Transition Coordinator works in partnership with parents and other key professionals to support secondary school pupils (age 14-16 years) who have additional
support for learning needs/disabilities to plan for their transition as they move into adulthood. Also provision of a post-school service until the young person reaches
the age of 24 years. This involves supportingyoung people with their future life choices which may inlude accessing further education, training, employment, adult day
servics. Also providing an advocacy role to ensure that the young person’s voice is heard and that they are fully involved in the decision making process.

Advocacy in Action is an independent advocacy organisation for people with a learning disability living in Falkirk and Clackmannanshire. Advocacy is helping
someone to speak up for themselves, supporting them to express their views and to stand up for their rights. We have a separate young people’s advocacy project in
Clackmannanshire, funded until 2013 by The Big Lottery. The project enables us to work with people who are learning disabled aged between 14 and 25 and who
