MTD – WGPSN Findings • Mining the Diamond (MTD) Concept introduced at 2011 Arctic Projects Executive Contractor SSH&E Forum and subsequent HSE Project Safety Leadership Team meetings (January 2012). • It was evident that careful study of observational data would be essential in the formulation of effective intervention strategies. • WGPSN reviewed current processes and committed to analyze the increased number of Observational Safety cards submitted to determine the Potential Hurt Level for each observation. • It has been observed through 2011 and 2012 that there has been more Near Misses than have been reported as Observational Safety cards and that several cards carried Minor Hurt and Moderate Hurt potential.
MTD - Forward Action Plan • Observation cards which could have been recorded as a Near Miss are to be fed back to Construction Supervision to ensure appropriate tracking and investigation takes place. • Near Miss Awareness training is being developed as part of WGPSN HSE Improvement Planning. • Incident Safety Bulletins are being issued to highlight requirement for accurate reporting and to communicate the Potential Hurt associated with an observation.
