The Product and Ingredients Form consists of a full description of the food including the<br />

recipe or <strong>for</strong>mulation used. This should include the meat and any edible casings and all added<br />

ingredients such as water, spices, restricted ingredients, etc. The <strong>for</strong>mulation should indicate the<br />

amount or percentage of each ingredient in the <strong>for</strong>mulation.<br />

This <strong>for</strong>m is only needed if there is more than one ingredient.<br />
