Trattamento sistematico delle lesioni
eccesso di stimolazione miocardica, poliuria da
aldosterone escape . Soprattutto nei casi diffi cili, il
rialzo della pressione arteriosa sistemica può produrre
effetti opposti agli attesi (aumento della ICP)
che è possibile provare a contrastare abbassando la
PaCO 2 o somministrando indometacina [115] . In
assenza di risposte favorevoli è necessario rinunciare
all’aumento della MAP.
g. Indometacina. In casi selezionati con onde plateau
(tipo A) non suscettibili di altre terapia e casi con
anormale capacità di autoregolazione può essere
usata l’indometacina [116] in infusione e in bolo,
monitorando la SjO 2 (per intercettare un possibile
eccesso di vasocostrizione acuta), il bilancio idrico
e la funzionalità renale.

Il monitoraggio ICP va interrotto quando i clinici abbiano
raggiunto la sensazione che la patologia abbia iniziato una
fase di reversione e il paziente abbia sopportato bene una
fase di de-escalation terapeutica. Anche la durata delle perturbazioni
della ICP viene valutata quale indice della compliance
endocranica. Nella fase di “ weaning ” dalla terapia anti-HICP
va abbandonata progressivamente la deliquorazione, recuperata
la normocapnia, sospesa l’infusione continua di sedativi
e analgesici, recuperata la normonatriemia, sospesa l’infusione
di catecolamine, controllata la febbre. La rimozione
del catetere va preceduta da una TC e si deve associare alla
sospensione della somministrazione di eparina.
