<strong>Medicine</strong> Services<br />

In cases where more than two fluoride<br />

treatments per year are medically necessary,<br />

providers are required to retain supporting<br />

clinical documentation in the recipient’s file<br />

indicating the need for additional treatments.<br />

Ancillary staff (e.g., physician assistants, nurse<br />

practitioners) are required to follow billing<br />

procedures as listed in the Reimbursement<br />

chapter of this section.<br />

<strong>Wisconsin</strong> Medicaid will separately reimburse<br />

providers for the appropriate level office visit<br />

or preventive visit at which the fluoride<br />

application was performed.<br />

Training Materials<br />

Training materials describing how providers<br />

may perform lift-the-lip oral screenings, apply<br />

fluoride varnish to a small child’s teeth, <strong>and</strong><br />

provide basic oral health guidance to parents is<br />

available on the Medicaid Web site.<br />

Hospital Admissions<br />

An external review organization (ERO), under<br />

contract with the Department of Health <strong>and</strong><br />

Family Services (DHFS), reviews the medical<br />

necessity of certain inpatient hospital<br />

admissions.<br />

Complete medical record documentation is<br />

essential for the ERO at the time that the ERO<br />

requests the recipient’s medical record from<br />

the hospital. Physicians must be certain that<br />

the recipient’s record adequately documents<br />

the recipient’s condition <strong>and</strong> continued need for<br />

inpatient care.<br />

Hospitals are responsible for notifying the<br />

ERO relative to the following admissions:<br />

• All elective medical admissions (excluding<br />

maintenance chemotherapy). <strong>Wisconsin</strong><br />

Medicaid defines an elective hospitalization<br />

as an admission that may be delayed<br />

without substantial risk to the patient.<br />

• All elective admissions for surgical<br />

procedures identified on the ERO’s<br />

outpatient procedure list.<br />

• All substance abuse admissions to general<br />

hospitals (all ages).<br />

• All elective psychiatric admissions to<br />

general hospitals (all ages).<br />

• All substance abuse admissions of<br />

individuals under age 21 to a specialty<br />

hospital (Institution for Mental Disease<br />

[IMD]).<br />

• All psychiatric admissions of individuals<br />

under age 21 to specialty hospitals (IMD).<br />

• All elective admissions to IMD for<br />

recipients age 65 or over.<br />

Certificate of Need Requirements<br />

for Recipients Admitted to an<br />

Institution for Mental Disease<br />

Federal <strong>and</strong> state regulations require IMDs to<br />

conduct <strong>and</strong> document a certification of need<br />

(CON) assessment for all recipients under the<br />

age of 21 who are admitted for elective/urgent<br />

or emergency psychiatric or substance abuse<br />

treatment services.<br />

The CON assessments must be completed by<br />

a team of professionals, including at least one<br />

physician, working in cooperation with the<br />

hospital. The completed CON form must be<br />

readily available for ERO or DHFS review.<br />

H<strong>and</strong>book, which is available on the Medicaid<br />

Web site. Providers without Internet access<br />

may call Provider Services.<br />

Immunizations<br />

<strong>Wisconsin</strong> Medicaid covers the immunizations<br />

identified by CPT subsections “Immune<br />

Globulins” (procedure codes 90281-90399) <strong>and</strong><br />

“Vaccines, Toxoids” (procedure codes 90476-<br />

90749).<br />

Immune globulin procedure codes <strong>and</strong> the<br />

unlisted vaccine/toxoid procedure code are<br />

manually priced by <strong>Wisconsin</strong> Medicaid’s<br />

pharmacy consultant. To be reimbursed for<br />
