Checklist for Trainees Pre-Registered with the PGME Office:
LETTER OF APPOINTMENT
Every trainee must receive a Letter of Appointment from the
PGME Office. You should return your signed LOA to the PGME
Office within 30 days of its issuance. The academic session runs
each year from July 1 st until June 30 th of the following calendar
year. All appointments will therefore end on or before June 30 th .
You will receive a separate LOA at a later date if your
appointment extends beyond June 30 th of the following calendar
year. The PGME Office is required to stamp receipt of the LOA
and send to the College of Physicians and Surgeons of Ontario
(CPSO) as part of your application.
IMMUNIZATION - Send in your immunization data
The immunization form that you receive from the PGME Office
must be completed and signed by a clinic, health centre or
health care professional other than yourself. Returning trainees
need only submit their annual TB test results. Appointments for
HBV carriers are conditional upon review of the Expert Panel on
Infection Control.
CPSO license
Register (or renew your registration) with the College of
Physicians and Surgeons of Ontario (CPSO)
CMPA malpractice insurance
Obtain membership in the Canadian Medical Protective
Association (CMPA) for your professional liability protection in
Ontario
