Transition Coordinator works in partnership with parents and other key professionals to support secondary school pupils (age 14-16 years) who have additional<br />
support for learning needs/disabilities to plan for their transition as they move into adulthood. Also provision of a post-school service until the young person reaches<br />
the age of 24 years. This involves supportingyoung people with their future life choices which may inlude accessing further education, training, employment, adult day<br />
servics. Also providing an advocacy role to ensure that the young person’s voice is heard and that they are fully involved in the decision making process.<br />

Advocacy in Action is an independent advocacy organisation for people with a learning disability living in Falkirk and Clackmannanshire. Advocacy is helping<br />
someone to speak up for themselves, supporting them to express their views and to stand up for their rights. We have a separate young people’s advocacy project in<br />
Clackmannanshire, funded until 2013 by The Big Lottery. The project enables us to work with people who are learning disabled aged between 14 and 25 and who<br />
