
le projet avait dès le départ l’optique de fournir une solution<br />
permettant de réunir tous les acteurs autourd’un sys-<br />
tème unique,capable de s’adapter àlalogiquedechacun,<br />
mais en fournissant àtout le monde un modèle organisationnel<br />
structurant. Les acteurs incluant aussi le patient<br />
lui-même et ses proches.<br />
Méthode<br />
–Respecter la logique «métier» dechacun.<br />
–S’appuyer sur les technologies existantes, opérables facilement<br />
par des non-spécialistes et àuncoût adapté au<br />
besoin.<br />
–Sortir delalogique «software» qui impose son format,<br />
en cherchant àsatisfaire partiellement le plus grand<br />
nombre d’utilisateurs.<br />
–Fournir une solution que les utilisateurs puissent faire<br />
évoluer eux-mêmes, simplement en adaptant le modèle<br />
selon leurs besoins, sans avoir besoin dedévelopper.<br />
–Bâtir le modèle àpartirdecas concrets, permettant d’intégrer<br />
rapidement toutes les contraintes opérationnelles<br />
de «vrais» clients.<br />
–Intégrer une logique deprocessus, de workflow, bien<br />
connue dansl’industrie,qui estentrain de se mettre en<br />
place dans lemonde de la santé.<br />
–Faireparticiper le patient ou/et ses proches, chaque fois<br />
que possible.<br />
–Considérer la problématiquedesécurité et de confidentialité<br />
des données comme l’une des contraintes majeures<br />
du système.<br />
–Réussir l’intégration avec les systèmes d’informations<br />
existants.<br />
–Prendre en compte le modèle organisationnel du client,<br />
et l’intégrer dansMedlink, grâce àlagestion des rôles et<br />
des matrices de prise en charge.<br />
–Une plate-forme souple qui s’adapte àl’évolution des<br />
pratiques médicales sans l’intervention de techniciens<br />
de l’informatique.<br />
