<strong>DMV</strong> TITLE AND REGISTRATION HANDBOOK<br />

Chapter A: General <strong>Title</strong> Requirements<br />

CHECKLIST FOR SUBMITTING TRANSACTIONS<br />

<strong>DMV</strong> has compiled this checklist for you to use as a tool when preparing transactions. These are common<br />

items found missing on transactions submitted. Please check for the following before submitting a title transaction:<br />

1. FEES<br />

a. The correct fees have been collected (replacement plates, plate transfers, custom plates<br />

<strong>and</strong> plate surcharges).<br />

b. A clear explanation is given <strong>of</strong> the registration fees that have been collected (i.e.<br />

replacement plates, plate transfers, plate surcharges, etc.).<br />

c. The shaded fee section on the front <strong>of</strong> the application for title is completed to break<br />

down title <strong>and</strong> registration fees. Although this area says “For <strong>DMV</strong> Use Only,” <strong>DMV</strong><br />

prefers that you complete the fee section <strong>of</strong> the application.<br />

2. THE APPLICATION CLEARLY LISTS ALL REGISTERED OWNERS OR<br />

LESSEE/LESSORS AND ANY SECURITY INTEREST HOLDER(S).<br />

d. The customer number <strong>and</strong> date <strong>of</strong> birth for all owners are listed as well as customer<br />

numbers for businesses <strong>and</strong> dates <strong>of</strong> birth if applicable. The name <strong>and</strong> address on the<br />

application matches the name <strong>and</strong> address on their customer file for the first owner<br />

listed (check against their <strong>Oregon</strong> Driver License, <strong>Oregon</strong> ID Card or Instruction Permit).<br />

Registered owners who do not have an <strong>Oregon</strong> Driver License will be assigned a<br />

Customer ID number from <strong>DMV</strong>. It is important to enter the name <strong>and</strong> date <strong>of</strong> birth for<br />

each registered owner correctly.<br />

e. If there are two signatures on the bottom <strong>of</strong> the application for title, both parties must<br />

be listed as owners on the application (joint registered owners or lessee/lessor).<br />

f. If one or more <strong>of</strong> the owners listed on the application are the same as on the ownership<br />

document, the current owner(s) must acknowledge <strong>and</strong> agree to the addition. If a<br />

registered owner is added or removed from a title with a security interest holder, <strong>DMV</strong><br />

needs authorization from the SIH listed on the title.<br />

g. If the registered owner is an individual (not a business) you must provide the residence<br />

address. (A PO Box by itself is not acceptable, even for small towns.)<br />

h. Survivorship may be requested by checking a YES or NO box to indicate survivorship,<br />

when two or more new owners are listed on the application for title. If neither box is<br />

checked, NO survivorship will be shown on the title.<br />
