Checklist for Trainees Pre-Registered with the PGME Office:<br />

LETTER OF APPOINTMENT<br />

Every trainee must receive a Letter of Appointment from the<br />

PGME Office. You should return your signed LOA to the PGME<br />

Office within 30 days of its issuance. The academic session runs<br />

each year from July 1 st until June 30 th of the following calendar<br />

year. All appointments will therefore end on or before June 30 th .<br />

You will receive a separate LOA at a later date if your<br />

appointment extends beyond June 30 th of the following calendar<br />

year. The PGME Office is required to stamp receipt of the LOA<br />

and send to the College of Physicians and Surgeons of Ontario<br />

(CPSO) as part of your application.<br />

IMMUNIZATION - Send in your immunization data<br />

The immunization form that you receive from the PGME Office<br />

must be completed and signed by a clinic, health centre or<br />

health care professional other than yourself. Returning trainees<br />

need only submit their annual TB test results. Appointments for<br />

HBV carriers are conditional upon review of the Expert Panel on<br />

Infection Control.<br />

CPSO license<br />

Register (or renew your registration) with the College of<br />

Physicians and Surgeons of Ontario (CPSO)<br />

CMPA malpractice insurance<br />

Obtain membership in the Canadian <strong>Medical</strong> Protective<br />

Association (CMPA) for your professional liability protection in<br />

Ontario<br />
