DMV TITLE AND REGISTRATION HANDBOOK Chapter A: General Title Requirements CHECKLIST FOR SUBMITTING TRANSACTIONS DMV has compiled this checklist for you to use as a tool when preparing transactions. These are common items found missing on transactions submitted. Please check for the following before submitting a title transaction: CHECKLIST FOR SUBMITTING TRANSACTIONS 1. FEES a. The correct fees have been collected (replacement plates, plate transfers, custom plates and plate surcharges). b. A clear explanation is given of the registration fees that have been collected (i.e. replacement plates, plate transfers, plate surcharges, etc.). c. The shaded fee section on the front of the application for title is completed to break down title and registration fees. Although this area says “For DMV Use Only,” DMV prefers that you complete the fee section of the application. 2. THE APPLICATION CLEARLY LISTS ALL REGISTERED OWNERS OR LESSEE/LESSORS AND ANY SECURITY INTEREST HOLDER(S). d. The customer number and date of birth for all owners are listed as well as customer numbers for businesses and dates of birth if applicable. The name and address on the application matches the name and address on their customer file for the first owner listed (check against their Oregon Driver License, Oregon ID Card or Instruction Permit). Registered owners who do not have an Oregon Driver License will be assigned a Customer ID number from DMV. It is important to enter the name and date of birth for each registered owner correctly. e. If there are two signatures on the bottom of the application for title, both parties must be listed as owners on the application (joint registered owners or lessee/lessor). f. If one or more of the owners listed on the application are the same as on the ownership document, the current owner(s) must acknowledge and agree to the addition. If a registered owner is added or removed from a title with a security interest holder, DMV needs authorization from the SIH listed on the title. g. If the registered owner is an individual (not a business) you must provide the residence address. (A PO Box by itself is not acceptable, even for small towns.) h. Survivorship may be requested by checking a YES or NO box to indicate survivorship, when two or more new owners are listed on the application for title. If neither box is checked, NO survivorship will be shown on the title. A-4 7/1/05

THE COMPLETE VEHICLE DESCRIPTION (YEAR, MAKE, BODY STYLE, VIN) IS ON THE APPLICATION. i. The VIN on the application for title matches that on the vehicle and the ownership document (title, replacement title application, etc.). If the VIN on the vehicle is different than on the title, you may need to get a more thorough VIN inspection or a corrected title depending on where the error occurs in the VIN. j. The VIN on the application for title matches the MCO. If the VIN on the vehicle is different than on the MCO, you will need to get the correct MCO from the manufacturer. k. The plate number of a transferred Oregon plate is listed in the "new plate number" box and the vehicle information for the vehicle from which the plate came is in the "remarks" section on either the front or back of the application. 4. ALL ERRORS MADE ON THE DOCUMENTS ARE EXPLAINED. l. A Statement of Error or Erasure, Form 735-502, should be completed and attached for name errors only. m. A new odometer disclosure must be obtained for errors made while recording the mileage. n. Other errors such as in the vehicle description or VIN or address information should be corrected. (see section titled Corrections On Title Applications in Chapter E.) 5. THE POWER OF ATTORNEY (POA) FORM IS COMPLETED AND ATTACHED. o. The VIN and the named attorney are included on the POA. p. The POA is properly exercised on the title. q. The POA is attached to the application. r. If the POA is a secure POA (for odometer purposes), be sure it is allowed to be used for this type of transaction. 6. THE FORMER OWNERS HAVE RELEASED THEIR INTEREST IN THE VEHICLE. s. The dealer release is included on the back of the title, on an odometer disclosure form or on a separate bill of sale. t. Connecting releases for all transfers of ownership of the vehicle are included. u. The releases match the name exactly as it appears on the title. 7. THE "REMARKS" SECTION ON THE 735-226 IS COMPLETED WITH ALL INFORMATION NOT INDICATED ANYWHERE ELSE ON THE APPLICATION. v. There is a clear explanation of why you collected replacement plate fees. (What happened to the old plates?) w. You listed what vehicle the plates came from for plate transfers, including the complete VIN for the vehicle the plates came from. 8. YOU ATTACHED ALL OF THE NECESSARY FORMS SUCH AS THE POA, BILLS OF SALE, LIEN SATISFACTIONS, DEQ CERTIFICATION AND STATEMENT OF ERROR OR ERASURE. 7/1/05 A-5
