Units providing dementia programs
must comply with the disclosure law
advanced by the State legislature.
The points of disclosure are:
• Statement of overall philosophy
& how it reflects residents’ special
dementia needs;
• Criteria to determine who may
reside in Special Care Unit;
• Process used for assessment &
establishment of service plan;
• Staffing credentials, job descriptions
& staff duties & availability including
dementia specific training;
• Physical environment as well as
design & security features;
• Frequency & type of programs &
activities;
• Involvement of families & availability
of family support programs;
• Fee schedules for additional services; &
• Statement that residents will
receive written notice 30 days prior
to changes in fee schedule.
