WRAPAROUND: Three major collaborative partners in our community -<br />

Summit Pointe, Juvenile Court, and the Department of Human Services<br />

are blending state funds in order to treat high-risk youth and their<br />

families more effi ciently within the community thus reducing the need<br />

for and cost of out of home placements. Wraparound implements<br />

family friendly strategies that focus on family strenths, identify and remove<br />

barriers for families, risk, needs, and protective factors with youth<br />

in the community. Since 2008, the Wraparound program has seen a<br />

100% growth in the number of families served.<br />

YOUTH LEADERSHIP: <strong>17</strong>2 young men attended ‘Take the Lead’, a day sponsored by Summit Pointe that<br />

provided middle school and high school students with an opportunity to interact and learn outside the<br />

classroom. <strong>The</strong> agenda provided an opportunity for the student to learn appropriate confl ict resolution<br />

skills and learn about resume development. <strong>The</strong> event was held at McCamly Plaza Hotel which allowed<br />

a number of business leaders from the community to join the students for an<br />

interactive lunch discussion about careers and employment opportunities.<br />

ADULT SERVICES: Summit Pointe<br />

Mental Illness (SMI). <strong>The</strong><br />

served from FY 08 to FY 09<br />

stemmed from a range of<br />

factors (changes in how<br />

customers were to be<br />

counted; General Fund<br />

cutbacks; decreases in<br />

Medicaid HMO members; shifts<br />

to Homelessness Program; and<br />

increased recovery-based<br />

services offered by the<br />

Veteran Administration).<br />

Services available to adults with mental illness include:<br />

• Assertive Community Treatment (ACT) - A multidisciplinary wraparound service designed to treat all<br />

aspects of care for the most acute and challenging customers with serious and persistent mental illness.<br />

• Client Services Management (CSM) - A community-based care coordination service that assists<br />

customers with serious and persistent mental illness obtaining services and supports that contribute to<br />

stability, recovery, and self-management of their illness.<br />

• Outpatient Services - Short-term therapy services to address emotional issues in persons dealing with<br />

emotional upset or mental illness.<br />

• NET Program - A medication-only service for persons who only require limited service involvement.<br />
