6.1.5 Monitor<strong>in</strong>g and Audit<strong>in</strong>g <strong>of</strong> <strong>Controlled</strong> <strong>Drug</strong> <strong>Use</strong><br />

All trusts have systems <strong>in</strong> place for <strong>the</strong> monitor<strong>in</strong>g and audit<strong>in</strong>g <strong>of</strong> controlled<br />

drugs at pharmacy and ward level. Where appropriate, each ward has a list<br />

specify<strong>in</strong>g which controlled drugs should be held as stock, and this list is<br />

regularly reviewed.<br />

Regular medic<strong>in</strong>es reconciliation checks should be carried out to ensure that<br />

nurs<strong>in</strong>g staff know when to order new stock and rotate stock so that it does<br />

not become out-<strong>of</strong>-date.<br />

Quarterly pharmacy audits <strong>of</strong> wards are part <strong>of</strong> trust controls assurance<br />

standards for medic<strong>in</strong>es management. The purpose <strong>of</strong> ward pharmacy audits<br />

is to ensure that:<br />

all controlled drugs are accounted for;<br />

usage appears to be appropriate;<br />

staff are adher<strong>in</strong>g to policy and procedure <strong>in</strong> relation to supply, storage<br />

and record keep<strong>in</strong>g.<br />

In <strong>the</strong> Western Trust, discharge prescriptions are checked by a pharmacist,<br />

and any concerns relat<strong>in</strong>g to controlled drugs raised with <strong>the</strong> AO. Cl<strong>in</strong>ical<br />

pharmacists review patient kardexes at ward level, and ward pharmacists<br />

carry out quarterly medic<strong>in</strong>es management checks.<br />

All concerns relat<strong>in</strong>g to <strong>the</strong> adm<strong>in</strong>istration <strong>of</strong> controlled drugs are raised with<br />

ward managers and highlighted to <strong>the</strong> AO. Monthly drugs <strong>of</strong> potential abuse<br />

reports are reviewed quarterly by pharmacists with ward managers and<br />

concerns are forwarded to <strong>the</strong> AO. Regular pharmacy and ward stock checks<br />

are carried out and an annual controlled drug audit is carried out <strong>in</strong> its<br />

maternity services. Omitted doses <strong>of</strong> medic<strong>in</strong>es are audited by nurs<strong>in</strong>g staff<br />

as part <strong>of</strong> National Patient Safety Association (NPSA) missed dose<br />

recommendations.<br />

In <strong>the</strong> South Eastern Trust, all discharge prescriptions are checked by a<br />

pharmacist, and patient kardexes are regularly reviewed dur<strong>in</strong>g <strong>in</strong>patient<br />

stays. <strong>Drug</strong>s <strong>of</strong> potential abuse reports are reviewed on a regular basis.<br />

All areas us<strong>in</strong>g controlled drugs undergo quarterly audits <strong>of</strong> controlled drugs,<br />

conducted by ward pharmacists, with <strong>the</strong> results forwarded to <strong>the</strong> AO.<br />

Audit f<strong>in</strong>d<strong>in</strong>gs are forwarded to cl<strong>in</strong>ical managers for discussion at ward<br />

managers’ meet<strong>in</strong>gs where action plans are drawn up to address deficiencies<br />

<strong>in</strong> practice. Results <strong>of</strong> three monthly controlled drugs checks are also<br />

discussed at <strong>the</strong> controls assurance medic<strong>in</strong>es management subgroup.<br />

Roll<strong>in</strong>g programmes <strong>of</strong> controlled drug checks are carried out <strong>in</strong> pharmacies<br />

and <strong>in</strong> wards where <strong>the</strong>re are a m<strong>in</strong>imum <strong>of</strong> twice daily stock checks.<br />
