<strong>JOB</strong> SUMMARY<br />
The postholder is a designated Health Visiting Team Manager in the Dungannon<br />
locality. He/she will lead and coordinate the delivery of a high quality and efficient<br />
health visiting service. He/she will enhance multidisciplinary working and will work<br />
across multidisciplinary and organisational boundaries to promote the welfare of<br />
individuals, families and communities. He/she will promote and develop appropriate<br />
strategies and opportunities to share knowledge and influence practice through<br />
research and education. The postholder will be available to advise Senior Managers<br />
about matters relating to the health visiting service and family/client groups.<br />
KEY DUTIES / RESPONSIBILITIES<br />
Providing Effective Health Care<br />
1.1 To be responsible for the day to day management and co-ordination of the<br />
designated services as described above, including management of budgets<br />
and relevant cost centres.<br />
1.2 To ensure adequate staff coverage of the area at all times, by planning off<br />
duty and annual leave, having regard for service needs and the cost-effective<br />
delivery of care.<br />
1.3 To assess, develop, implement and evaluate programmes of care within the<br />
team and in accordance with Trust policy.<br />
1.4 Monitor the work of the team to ensure equal distribution of work using<br />
individual work and caseload profiles as a measuring tool.<br />
1.5 To promote the use of a range of different assessment methods through<br />
appropriate nursing professionals to ensure valid, reliable and comprehensive
client centred conclusions, which manage risk and are appropriate to need,<br />
context and culture.<br />
1.6 To identify and determine therapeutic programmes which are based on<br />
evidence in the interest of patients/clients and which involve other<br />
practitioners to improve health and social care outcomes.<br />
1.7 To co-ordinate the use of evidence based therapeutic programmes, ensuring<br />
their execution is appropriate and effective in meeting client need.<br />
1.8 Oversee delivery of the Regional Child Health Promotion Programme for<br />
respective teams as laid down at Regional and Board level.<br />
1.9 Ensure delivery of parenting programmes as laid down in regional guidance<br />
and appropriate evaluation.<br />
1.10 Ensure the delivery of services which support families ensuring an emphasis<br />
on the anticipation and prevention of problematic or dysfunctional parenting<br />
and its impact on children.<br />
1.11 Oversee the delivery of services which include the provision of advice and<br />
support on interventions in dealing with children with behavioural disorders or<br />
physical and mental disorders.<br />
1.12 Identify the need for health education models within the context of the relevant<br />
teams and plans programmes accordingly.<br />
1.13 To ensure delivery of areas of clinical practice such as nurse prescribing in<br />
line with current best practice as laid down within Trust guidelines.<br />
1.14 Organise and oversee the provision of child health clinics as appropriate<br />
within the community setting.<br />
1.15 Monitor and promote the regional immunisation programmes aiming to<br />
maintain and improve uptake rates within the Trust population.<br />
1.16 To ensure practice is delivered adhering to regional and Trust child protection<br />
policies, procedures, clinical protocols and guidelines based on legislation,<br />
safeguarding policies, evidenced based nursing practice and the NMC<br />
guidelines for professional practice.<br />
1.17 Liaise closely on matters/concerns relating to child protection with the Child<br />
Protection Nurse Specialist and Deputy / Head of Health Visiting and School<br />
Nursing and provide advice to relevant staff or attend Case Conferences in<br />
the absence of the Child Protection Nurse Specialist.<br />
1.18 Participate in established communication systems within the Trust.
Improving Quality and Health Outcomes<br />
2.1 To participate in and assist in the development of clinical and social care<br />
governance arrangements in the context of his/her sphere of responsibility.<br />
This will include the co-ordination of Risk Assessments within his/her sphere<br />
of responsibility.<br />
2.2 To maximise opportunities to apply new knowledge to his/her own practice<br />
and the practice of the team in a structured way that is capable of evaluation.<br />
2.3 To advise on the assessment of risk and risk management to nurse<br />
managers, senior nurses and other agencies.<br />
2.4 To advise senior managers on the fulfillment of service requirements and<br />
quality standards.<br />
2.5 To participate in and advise on audit of current health visiting practice in the<br />
Trust.<br />
2.6 To promote the improvement of quality and clinical effectiveness within<br />
resource constraints.<br />
2.7 To practice within the NMC code of professional conduct and to monitor that<br />
all team members practice under the code.<br />
2.8 To participate in the Trust’s programme of individual performance review with<br />
the Deputy/ Head of Health Visiting and School Nursing and the Health<br />
Visiting Team.<br />
Evaluation and Research<br />
3.1 To develop policies, procedures, clinical protocols and guidelines based<br />
legislation; evidence based nursing practice and NMC guidelines for<br />
professional practice.<br />
3.2 Critically appraise and synthesise the outcomes of relevant research,<br />
evaluations, audits and policy documents and apply them to practice or make<br />
comments on elements of risk.<br />
3.3 Alert appropriate individuals and organisations to gaps in evidence and or<br />
practice knowledge that require resolution through research.<br />
3.4 To ensure collection of statistical data relevant to Health Visiting, initiating<br />
research to improve and enhance nursing practice.<br />
Leading and Developing Practice<br />
4.1 To provide individual support and guidance to nurses within the team.
4.2 To provide leadership, supervision, advice and support for nursing staff, to<br />
enable effective working in the best interest of the client group<br />
4.3 To assess education and training needs with the team and accessing unidisciplinary<br />
and multi-disciplinary training.<br />
4.4 To ensure staff has access to legal advice when appropriate.<br />
4.5 To co-ordinate and contribute to induction programmes for new staff.<br />
4.6 To ensure that there is a supportive learning environment within the team<br />
where pre and post registration students have access to a mentor / practice<br />
teacher as appropriate.<br />
Innovation and Changing Practice<br />
5.1 To advise on the development of practice and health and social care<br />
outcomes through evolving national, regional and local policy; facilitating<br />
change in ways which are effective and in the context of Health Visiting<br />
practice.<br />
5.2 To assist in the development of strategies, resources and technologies, which<br />
positively impact on the health and social care outcomes of clients.<br />
5.3 To network with appropriate professionals to share innovation, developing self<br />
and others in the area of relevant nursing health visiting practice.<br />
5.4 To ensure his/her own practice is developing and improving in a structural<br />
way be accessing inter-professional clinical supervision and appropriate<br />
training.<br />
5.5 To identify and report on a need for resources to improve his/her own learning<br />
and practice and that of the teams for which he/she is responsible.<br />
WORKING ACROSS PROFESSIONAL AND ORGANISATION BOUNDARIES<br />
6.1 To foster, develop and sustain appropriate relationships and networks across<br />
professional and organisational boundaries to improve health and social care<br />
outcomes and health and social care delivery to the identified client group.<br />
6.2 To act as a point of co-ordination with other agencies.<br />
WORKING RELATIONSHIPS<br />
7.1 To be directly responsible and accountable to the Deputy / Head of Health<br />
Visiting and School Nursing for the provision of an effective, efficient
Department.<br />
7.2 To closely liaise with all Senior Managers in order to effectively discharge the<br />
stated duties of the post.<br />
7.3 To act up in the absence of the Deputy /Head of Health Visiting and School<br />
Nursing as required, or represent the Deputy/Head of Health Visiting and<br />
School Nursing at delegated meetings.<br />
GENERAL MANAGEMENT RESPONSIBILTIES<br />
1. Review individually, at least annually, the performance of immediately<br />
subordinate staff, provides guidance on personal development requirements and<br />
advises on and initiates, where appropriate, further training. ( including KSF<br />
appraisal reviews)<br />
2. Maintain staff relationships and morale amongst the staff reporting to him/her.<br />
3. Review the organisation plan and establishment level of the service for which<br />
he/she is responsible to ensure that each is consistent with achieving objectives,<br />
and recommend change where appropriate.<br />
4. Delegate appropriate responsibility and authority to the level of staff within<br />
his/her control consistent with effective decision making, while retaining overall<br />
responsibility and accountability for results.<br />
5. Participate, as required, in the selection and appointment of staff reporting to<br />
him/her in accordance with procedures laid down by the Trust.<br />
6. Take such action as may be necessary in disciplinary matters in accordance with<br />
procedures laid down by the Trust.<br />
GENERAL REQUIREMENTS<br />
The post holder will be required to:<br />

