below for further information in completing your response with regard to<br />

fee proposal and overall compensation. DO NOT APPROACH THE<br />

INSURANCE MARKET. At this time the Authority is only seeking a response<br />

based on your organization’s knowledge and experience. See Attachment<br />

A for current insurance placement and premium. Premiums shown per<br />

Attachment A do not include any commissions.<br />

Disclosure of Income: Describe the transparency policies and procedures<br />

your company has in place to ensure disclosure of ALL income, (direct,<br />

indirect, contingent, affiliated company, etc.) received by your company<br />

and/or affiliates in connection with the placement of the Authority’s<br />

property and casualty insurance program.<br />

Fee income: A “fee for service” compensation format to the awarded<br />

broker is preferred by the Authority. Please provide:<br />

1. An annual “set” consulting or brokerage fee for all services rendered;<br />

2. A detailed breakdown of all services included in the “set” fee; and<br />

3. The methodology used in determining the fee.<br />

Proposers should also include a detailed summary of any additional<br />

services including the price for all such services, (if any), rendered that<br />

would be considered outside of the fee proposal.<br />

Commission income: If commission compensation is to be included in<br />

whole or in part, describe the total package of services incorporated into<br />

any and all commission rates where applicable, (i.e. consulting, loss<br />

control, claims admin). Please provide a detailed summary of any<br />

additional services including the price for all such services rendered that<br />

would be considered outside of the commission rate.<br />

4.2.4 Proposal Life<br />

A statement must be included that indicates the length of time during<br />

which the Authority may rely on all proposal commitments. The Authority<br />

requires that this period of time not be less than 180 days from the due<br />

date for submission of proposals. Any proposal accepted by the Authority<br />

for the purpose of contract negotiations must remain as committed<br />

through the contract negotiation period.<br />

4.2.5 Signature of Authorized Representative<br />

A person authorized to commit the Respondent to its representations<br />

must sign the transmittal letter. Such person’s authority to so act must be<br />

consistent with the information contained in Section 4.3.9 of this RFP.<br />

4.2.6 Other Information<br />

4.3 BUSINESS PROPOSAL<br />

This item is optional. Any other information the Respondent may wish to<br />

briefly summarize will be acceptable.<br />

The Business Proposal must address the following topics except those specifically<br />

identified as “optional.”<br />
