Transition Coordinator works in partnership with parents and other key professionals to support secondary school pupils (age 14-16 years) who have additional support<br />

for learning needs/disabilities to plan for their transition as they move into adulthood. Also provision of a post-school service until the young person reaches the age of<br />

24 years. This involves supportingyoung people with their future life choices which may inlude accessing further education, training, employment, adult day servics.<br />

Also providing an advocacy role to ensure that the young person’s voice is heard and that they are fully involved in the decision making process.<br />