Protection Acts 1998. Employees are required to be conversant with the<br />

whom he/she comes into contact in the course of work, in a pleasant, courteous<br />
and respectful manner.<br />
9. Available / able to work any 5 days out of 7 over the 24 hour period, which<br />
may include on-call / stand-by / sleep-in duties, shifts, night duty, weekends<br />
and Public Holidays if required immediately on appointment or at a later stage<br />
following commencement in response to changing demands of the service.<br />
10. Understand that this post may evolve over time, and that this Job Description<br />
will therefore be subject to review in the light of changing circumstances.<br />

Waiting List Information<br />
Following interviews for the above post a waiting list may be compiled for future Permanent<br />
and Full-Time Health Visiting Team Manager Band 7 posts based throughout the<br />
Southern Health and Social Care Trust. This waiting list may be held for a period of 9 - 12<br />
months from the date of interview<br />

1 You must be an employee of the Southern Health & Social Care Trust or an<br />
individual participating in the Trust’s Employability Project or the Trust’s<br />
Disability Project, to be eligible to apply for this post.<br />

2 Degree in Nursing or recognised professional qualification or equivalent /<br />
Higher qualification AND 3 years Health Visiting Experience<br />
3 Be a Registered Nurse or Midwife and Registered Specialist Community<br />
Public Health Nurse (Health Visitor) currently on part one and part three of the<br />
NMC live register.<br />
4 Have experience of working with families where there are child protection<br />
concerns including recognition, referral and responding through multi-agency<br />
procedures.
5 Evidence of continuous personal development.<br />
6 Hold a full current driving license valid for use in the UK and have, on<br />
appointment, access to a car 1 .<br />
