quality on DISPlay: Two Honorees Share Their Work The NHPCO Quality and Standards Committee held its second annual “Quality on Display” event at the November 2012 Clinical Team Conference. In light of the hospice quality reporting requirements (and the October 1, 2012 deadline to begin data collection), the Committee asked providers to share—on 4-by-8-foot visual displays—how they were preparing their organizations and staff. NewsLine is proud to share the work of BAYADA Hospice in Moorestown, NJ, and Hospice of the Bluegrass in Lexington, KY. Both organizations chose to showcase their strategies for implementation of the NHPCO Comfortable Dying Measure (NQF 0209), one of the two measures currently required for quality reporting to CMS. While their visual displays cannot be duplicated here, each organization has graciously created a PDF presentation that outlines its strategy and implementation process. 22 NewsLine
BAYADA Hospice • Moorestown, NJ There were three primary components in our strategy: • Education: This included providing staff with an overview of the NHPCO Comfortable Dying Measure and a six-page FAQ document, where detailed answers to the measure were provided. • Technology Design: This included the revamping of assessments to determine eligibility for participation in the measure, as well as collaboration with our EMR vendor, Homecare Homebase. • Process Design: It was also critical to have a process in place to identify who in the organization will be responsible for the daily logistics of ensuring that every patient requiring follow-up receives it in a timely manner. While we were strategically positioned to gather and report on this measure through our partnership with Homecare Homebase, it did take several months to prepare our practice for this change. The Hospice Services (HOS) office developed education for our staff about this new measure, including a mandatory assessment through the Learning Management System (LMS). This was accomplished with much assistance from our Visit Virtual Office senior associate, Margaret Donohue, and our Learning Curriculum and Design training manager, Victoria McDevitt. Changes to the Homecare Homebase assessments also went into effect, so that our admissions nurses could easily identify a patient who requires follow-up and should be reported in this measure. Additionally, HOS worked with input from four other hospice directors—Kristin Barnum, Linda Trout, Phyllis Tarbell, and Angela Snyder—to determine how to adapt daily operations to ensure that BAYADA staff was educated about this requirement and available for the required 48-to-72 hour follow-up, even on weekends and holidays.
