Data analysed for this chapter<br />

Data analysed for this chapter include all reports made to <strong>SHOT</strong> in the years 2006–<strong>2009</strong> including all Near Miss<br />

reports and reports that were later withdrawn.<br />

Introduction<br />

This is the second year that <strong>SHOT</strong> has presented participation data in this format. These data have been accumulated<br />

since the implementation <strong>of</strong> the BSQR 2005, 2 and, as with previous years, <strong>2009</strong> has seen an increase in the number <strong>of</strong><br />

reports made though the increase has not been as steep as that seen in 2008. The total number <strong>of</strong> reports analysed<br />

here is made up from those evaluated by <strong>SHOT</strong> as reportable (1279), reports that were classified as Near Miss (799),<br />

and reports later withdrawn as not <strong>SHOT</strong> reportable (397).<br />

The term ‘reporting organisation’ is used throughout this chapter. A reporting organisation may be a single hospital or<br />

may be a Trust or Health Board. While a reporting organisation may be registered on SABRE and report all <strong>of</strong> its incidents<br />

under a single login, there are a number <strong>of</strong> organisations that have several reporters registered for reporting to SABRE.<br />

For clarity <strong>SHOT</strong> has grouped all <strong>of</strong> the <strong>SHOT</strong> data downloaded from SABRE by reporting organisation, even where there<br />

are separate registered reporters from that site. In <strong>2009</strong> there were 295 SABRE registered reporters and there were 26<br />

reporters who made no report to the MHRA via SABRE. Some <strong>of</strong> these may have made <strong>SHOT</strong> only reports.<br />

The number <strong>of</strong> reports per 10,000 components issued has increased for the UK as a whole. However, there remains a<br />

marked variation between the four UK countries, with Wales increasing its reporting rate by 60% between 2008 and<br />

<strong>2009</strong>.<br />
