CALGARY AREA HOSPICES<br />

Patient/Family information<br />

Hospices are available to patients within the Calgary zone who require end of life care. People<br />

admitted to hospice have a life threatening illness where cure is no longer possible and goals of care<br />

are focused on quality of life, care and comfort.<br />

When being at home is no longer possible, or an acute care admission is no longer required, the<br />

specialized teams of health care professionals and volunteers at hospice provide 24-hour care and<br />

support for patients and families. The interdisciplinary team focuses on easing the physical, emotional<br />

and spiritual suffering that often accompany advanced, life-threatening illness. In the peaceful hospice<br />

environment, friends and families are encouraged to spend time with their loved one. Accommodations<br />

for family overnight stays are available if required.<br />

Alberta Health Services (AHS) – Calgary Zone provides hospice care through six contracted service<br />

providers and one AHS Hospice throughout the greater Calgary area. All hospices meet high<br />

standards and provide similar care. The priority is to ensure all patients have timely access to hospice<br />

care.<br />

An assessment by a member of the AHS Palliative Consult Team is required to determine<br />

eligibility for Hospice care<br />

Process:<br />

• From an acute care facility: The attending doctor or nursing staff can initiate a palliative care<br />

consult<br />

• From home: The family doctor can fill out a referral to general home care who can then initiate<br />

a referral to palliative home care or to a member of the palliative consult team<br />

• From Long Term Care & Supported Living: The attending doctor or nursing staff can request<br />

a palliative care consultation<br />

• Rural areas including hospitals and home care: The family doctor, attending doctor or<br />

nursing staff can initiate a referral to the rural palliative care consult team<br />
