University of Toronto Faculty of Medicine PGME 2012-2013 Registration Process: Once the PGME Office receives the required documents from the University of Toronto postgraduate program, a Letter of Appointment (LOA) will be issued to the resident. This letter must be signed by the resident and returned to the PGME Office. A copy will be forwarded to the CPSO. Along with the LOA, the resident will receive a PIN/password letter. Using the PIN/Password, the resident will be able to register on-line at the PGME website, at http://pgme.med.utoronto.ca , and print off his/her: Confirmation of Registration Form– provided all the information noted above (1-6) has been received and the CPSO has issued a postgraduate medical education certificate of registration for the duration of the elective. The Confirmation of Registration form will allow the trainee to register at the appropriate hospital Credentials/Medical Education Office in Toronto and access library services. Please note that applications to University of Toronto postgraduate medical education departments for elective training must not exceed 6 months in duration. Updated: May 7, 2012 47
University of Toronto Faculty of Medicine PGME 2012-2013 D) For U.S. Citizens currently in Residency Training in the U.S. Eligibility for Licensure: In order to take up an elective at the University of Toronto, candidates must be eligible for licensure by the province’s medical licensing body, the College of Physicians and Surgeons of Ontario (CPSO). For applicable CPSO requirements for U.S. citizens who are graduates of a U.S. medical school and are currently in residency training in the United States, please visit the following link: http://www.cpso.on.ca/uploadedFiles/downloads/cpsodocuments/registr ation/CRPEelectiveCDNUS.pdf Because CPSO requirements are subject to change, candidates should contact the CPSO directly to confirm these requirements. Please consult the CPSO website at www.cpso.on.ca . Required Documents: U.S. citizens who wish to take an elective under the supervision of a University of Toronto postgraduate program director/training program must have the support of an acceptable sponsoring agency (embassy, university, hospital) and submit the following documentation through their sponsoring agency to the Toronto PGME Office: 1. written permission from the University of Toronto program director agreeing to the elective 2. written permission from current program director at the applicant’s home university 3. up-to-date curriculum vitae that includes the applicant’s date of birth, Social Insurance Number (SIN), citizenship status and e- mail address 4. copy of medical degree 5. complete immunization record 6. evidence of full financial support from the funding institution/organization for the duration of the elective training 48
