Drugs<br />

Procedure codes for Medicaid-covered<br />

provider-administered drugs are listed in the<br />

physician services maximum allowable fee<br />

schedules. Providers should use the<br />

appropriate fee schedule in conjunction with<br />

the most recent HCPCS coding book for<br />

descriptions.<br />

Diagnosis Restrictions<br />

Diagnosis restrictions that apply to NDCs also<br />

apply to corresponding HCPCS codes when<br />

billed as provider-administered drugs.<br />

<strong>Wisconsin</strong> Medicaid requires a valid <strong>and</strong><br />

acceptable International Classification of<br />

Diseases, Ninth Revision, Clinical<br />

Modification (ICD-9-CM) diagnosis code on<br />

claims for selected provider-administered<br />

drugs. Diagnosis code restrictions are based on<br />

Food <strong>and</strong> Drug Administration-approved<br />

indications <strong>and</strong> compendium st<strong>and</strong>ards.<br />

Refer to Appendix 32 of this section for a list<br />

of drugs that are diagnosis restricted <strong>and</strong> the<br />

respective HCPCS codes, allowable diagnosis<br />

codes, <strong>and</strong> disease descriptions. This list may<br />

be updated periodically. Refer to the Physician<br />

page of the Medicaid Web site for an updated<br />

list.<br />

If the recipient’s diagnosis is not one of the<br />

allowable diagnoses for the code, providers are<br />

required to obtain PA.<br />

Prior Authorization Requirements<br />

Physician services providers are required to<br />

obtain PA for provider-administered drugs in<br />

Appendix 15 of this section <strong>and</strong> for provideradministered<br />

drugs that are not provided with<br />

one of the allowable diagnosis codes listed in<br />

Appendix 32. Providers are required to use the<br />

Prior Authorization/“J” Code Attachment<br />

(PA/JCA), HCF 11034, along with the Prior<br />

Authorization Request Form (PA/RF), HCF<br />

11018, to request PA.<br />

Providers are required to include with the PA<br />

request peer-reviewed medical literature from<br />

scientific medical or pharmaceutical<br />

publications in which original manuscripts are<br />

rejected or published only after having been<br />

reviewed by unbiased independent experts.<br />

Only the diagnosis codes listed in Appendix 32<br />

of this section are reimbursable without PA.<br />

Unclassified Drugs<br />

Providers should not submit claims with<br />

HCPCS procedure code J3490 when there is<br />

another procedure code that better describes<br />

the drug. Claims with J3490 will be denied if<br />

there is a more specific code that may be<br />

used.<br />

Procedure code J3490 requires PA only when<br />

the drug may also be used as a fertility drug.<br />

To be reimbursed for an unclassified drug that<br />

does not require PA or a HCPCS code that<br />

does not have a maximum allowable fee listed<br />

in the fee schedule for physicians, providers<br />

are required to submit a paper CMS 1500<br />

claim form <strong>and</strong> attach the following<br />

information:<br />

• Name of drug.<br />

• National Drug Code.<br />

• Dosage.<br />

• Quantity (e.g., vials, milliliters, milligrams).<br />

Reimbursement<br />

Reimbursement for provider-administered<br />

drugs includes reimbursement for the<br />

administration of the drug. Therefore, providers<br />

should not submit claims for an administration<br />

code (i.e., CPT procedure codes 90782-90788<br />

or HCPCS codes G0351-G0358) concurrently<br />

with a drug code or concurrently with any<br />
