to enforce their legal rights.
They investigate complaints against
hospitals, nursing homes, home
health agencies, assisted living, supervised
living facilities and boardand-care
facilities.
Office of Ombudsman for
Long Term Care

Advocates for person-directed living,
throughout the health care continuum,
which respects individual values
and preferences and preserves individual
rights. An ombudsman is an
independent consumer advocate.
Ombudsmen investigate complaints
concerning the health, safety, welfare
and rights of long-term care
consumers, work to resolve individual
concerns, identify problems and
advocate for changes to address
them at no charge to the consumer.
CHORE & MINOR
MAINTENANCE PROGRAMS
Helps seniors maintain their homes
by making referrals for maintenance
chores such as snow shoveling/removal,
yard work and housekeeping.
Some of these programs use a
sliding-fee arrangement in determining
the cost for services while others
are market rate. Those who meet income
guidelines may be eligible for
housekeeping services through their
human service offices.

FINANCIAL ASSISTANCE
- PROGRAMS COUNSELING
Financial assistance is available
to seniors with limited monthly income
and resources. There are
specific guidelines which qualify
individuals for these assistance
programs. Call your local county
human services department
about the various programs.
Social Security Administration

regarding retirement, survivor’s
benefits, Medicare and Supplemental
Security Income (SSI). SSI
guarantees additional monthly income
to low-income seniors, ages
65 and older, and physically challenged
individuals.
ENERGY & FUEL ASSISTANCE
Energy Assistance Program
(EAP) Referral Line

Call to locate the local agency
serving your county that can
provide help to qualified households
for assistance with heating
bills. Households with the lowest
monthly incomes and highest energy
costs receive priority. You do
not need to be behind in heating
bills to qualify. EAP may make referrals
to resources to help with
furnace repairs or replacement
and other weatherization services
if necessary.

metropolitan area.
Care management and health care advocacy
for seniors, people living with a disability or
