2.1.8 Pooled analysis, meta-analyses, and<br />

o<strong>the</strong>r studies<br />

Steenland et al. (2001) rep<strong>or</strong>ted on a case–<br />

control analysis nested with<strong>in</strong> a pooled study <strong>of</strong><br />

data from ten coh<strong>or</strong>ts from a variety <strong>of</strong> <strong>in</strong><strong>dust</strong>ries<br />

and countries (Table 2.2 onl<strong>in</strong>e). It <strong>in</strong>cluded<br />

<strong>in</strong><strong>f<strong>or</strong>m</strong>ation on diatomaceous, granite, <strong>in</strong><strong>dust</strong>rial<br />

sand, and pottery w<strong>or</strong>kers, and w<strong>or</strong>kers <strong>in</strong> tungsten,<br />

t<strong>in</strong>, and gold m<strong>in</strong>es. Results from all <strong>of</strong> <strong>the</strong><br />

studies had been previously published, although<br />

not all had <strong>or</strong>ig<strong>in</strong>ally employed quantitative estimates<br />

<strong>of</strong> <strong>crystall<strong>in</strong>e</strong> <strong>silica</strong> exposure; and f<strong>or</strong> half,<br />

<strong>the</strong> duration <strong>of</strong> follow-up had been extended. All<br />

<strong>in</strong>dices <strong>of</strong> cumulative <strong>crystall<strong>in</strong>e</strong> <strong>silica</strong> exposure<br />

(cumulative, unlagged and lagged; log cumulative,<br />

unlagged and lagged) showed highly significant<br />

trends with lung cancer risk (P < 0.0001), and<br />

average exposure demonstrated a less significant<br />

trend (P < 0.05). Of <strong>the</strong>se <strong>in</strong>dices, log cumulative<br />

exposure led to homogeneity <strong>in</strong> f<strong>in</strong>d<strong>in</strong>gs across<br />

<strong>the</strong> coh<strong>or</strong>ts (P = 0.08 and 0.34 f<strong>or</strong> unlagged and<br />

15-year lag respectively). F<strong>in</strong>d<strong>in</strong>gs were similar<br />

f<strong>or</strong> <strong>the</strong> m<strong>in</strong><strong>in</strong>g and non-m<strong>in</strong><strong>in</strong>g subgroups.<br />

No adjustment was made f<strong>or</strong> smok<strong>in</strong>g and<br />

o<strong>the</strong>r confounders, although it was noted that<br />

smok<strong>in</strong>g had previously been shown not to be<br />

a maj<strong>or</strong> confounder <strong>in</strong> five <strong>of</strong> <strong>the</strong> ten studies.<br />

Analyses <strong>of</strong> subsets <strong>of</strong> <strong>the</strong> data omitt<strong>in</strong>g coh<strong>or</strong>ts<br />

with suspected o<strong>the</strong>r confounders (radon <strong>in</strong><br />

South African gold m<strong>in</strong>es, and arsenic <strong>or</strong> PAHs<br />

<strong>in</strong> Ch<strong>in</strong>ese t<strong>in</strong> m<strong>in</strong>ers and pottery w<strong>or</strong>kers) did<br />

not change <strong>the</strong> overall f<strong>in</strong>d<strong>in</strong>gs. [The W<strong>or</strong>k<strong>in</strong>g<br />

Group noted that <strong>the</strong> robustness <strong>in</strong> <strong>the</strong> f<strong>in</strong>d<strong>in</strong>gs to<br />

exclusion <strong>of</strong> coh<strong>or</strong>ts with potential confounders<br />

from o<strong>the</strong>r occupational exposures <strong>in</strong>dicates that<br />

any confound<strong>in</strong>g <strong>in</strong> <strong>the</strong> <strong>in</strong>dividual studies were<br />

unlikely to have had an impact on <strong>the</strong>ir f<strong>in</strong>d<strong>in</strong>gs<br />

related to <strong>crystall<strong>in</strong>e</strong> <strong>silica</strong>.]<br />

The presence <strong>of</strong> silicosis <strong>in</strong> an <strong>in</strong>dividual is a<br />

biomarker <strong>of</strong> high exposure to <strong>crystall<strong>in</strong>e</strong> <strong>silica</strong><br />

