Student Application Process 20 After you have explained to the students the financial and other aspects of participating in your program, you can advise them to refer to our webpage at http://www.international.appstate.edu/education/aoep/how where they will be able to access your program specific online student application. The AOEP Application Form includes: Memorandum about the U.S. Department of State’s Public Announcements Registration Information Participant Agreement Student Conduct Code for Participants in AOEP Study Abroad Health Report and Release Health Disclosure Consent for Release of Physical and Mental Health Information Consent for Release of Information to Parent/Guardian Judiciary Clearance (please see NOTE below) AOEP Student Handbook Acknowledgement Release for Use of Photograph Program courses, payment schedule and cancellation policy NOTE about the Judiciary Clearance Form: You may want to mention that the presence of a conduct record does not automatically disqualify students from a study abroad experience. For more information about this issue, please refer to the Students with Conduct Records section on page 28. Students need to know that they are not officially registered for the program until they bring their deposit payment by check or cash to OIED right after they have submitted their online application. You may want to explain to the students where our office is located on campus. If enrollment in your program is restricted, please let us know and provide us with Banner ID #s of "approved" students. Once a student has submitted deposit payment to OIED, you will receive an email notification of his/her official enrollment in your program along with the student’s information (name, banner id #, major, emergency contact information, medical information released, etc. In consultation with Student Health Services, OIED will address with you any student health concerns disclosed in the application form that may affect a student’s travel and overseas experience. Export Controls Regulations If export controls restrictions apply to your program, please inform the students at that time of any potential issues they need to be aware of. If you have questions about how your students should be briefed on this subject, please contact Julie Taubman, the Director of Research Protections, at 262-7981 or compliance@appstate.edu. 12.
