Office Visits <strong>and</strong> Counseling<br />

A physician or a physician’s designee may be<br />

reimbursed for counseling (including counseling<br />

a recipient for available courses of treatment)<br />

using E&M office visit procedure codes<br />

99201-99215, even if counseling was the only<br />

service provided during the visit. Counseling<br />

may include the discussion of treatment options<br />

that are not covered by <strong>Wisconsin</strong> Medicaid<br />

(e.g., experimental services). Counseling<br />

procedure codes 99401-99404 are nonreimbursable<br />

as physician services.<br />

Physician Counseling Visits Under<br />

s. 253.10, Wis. Stats.<br />

Refer to the <strong>Surgery</strong> Services chapter of this<br />

section for more information about physician<br />

counseling visits related to abortions.<br />

Preventive <strong>Medicine</strong><br />

Services<br />

Preventive medicine services are those office<br />

visits that relate to preventive medicine E&M<br />

of infants, children, adolescents, <strong>and</strong> adults.<br />

Preventive medicine services include the<br />

following:<br />

• Counseling.<br />

• Anticipatory guidance.<br />

• Risk factor reduction interventions.<br />

Annual Physicals<br />

<strong>Wisconsin</strong> Medicaid reimburses a maximum of<br />

one comprehensive, routine physical<br />

examination per calendar year per recipient.<br />

Recipients may use this examination to fulfill<br />

employment, school entrance, or sports<br />

participation requirements.<br />

Note: <strong>Wisconsin</strong> Medicaid considers<br />

preventive medicine visits for recipients<br />

under age 21 as HealthCheck visits.<br />

HealthCheck Screenings<br />

HealthCheck is <strong>Wisconsin</strong> Medicaid’s federally<br />

m<strong>and</strong>ated program known nationally as Early<br />

<strong>and</strong> Periodic Screening, Diagnosis, <strong>and</strong><br />

Treatment (EPSDT). HealthCheck services<br />

consist of a comprehensive health screening of<br />

Medicaid recipients under 21 years of age that<br />

includes all the following:<br />

• A comprehensive health <strong>and</strong><br />

developmental history (including<br />

anticipatory guidance).<br />

• A comprehensive unclothed physical<br />

exam.<br />

• An age-appropriate vision screen.<br />

• An age-appropriate hearing screen.<br />

• An oral assessment plus referral to a<br />

dentist beginning at age 3.<br />

• Appropriate immunizations.<br />

• Appropriate laboratory tests.<br />

Preventive medicine procedure codes 99381-<br />

99385 or 99391-99395 should only be used by<br />

providers when submitting claims for<br />

comprehensive HealthCheck screens. Other<br />

preventive visits should be billed using the<br />

appropriate office visit code. Providers should<br />

also indicate modifier “UA” with the<br />

appropriate procedure code if a comprehensive<br />

screen results in a referral for further<br />

evaluation <strong>and</strong> treatment. If a comprehensive<br />

HealthCheck screen does not result in a<br />

referral for further evaluation or treatment,<br />

providers should only indicate the appropriate<br />

procedure code, not the modifier.<br />

Interperiodic Visits<br />

<strong>Wisconsin</strong> Medicaid covers medically<br />

necessary interperiodic screening exams to<br />

follow up on detected problems or conditions.<br />

Examples of interperiodic screenings include:<br />

• Immunizations.<br />

• Retesting for an elevated blood lead level.<br />

• Retesting for a low hematocrit.<br />

Providers should submit claims for interperiodic<br />

visits using the appropriate office visit<br />

procedure code (99201-99205, 99211-99215)<br />