<strong>dust</strong>. Acc<strong>or</strong>d<strong>in</strong>gly, studies <strong>of</strong> <strong>in</strong>dividuals with<br />

silicosis have <strong>the</strong> potential to provide useful<br />

<strong>in</strong><strong>f<strong>or</strong>m</strong>ation on <strong>the</strong> lung cancer risk associated<br />

with exposure to <strong>crystall<strong>in</strong>e</strong> <strong>silica</strong>. Three metaanalyses<br />

have focused on <strong>the</strong> risk <strong>of</strong> lung cancer<br />

among <strong>in</strong>dividuals with silicosis (Smith et al.,<br />

1995; Tsuda et al., 1997; Lacasse et al., 2005). Erren<br />

et al. (2009) also provide summary <strong>in</strong><strong>f<strong>or</strong>m</strong>ation<br />

<strong>in</strong> an electronic supplement to <strong>the</strong>ir article. Four<br />

o<strong>the</strong>rs have looked at <strong>crystall<strong>in</strong>e</strong> <strong>silica</strong> exposure<br />

(<strong>in</strong>clud<strong>in</strong>g silicosis status unknown and those<br />

without silicosis; Steenland & Stayner, 1997;<br />

Kurihara & Wada, 2004; Pelucchi et al., 2006;<br />

Erren et al., 2009). The number <strong>of</strong> studies <strong>in</strong>cluded<br />

ranged from 11 <strong>in</strong> a meta-analysis focused on<br />

<strong>in</strong>dividuals without silicosis (Erren et al., 2009)<br />

to 43 (Pelucchi et al., 2006) <strong>in</strong> a study <strong>of</strong> those<br />

with and without silicosis. Reasons f<strong>or</strong> this variation<br />

<strong>in</strong>cluded: <strong>the</strong> publication date, <strong>the</strong> time<br />

period <strong>of</strong> <strong>in</strong>terest, whe<strong>the</strong>r <strong>the</strong> study was focused<br />

on those with <strong>or</strong> without silicosis, <strong>the</strong> <strong>or</strong>ig<strong>in</strong>at<strong>in</strong>g<br />

country <strong>of</strong> <strong>the</strong> studies, and analysis-specific<br />

criteria. F<strong>or</strong> example, Steenland & Stayner (1997)<br />

rejected studies <strong>of</strong> m<strong>in</strong>ers and foundry w<strong>or</strong>kers<br />

on <strong>the</strong> assumption that <strong>the</strong>y had <strong>the</strong> greatest<br />

potential f<strong>or</strong> confound<strong>in</strong>g exposures, and Smith<br />

et al. (1995) rejected certa<strong>in</strong> studies <strong>the</strong>y deemed<br />

under <strong>or</strong> overestimated <strong>the</strong> risk <strong>of</strong> lung cancer.<br />

Overall, <strong>of</strong> <strong>the</strong> total <strong>of</strong> 112 publications <strong>in</strong>cluded<br />

by one <strong>or</strong> m<strong>or</strong>e <strong>of</strong> <strong>the</strong> seven meta-analyses, none<br />

were common to all analyses.<br />

analyses except f<strong>or</strong> those devoted to categ<strong>or</strong>ies<br />

without silicosis found an elevated lung cancer<br />

risk, whe<strong>the</strong>r occurr<strong>in</strong>g among those with<br />

silicosis <strong>or</strong> among <strong>crystall<strong>in</strong>e</strong>-<strong>silica</strong>-exposed<br />

w<strong>or</strong>kers, <strong>or</strong> aris<strong>in</strong>g from coh<strong>or</strong>t <strong>or</strong> case–control<br />

studies. [The W<strong>or</strong>k<strong>in</strong>g Group noted that studies<br />

that restrict <strong>the</strong>ir analysis to <strong>in</strong>dividuals without<br />

silicosis potentially limit <strong>the</strong>ir range <strong>of</strong> <strong>crystall<strong>in</strong>e</strong><br />

<strong>silica</strong> exposure, because <strong>in</strong>dividuals with <strong>the</strong><br />

highest exposures tend to be omitted. Limit<strong>in</strong>g<br />

<strong>the</strong> range <strong>of</strong> exposure results <strong>in</strong> reduced power to<br />

detect associations.] Overall, <strong>the</strong> rate ratios were<br />
